The heart and abdomen experience twisting, severe pain, sometimes feeling as hard as a board, as if bound by a rope, as if tendons are being pulled, as if pierced by an awl, or as if cut by a knife; the pain is extreme and unbearable. In mild cases, there is also slight twisting pain with severe distension and stifling sensation, which can be cured by bloodletting (Fangsha). If it does not resolve after bloodletting, one must carefully examine the pulse and symptoms to determine the cause, distinguishing whether it is obstructed by summerheat and turbid qi, food stagnation, or phlegm and blood stasis, and then apply treatment. Several doses must be taken continuously until the condition slightly stabilizes before gradual regulation can begin. In this condition, many people in the world undergo bloodletting several times without success, resign themselves to fate, refuse to take medicine, and thus the sha toxin attacks and damages the zang-fu organs, leading only to death. What a pity!
Clinical Cases:
The wife of Mr. Lianqi Zhu suffered from critical sha-induced abdominal cramping in summer. Gua Sha and bloodletting failed to cure her. Three other physicians were too afraid to take responsibility, leaving the family helpless. Brother Wang Songchen invited me to examine her. The six pulses were faint and hidden. After treatment, she did not improve, and that night the cramping pain returned as before. The next morning, Brother Song Yifang, the son of the virtuous gentleman, invited me again. The right hand pulse was hidden. I performed bloodletting with thirty-two needles combined with Gua Sha. Then, I administered Baohua San (Precious Flower Powder) and Chenxiang Wan (Agarwood Pill), given with clear tea slightly cooled. Simultaneously, I prescribed agents for dispersing sha, detoxifying, invigorating blood, and regulating qi. Relatives still feared there was no hope for rescue and asked me to stay until after lunch. Tan Jun and Brother Xia Wang said, "She has fallen asleep, how is she?" I replied, "Sleeping indicates that the spirit has stabilized and qi and blood are harmonizing; she will likely rest peacefully." The following day, she recovered.
Mr. Jia Qing experienced diarrhea and abdominal pain resembling rope-like twisting. He invited me to treat him. The pulse was surging, rapid, and excess. Bloodletting failed to cure him. I used Wuyao Shunqi Tang (Aucklandia Decoction to Regulate Qi) with added Dahuang (Rhubarb Rhizome), which purged the stagnation and led to recovery.
The wife of my old friend Brother Zhulan An, who was Mother Gongfan, vomited phlegm and saliva and suffered from abdominal cramping. Medical treatment was heavy-handed, and she did not recover after six days. I was invited to diagnose her; the left pulse was faint and hidden. I said, "This is sha." I instructed her to undergo Gua Sha, which provided slight relief, but she refused medication. The next day, she became comatose with severe pain again. The whole family panicked, and relatives filled the house. I was invited again. I pricked one needle at the tip of the left middle finger to release toxic blood, and instructed her to undergo Gua Sha. It did not resolve. I used Jiangxiao Taohua San (Costus and Peach Blossom Powder) mixed with Amomum Villosum Soup, administered slightly cooled. Simultaneously, I used Fangfeng San Sha Tang (Saposhnikovia Decoction to Disperse Sha) added with Dougen (Millettiae Radix), Qiancao (Rubia Cordifolia Root), Danshen (Salvia Miltiorrhiza Root), Jinyinhua (Lonicerae Flos), Shanzha (Crataegi Fructus), and Bolu Zi (Brassicae Semens), administered slightly cooled, which brought peace.
Young Master Ya, son of Gentleman He, suffered from intestinal colic in the first lunar month. He invited me to treat him. The pulse was hidden. I instructed him to undergo Gua Sha and used Chenxiang Yujin San (Agarwood and Turmeric Powder) and Lengzhu Tang (Curcuma and Bitter Orange Decoction), administered chilled. He improved slightly. At dusk, the cramping pain returned severely, and he cried out incessantly. He invited me again. I used Xixin Dahuang Wan (Asarum and Rhubarb Pill), administered with clear tea slightly chilled. He then recovered using Zipu Tang (Magnolia Bark Decoction).
