Treating miasma disease. Coldness of the four limbs. Unceasing sweating on the head. The two feet feel as if they are absent, or occasional hiccups occur.
Chuanwu (Radix Aconiti Kusnezoffii), Dafuzi (Radix Aconiti Lateralis Preparata), Tianxiong (Radix Aconiti Lateralis) in equal parts. Decoct in water and take orally.
Miasma disease presenting with coldness of the four limbs and a sensation that the two feet are absent indicates extreme deficiency-cold of the Spleen and Stomach, with a complete lack of Yang Qi in the Middle and Lower Jiao. Unceasing profuse sweating on the head indicates that Yang Qi is floating entirely to the upper part and is about to collapse. Hiccups (E'ni, 呃逆) are commonly called hiccuping; medical texts describe "Jue" (collapse) as hoarse voice, absence of freezing sensation, resembling falling leaves when Yang Qi is about to depart. These syndromes are caused by physicians mistakenly using cooling and purging herbs, leading to critical conditions. This formula uses Chuanwu to dispel cold-dampness and attack cold-phlegm; Fuzi tonifies the deficient Yang of the Lower Jiao; Tianxiong tonifies the deficient Yang of the Upper Jiao. Used together, they collect Heart fluids and strengthen True Yang. When sweating stops, hiccups cease, and the four limbs become warm, recovery is possible. According to these two methods of warming the Middle and securing the Lower, it is essential for treating severe miasma disease. [Note: 'Di' here may be a textual error or specific context marker]. Obvious miasma diseases are clearly heat syndromes, while Yin-cold syndromes are hidden and difficult to see. Therefore, the method of using hot herbs to treat heat syndromes seems strange to those not versed in profound principles. However, one must adhere to the meaning of "treating cold with heat and heat with cold" from the *Neijing* (Inner Canon), as seen in the discussion on Fuzi.
Treating miasma disease characterized by excess in the upper and deficiency in the lower, aiming to regulate the ascending and descending of Yin and Yang, and to supplement and connect True Qi. Also treats dizziness due to deficiency-wind and incessant spitting of saliva.
Heixi (Lead Tin Alloy), Shuiyin (Mercurium), Liuhuang (Sulfur), Zhusha (Cinnabaris). (Each 1 liang, stir-fried and ground separately).
First, place clean iron into a crucible with lead. Melt first to remove impurities (1 liang). Clean again and put back into the crucible. Use an iron spoon to stir. Gradually add 1 qian of powdered Sulfur. If flames rise, it does not matter; just stir-fry quickly until the lead is cooked and loses its nature, and the Sulfur is burned away, leaving only cooked lead. Then pour onto the ground or paper to harden. Grind finely, sieve through gauze to obtain lead powder. The remaining red substance is stir-fried, ground, and sieved again. Next, place the crucible on a slow fire, melt the lead powder again, add 1 liang of Sulfur, and stir-fry frequently until yellow smoke rises. Immediately remove and place in a cool area. After a short while, return to the fire and stir-fry. The lead and Sulfur turn black and blend perfectly. Place in a cool area. When slightly cool, return to a mild fire. Add Mercurium after a short while, stirring with a spoon. Do not allow blue smoke to rise. Then add powdered Cinnabaris, stir-frying frequently until thoroughly blended. Pour onto the ground or paper to harden. Grind separately into powder. Make pills with rice paste, size of mung beans. Take 20 pills each time, delivered with salt water.
Miasma disease syndromes often present with heat in the upper and cold in the lower. This formula uses Liuhuang (Sulfur), a very hot herb, to supplement True Yang and strengthen True Fire. It is assisted by Zhusha (Cinnabaris), Heiqian (Black Lead), and Shuiyin (Mercurium), which are all extreme Yin substances with cold properties, thus removing severe rebellious cold and addressing latent Yang. Liuhuang is also called General; its function is to break evil and restore righteousness, eliminate stagnation and restore clarity, bring forth Yang essence, dissipate Yin and return Yang, transform Po and generate Hun. Therefore, it is used for miasma to guide Yang Qi from the Upper Jiao down to the Lower Jiao, causing Yin to ascend and Yang to descend, achieving the state of Water and Fire having both met (Shuihuo Jiji). It anchors Yang Qi, preventing it from floating upward and leaking outward, keeping Yuan Qi warm and constant. Those already suffering from miasma will recover; those not yet ill can avoid it.
