In postpartum women, blood loss from delivery leads to the depletion of Yin blood downwards, causing the Yang Qi of the whole body to wither and decline. The orifices become deficient, wind moves internally, and external pathogens invade simultaneously. The five evils (Wu Xie) intertwine and persist, giving rise to disease. Furthermore, daily affairs urge them on constantly; if eye diseases are treated lightly, it may lead to a precarious life.
If eye disease occurs before delivery, it is often complicated by the presence of the fetus. Its root lies in the obstruction of Yin and Yang Qi. One must fear that pathogenic factors are excessive while healthy Qi is insufficient; therefore, physicians must devise unique and careful treatment strategies.
This chapter generally discusses eye diseases occurring during pregnancy, at the time of delivery, and after delivery. Specifically, during late pregnancy approaching delivery, the vessels throughout the body boil and tremble, causing unbearable hardship. After delivery, both Qi and Blood are injured, leaving the body as empty as a valley. After the infant suckles milk, or if the mother engages in manual labor such as needlework, even with utmost care and nurturing, it is difficult to quickly restore one's innate constitution. At this time, all external pathogens can invade due to deficiency. Healthy Qi is weak, and pathogenic factors are strong, attacking from both inside and outside. The essence within the meridians gradually withers. Consequently, many people die from postpartum consumption and tuberculosis-like syndromes, let alone eye diseases. The *Classic of Poetry* states: "Alas, my parents, who bore me with toil." To repay their virtue is like the vast heaven, boundless. Ancient scholars referred to their birthdays as "Mother's Hardship Day," abstaining from meat and alcohol, deeply understanding the meaning of the poem. Treatment should focus on major tonification with mild harmonization. Formulas such as Ganren Yangrong Decoction (Ginseng Nourishing the Spirit Decoction), Renshen Bwei Tang (Ginseng Tonifying Stomach Decoction), or Airen Lixue Tang (Mugwort Regulating Blood Decoction) should be modified. It is absolutely imperative not to use cold, dispersing agents, nor to delay treatment, lest Qi become chaotic and Blood coagulate, causing the disease to penetrate deeper, making recovery difficult. Regarding treatment when the fetus is also involved, extreme caution is required. Since the middle Jiao is obstructed, Yin and Yang inevitably become separated, with Fire above and Water below; thus, not only do eye diseases occur, but the feet also swell. The symptoms in the eyes are often false excess, while the condition of the feet is true deficiency and cold. If one considers using clearing and draining methods, one must know the principle of "having the cause implies no damage" (treating the disease does not harm the healthy Qi). If one considers using warming and tonifying methods, it seems mismatched for the upper and lower symptoms. Frequently, incompetent physicians are caught off guard. In reality, tracing back to the source and exploring the root, using Baotai Liuqi Yin (Fetus-Securing Qi-Flowing Decoction) and Zhengqi Tianxiang Tang (Upright Qi Heavenly Fragrance Decoction) to protect internally and attack externally, employing methods of both benefit and reduction, will ultimately be effective. Alas, within moments, two lives are entrusted. Sincere emotion connects everywhere. Those who read this treatise without reflection, or who exploit such cases for profit, are treacherous and unprincipled; they must not be allowed to practice medicine.
