The opacity (obstruction) sometimes gathers and sometimes disperses; damp-heat stagnates in the brain, turbid like clouds covering the moon. When opening the eyes to view stars, they appear small. Upon onset, there is much pain and astringency; after it passes, vision remains poor. Its coming and going resembles traces of duckweed; treatment must not be hasty.
This condition resembles an opacity (nebula) but is not a true obstruction. It may be round or irregular. It appears when painful and hides when painless, appearing and disappearing without fixed timing, gathering and scattering inconsistently, hence named "Floating Duckweed" (Fuping). Generally, the original ailment was headache due to wind-phlegm. A quack doctor mismanaged it by exacerbating both deficiency and excess (deficiency made deficient, excess made excessive), causing damage to Yuan Qi (Primordial Qi), while the pathogenic factor remained. The mediocre physician knew only to tonify the deficiency but dared not treat the excess, thus allowing damp-heat to sink deeply into the Brain House (Naohu). When the annual climate is discordant or triggered by emotional/interpersonal stimuli, the disaster erupts. Those with gentle temperaments may suffer several episodes per season; those with irritable temperaments may suffer several times per month. Although it may heal spontaneously without medication, it ultimately becomes a chronic stubborn disease. Physicians should employ methods to thoroughly cleanse the pathology ("伐毛洗髓" - plucking hair and washing marrow, implying radical cure), flexibly adapting to human circumstances, so that the root of the disease is eradicated.
For the above four conditions, which have persisted for a long time, aggressive dispersal methods need not be discussed. Palpate the pulse, inquire about the immediate environment, preferences/dislikes, and urination/defecation. Use formulas such as Yueju Wan (Yueju Pill), Xiaoyao San (Free and Easy Wanderer Powder), Shugan San (Soothe Liver Powder), Juhua Chatiao San (Chrysanthemum Head-Tying Powder), Buyun Dan (Cloud-Piercing Elixir), Buxin Dan (Heart-Nourishing Elixir), and Huanshao Dan (Rejuvenating Elixir). Subsequently use Renshen Guben Tang (Ginseng Root-Fortifying Decoction), adjusting the dosages of Sheng Dihuang (Raw Rehmannia Root) and Shu Dihuang (Prepared Rehmannia Root) according to circumstances. Apply medicinal drops to remove opacities and use surgical incision to remove blood stasis. Although the primary approach follows that of Jiayan, one must strictly adhere to medical laws and ethics.
All cases of Black Opacity (Heiyi) have their origins. The fierce winds and thunder of Xun and Zhen are unbearable; do not blame Heaven for being stingy with your fate. The cause here is not trivial like a fly's head.
This condition begins with slight itching, progresses to astringency, and then becomes piercing pain. After a long time, redness, swelling, tearing, photophobia, and prolonged eye closure occur. An opacity floats up on the Wind Wheel (Cornea/Black Eye), black and round, varying in size and height, resembling a crab's eye, yet it is not caused by a rupture of the wheel. Furthermore, internal and external attacks combined constitute what is called "Crab Eye." It falls off unnoticed; after falling off, further management leaves no trace. Anger should not be vented, diet should be appropriate, and precautions should be taken regarding the aforementioned symptoms.
This condition is rare; I encountered only one poor boy in my life who was emaciated yet capable of labor. Suffering from this illness, he continued to gather firewood and sell grass, working hard as usual. Out of pity, I prescribed Sijunzi Tang (Four Gentlemen Decoction) augmented with Cornus officinalis (Shanzhuyu) and Coptidis Rhizoma (Huanglian) stir-fried with wine. The pain ceased, and he could open his eyes to see. Upon further administration, the opacity appeared scorched and shrank. I then removed Huanglian and added Paeoniae Radix Alba (Baishao), Ophiopogonis Radix (Maidong), and Arctii Fructus (Niubangzi). Before completing three doses, the eye returned to normal. I also gave him Sizhu Bingzi (Assisting Spleen Honey Cakes), four liang, resulting in complete recovery. However, this was merely a lucky coincidence. For robust men accustomed to rich diets, one must follow the treatment methods for Crab Eye before medication and before rupture.
