A pair of green and white eyes, yet no actual nail exists. Even a divine physician cannot extract it; madness and blindness may last a lifetime.
Upon the onset of this condition, the patient experiences fever and aversion to cold, with sudden and severe redness and swelling. The pain in the brow bone and temporal region exceeds normal limits, and a white nebula (corneal opacity) forms. If treatment is delayed, the nebula penetrates deeply inward like a nail. This results in turbidity of the cornea (Hun Jing) accompanied by obstruction, surrounded by red blood vessels, rendering day and night indistinguishable. Among the extremely malignant eye opacities, this is named "Ding" (Tubercle/Carbuncle). It is caused by the collapse of Qi and Blood, the sinking of Spring Yang within the Wood element (Liver), and the rising of Yin Wind. How can this be verified? Fever and aversion to cold, with sudden onset of redness and swelling—are these not signs of Qi and Blood collapse? The abrupt formation of a nebula that quickly invades inward—is this not due to the sinking of Spring Yang? Pain located in the brow bone and temporal region—is this not due to the rising of Yin Wind? Furthermore, when Yang sinks, Fire must become stagnant, hence the surrounding red blood vessels. When Yin rises, Cold prevails, leading to turbidity of the cornea and obstruction. If a person suffers from this disease, merely losing vision without the eyeball becoming dry and atrophic is considered a stroke of luck.
Nine out of ten cases are incurable. Even if the family firmly believes, one must clearly inform them of the prognosis, record the case for verification, to avoid future disputes. There are two methods for medication, requiring detailed pulse diagnosis: if the pulse is wiry, large, and rapid, one may boldly use strong purgatives and strongly cold-natured herbs. If the pulse is soft, small, deep, and choppy, one should first tonify Yuan Qi (Primal Qi); as Qi becomes robust, it will naturally push the pathogen out. As the Ding gradually shrinks, apply ointments or grinding medicines. Without the slightest error, the Ding will fall off without damaging the Feng Lun (Wind Wheel, referring to the cornea). Otherwise, the cornea will either become concave or convex. Students must listen respectfully and not neglect this.
The wife of Mr. Kong, on the fifth day, took her youngest son for a high vantage point view, and her left eye suddenly developed this condition. By the eleventh day, she invited me to treat her; she had already been blind for four days. She wore a head wrap and padded clothing, and her sleep and diet were severely neglected. Her pulse was floating, large, and slightly wiry, clearly indicating deficiency of Yuan Qi with wind invasion, where the fierce wind transformed into heat. I therefore used Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) combined with Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder), administered twice daily. The pain ceased, her spirit calmed, and she desired vegetable food. Subsequently, I prescribed Gui Pi Tang (Restore the Spleen Decoction) augmented with Fuzi (Prepared Aconite Root) and Fangfeng (Saposhnikovia Root). She removed her lined jacket, and her irritability vanished. Finally, all eye drops and internal medications proved effective. One day, the Ding suddenly dropped off, resembling a small orange seed; it could not be cut even with scissors, and her vision returned. Shortly after, she completely recovered. Some said: This woman raised an orphan and maintained chastity, supporting herself through labor, never harboring resentment. The efficacy of the medicine was likely aided by heavenly grace. Although virtuous and cautious mothers and teachers may receive heavenly protection, such cases might occur. But what about men with wolfish hearts and dog-like conduct, who live to old age with eyes as clear as children's, or who occasionally fall ill until death but recover rapidly upon my treatment? What explains those cases?
