If the pulse is floating and rapid, disperse [the pathogen]; if the pulse is deep and solid, purge [the interior]. If both exterior and interior are excess patterns, resolve the exterior and attack the interior. For lesions presenting with numbness and itching, or severe pain, or no pain at all, apply moxibustion, combined with detoxifying therapies.
A male patient suffered from a carbuncle on his foot, experiencing itching, nausea, vomiting, and occasional confusion. His pulse was floating and rapid. After applying direct moxibustion for more than twenty moxa cones (zhuang), he began to feel pain. He took one dose of Duoming Dan (Life-Restoring Pill), after which the swelling subsided/rised. He then applied Shenyi Gao (Miraculous Ointment) externally and took Jingfang Baidu San (Schizonepeta and Saposhnikovia Decoction) internally, resulting in recovery.
An elderly female patient suffered from this condition on her big toe, with severe pain. She was advised to undergo moxibustion but refused, instead exclusively taking detoxifying herbs. This led to deficiency of True Qi (Zheng Qi) and exacerbation of Evil Qi (Xie Qi), ultimately resulting in death.
Generally speaking, should one apply these methods indiscriminately? Moreover, since the lower limbs are located in the "Extreme Yin" region, medicinal efficacy is difficult to reach there. Relying solely on oral medication may result in slow action that fails to address the urgency; therefore, moxibustion is superior. Thus, sores on the lower body should generally be treated with Garlic-Separator Moxibustion (Gesan Jiu). If painful, moxa until the pain ceases; if not painful, moxa until pain arises. If there is still no sensation after moxibustion, apply direct moxibustion and prick the four sides of the carbuncle to release evil blood. Place one pill of Duoming Dan into the pore of the carbuncle head, and cover it with an ointment patch. If pricking yields no pain or no blood, heat the needle red... [Text fragment]. If red threads extend towards the heart and abdomen, prick the end of the thread to release evil blood, and take Jingfang Baidu San. If the red thread is close to the heart and abdomen, puncture the carbuncle head to drain evil fluids, and apply an ointment patch. For cases with numbness, take Duoming Dan. For trismus (lockjaw) or lesions in the throat, hold one or two pills in the mouth. Sudden death in humans is often caused by carbuncle toxicity. Use a lamp to inspect the whole body; if small sores are found, they are carbuncles. Apply urgent moxibustion. Once conscious, administer detoxifying herbs or Duoming Dan. Ingestion of contaminated food entering the flesh can generate carbuncles; caution is essential.

This medicine promotes the emergence of pus in cases where it does not rise... [Text fragment]. It possesses the power to restore life and is the supreme treasure among treatments for malignant syndromes.
