Breast milk is transformed from the Qi and Blood of the Chong (Penetrating Vessel) and Ren (Conception Vessel) meridians; therefore, when it flows downward, it becomes menstruation, and when it flows upward, it becomes breast milk. Postpartum diet should be light and bland, avoiding excessive saltiness, as salt can stop bleeding, reduce lactation, and induce coughing. In cases of scanty lactation due to Qi and Blood deficiency, whether caused by excessive blood loss during delivery, pre-existing illness before delivery, or poor postpartum care among impoverished women and servants leading to withered blood vessels, or in women around forty years old whose Qi and Blood are gradually declining resulting in no lactation, Tongmai Tang (Pulse-Connecting Decoction) should be taken urgently. For deficiency patterns, tonification is indicated. If lactation is impending but not yet smooth, Yongquan San (Springing Spring Powder) is suitable. For stagnation patterns, unblocking is indicated. For obese women suffering from lactation blockage due to Phlegm-Qi obstruction, Loulu Tang (Rhizoma Euphorbiae Humifusae Decoction) is appropriate. For obstruction patterns, promoting movement is indicated. Alternatively, eating porridge made with Adzuki Bean (Vigna angularis) can facilitate flow. If lactation is insufficient to nurse the infant, leading to emaciation of both mother and child and reduced appetite, Shenzhu Dihuang Tang (Ginseng, Atractylodes, and Rehmannia Decoction) is recommended.
Tongmai Tang (Pulse-Connecting Decoction)
Huangqi (Astragali Radix, raw, 1 liang), Danggui (Angelicae Sinensis Radix, 5 qian), Baizhi (Angelicae Dahuricae Radix, 1 qian), Tongcao (Tetrapanacis Medulla, 2 qian), Seven-star Pig Trotters (one pair).
Boil the ingredients into a decoction, skim off the floating oil, and take the herbal medicine. After ingestion, sleep with the face covered to induce lactation. If ineffective, take another dose.
For immediate postpartum women with no milk, decoct with half water and half wine. Do not use pig trotters; for young, vigorous individuals with robust constitutions, add Honghua (Carthami Flos, 3 fen) to eliminate lochia.
