Zhu Quansheng stated: When a woman's menstruation is irregular, various diseases will arise. Even if the treatment is appropriate, it is difficult to achieve quick results. Therefore, for any woman with irregular menstruation, except for acute conditions caused by external contraction of seasonal pathogens (Wai Gan Shi Qi), regardless of what other disease she may have, one must first examine its nature of cold, heat, deficiency, or excess, and regulate the menstruation first. Once the menstruation is regulated, the remaining diseases can be cured with slight clearing, resolving, and regulating. Thus, in gynecology, regulating menstruation is the primary principle. (Commentary: Acute conditions from external contraction of seasonal pathogens cannot be delayed for a moment, so they are discussed separately. Other diseases can also be treated concurrently while regulating menstruation. The first two items represent standard treatment for standard conditions, while the following represent variable situations.)
Menstruation arriving early (Jing Shui Xian Qi), with dark purple color and heavy flow, indicates that both water and fire in the kidney are excessively vigorous, which is a condition of excess. However, in women of prime age, the water is normal true water, while the fire is pathogenic fire. Pathogenic fire should not be allowed to remain excessive, nor should true water be depleted. Therefore, treatment involves only slightly clearing the heat without draining the kidney water.
Mr. Fu Qingzhu's Clearing Menstruation Powder (Qing Jing San)
* *Paeoniae Radix Rubra* (Dan Pi)
* *Paeoniae Radix Alba* (Bai Shao), stir-fried with wine
* *Lycii Cortex* (Di Gu Pi)
* *Rehmanniae Radix Praeparata* (Da Shu Di)
* *Artemisiae Annuae Herba* (Qing Hao)
* *Phellodendri Cortex* (Huang Bo), stir-fried with salt water
* *Poria* (Bai Fu Ling)
Poria and Paeonia in this formula are contraindicated during menstruation and postpartum periods, as their use often leads to complications such as metrorrhagia (Beng Lin), strangury, and turbid stagnation; it is suitable to take this formula after menstruation has ceased. For those with excessive sweating, remove *Paeoniae Radix Rubra*. For bone-steaming fever (Gu Zheng), substitute *Rehmanniae Radix Praeparata* with *Rehmanniae Radix Recens* (Sheng Di Huang). For those with unstable kidney essence (Shen Yuan Bu Gu), add *Rosa Laevigata Fructus* (Jin Ying Zi) and *Corni Fructus* (Shan Yu Rou). (Commentary: Famous formulas in books merely provide a general framework. Individuals differ in constitution, emotions, concurrent symptoms, and daily routines; thus, herbs may not suit everyone equally, and dosages are omitted here. Detailed discussions are provided in the formula commentary only when specific herbs are strictly necessary.)
Menstruation arriving early, with dark purple color and scanty flow, or even forming clots, indicates that heat has injured the yin aspect, resulting in excessive fire and insufficient water. Insufficient water causes fire to become floating yang; it should not be drained, as draining it would weaken the ministerial fire (Ming Huo). Therefore, treatment should tonify true water without damaging true yang. (Commentary: This syndrome is common in emaciated women who tend to be light-tempered and irritable; medications should be soft and moistening.)
Fu's Two Places Decoction (Liang Di Tang) (Referring to Mr. Qingzhu, hereinafter referred to as Fu)
* *Rehmanniae Radix Recens* (Sheng Di Huang), stir-fried with wine
* *Lycii Cortex* (Di Gu Pi)
* *Scrophulariae Radix* (Yuan Shen)
* *Paeoniae Radix Alba* (Bai Shao Yao), stir-fried with wine
* *Ophiopogonis Tuber* (Mai Dong Rou)
* *Colla Corii Asini* (E Jiao)
This formula is very appropriate. Since depletion of water inevitably leads to bone-steaming fever, this formula can address both. In cases of yin deficiency with hyperactive fire, liver yang must necessarily rebel and cause much stagnation; add *Haliotidis Concha* (Shi Jue Ming) to subdue rebellion and add *Fritillariae Thunbergii Bulbus* (Xiang Bei Mu) to relieve stagnation. If these syndromes are absent, do not affect the upper and middle jiaos. (Commentary: "These syndromes" refers to the "upper and middle jiaos" mentioned below.)
