The Yellow Emperor asked: When a woman is pregnant and becomes aphonic (loss of voice) in the ninth month, what is the cause? Qibo replied: This is due to the obstruction of the collateral vessels of the uterus. These collaterals are connected to the Kidney, and the Shaoyin (Lesser Yin) channel passes through the Kidney and connects to the root of the tongue; therefore, speech is impaired. It will recover after the tenth month. (After delivery in the tenth month, the uterine collaterals resume their flow, and without treatment, voice naturally returns.) He also said: If a pregnant woman has an illness requiring medication, how should it be handled? Qibo replied: "If there is a pathogen, there will be no destruction [of the fetus or mother]; if there is no pathogen, there is no destruction." One should only reduce the pathogenic factor by more than half. Generally, when a pregnant woman is ill, medication cannot be avoided. The physician must carefully assess the severity of the disease and weigh the potency of the herbs, prescribing moderate doses without using too many ingredients. Once the disease process has subsided, medication should be stopped. Thus, the disease is eliminated, the mother remains safe, and the fetus is unharmed; why fear such aggressive treatments?
Regarding the initial stages of conception, ancient texts state: The first month is called "Beginning of Embryo," the second month "Beginning of Paste," and the third month "Beginning of Germ." At this stage, blood does not circulate freely as usual; the image of the embryo begins to transform but lacks a fixed form, changing according to what the mother sees. The mother should associate with virtuous and noble people. To conceive a male, one may handle bows and arrows, play with beautiful jade, or observe peacocks. In the fourth month, the essence of Water is received to form blood vessels. In the fifth month, the essence of Fire is received to form Qi. In the sixth month, the essence of Metal is received to form sinews. In the seventh month, the essence of Wood is received to form bones. In the eighth month, the essence of Earth is received to form skin. In the ninth month, the essence of Stone is received to form hair and body coverings. By the tenth month, the Zang-Fu organs, joints, and spirit are all complete. Ancient methods of prenatal education included sleeping on one's side, sitting upright, standing straight, avoiding strange tastes, maintaining dignified movements, keeping emotions peaceful and joyful, residing in quiet rooms, listening to good words, hearing poetry and books recited, discussing rituals and music, avoiding obscene sounds, and refraining from viewing ugly sights. Doing so results in a child with correct features, superior talent, loyalty, filial piety, virtue, intelligence, and wisdom. This is known as internal感应 (resonance/induction) caused by external images. It is most important to keep the bedroom clean so the infant is not exposed to the toxicity of licentious individuals, avoiding dangerous pox and rashes, and preventing miscarriage or difficult labor. If the mother is startled, the fetus may suffer from fetal fright syndromes; if the mother consumes hot-toxic foods, the fetus may develop fetal heat sores; if the mother contracts wind-cold, the fetus may suffer from fetal cold leading to vomiting and diarrhea. All ailments in the newborn's first month stem from maternal imbalance during pregnancy. If the pregnant woman experiences anger, excessive joy, sorrow, or grief, or contracts Wind-Cold, Summer-Heat, or Dampness, or bathes frequently, handles cold water, overexerts herself, or moves lightly/improperly, she may experience discomfort after conception, including sore back and abdomen, bearing-down sensations, fever, fatigue, poor appetite, nausea, chest fullness, and weakness. These are caused by deficiency of both Qi and Blood failing to nourish the body. Some women continue to menstruate during pregnancy, termed "Prosperous Menstruation" (Sheng Tai), due to abundant blood. Some do not deliver for a month past term, or even at twelve to thirteen months up to twenty-four to thirty months, due to insufficient Qi and Blood; they should take strong tonics for Qi and Blood. Noblewomen often deliver in the eleventh or twelfth month.
