The manifestation presents as a lump on the arm resembling the size of a duck egg, all caused by disharmony between Ying (Nutrient) and Wei (Defensive) Qi. Immediate resolution is required; otherwise, the upper and lower tendons and bones will interconnect, leading to inconvenience in movement, heaviness and pain when the hand hangs down, extreme pain penetrating to the bone, or potentially causing tendon damage and fist clenching. If it occurs on the inner aspect of the arm, it is named "Fish Belly" (Yudu). It is appropriate to use Huoming Yin (Life-Saving Decoction) added with Qianghuo (Notopterygii Rhizoma et Radix), Duhuo (Angelicae Pubescentis Radix), Guizhi (Cinnamomi Ramulus), and Jiegeng (Platycodonis Radix), decocted with half water and half wine. Alternatively, Dueming Dan (Life-Rescuing Pill) or Zijin Ding (Purple Gold Pellet) can be used concurrently; for robust constitutions, purgative methods may be applied. If there is diffuse swelling without a head, and poison-resolving medicines fail to dissipate it, Shiquan Dabu Tang (Complete Great Tonifying Decoction) added with Guizhi and Jiegeng should be used to expel toxins outward. If an abscess arises inside the elbow, it belongs to the Three Yin Meridians, resulting from stagnant fire in the Pericardium Collateral; the guiding herbs are Huanglian (Coptidis Rhizoma), Shengma (Cimicifugae Rhizoma), and Chaihu (Bupleuri Radix). If an abscess arises outside the elbow, it belongs to the Three Yang Meridians, resulting from accumulated toxicity in the Large Intestine and Small Intestine; the guiding herbs are Ben (Ligustici Rhizoma, here referring to Gaoben), Shengma, and Chaihu. Both conditions should be treated with formulas such as Huanglian Xiaodu San (Coptis Poison-Resolving Powder) and Huoming Yin; subsequently, tonifying and supporting medicines should be used. Another presentation involves both the interior and exterior of the arm being red, with only the elbow joint slightly smaller, commonly known as "Lotus Root Package." Differentiated by interior/exterior corresponding to Yin/Yang, the treatment is the same as above.
Main Formula: Chishao (Paeoniae Radix Rubra), Qianghuo, Fangfeng (Saposhnikoviae Radix), Jinyinhua (Lonicerae Japonicae Flos), Duhuo, Jiegeng, Guizhi. Decoct with water and wine. Follow with Baizhi Shengma Tang (Angelica Dahurica and Cimicifuga Decoction) (Formula seen in General Section).
Youni Gao (Oil Mud Ointment): Treats Lotus Root Package.
Tongyou (Verniciae Oleum), Yanni Xin (Salt-mud wrapped Realgar, 1 liang, mixed evenly). Apply frequently with goose feathers.
The condition arises from long-term consumption of rich and greasy foods and prolonged emotional depression, leading to Spleen deficiency and Qi stagnation. It manifests in the abdomen, located between the skin and fascia, with severe abdominal swelling and pain, prone to ulceration and putrefaction. Initially, Garlic-separated Moxibustion is suitable, along with internal administration of Huoming Yin, followed by supporting medicines. If there is redness and swelling without pus, Tuoli Xiaodu San (Supporting and Poison-Resolving Powder) should be used to accelerate ulceration; if it does not ulcerate, Daizhen Wan (Needle-Replacing Pill) or acupuncture/lancing should be used. If there is diffuse swelling that is hard, without redness, Tuoli Shixuan San (Supporting Ten-Accumulations Powder) should be used, followed by the main formula. Avoid harsh purgatives and cooling medicines. After ulceration, Shiquan Dabu Tang (Complete Great Tonifying Decoction) must be used, doubling the dosage of Renshen (Ginseng Radix et Rhizoma) and Huangqi (Astragali Radix), adding Ganjiang (Zingiberis Rhizoma) and Fuzi (Aconiti Lateralis Radix Praeparata) to tonify and support.
Main Formula: Renshen, Baizhu (Atractylodis Macrocephalae Rhizoma), Fuling (Poria), Baizhi, Zhike (Aurantii Fructus), Danggui (Angelicae Sinensis Radix), Chuanxiong (Chuanxiong Rhizoma), Huangqi (each equal parts); Gancao (Licorice Root, roasted, half dose).
Decoct with water and take orally.
