Pain throughout the whole body occurs in conditions such as Shanghan (Cold Damage), Shangshu (Summerheat Injury), Shangshi (Dampness Injury), Huoluan (Colic with Vomiting and Diarrhea), Yindu (Yin Toxin), and all disorders involving Cold-Damp, Damp-Heat, Internal Injury, alternating Chills and Fever, and Disharmony of Qi, Blood, and Channels. For instance, in Shanghan with fever, body pain, and stiffness, accompanied by a Floating-Tight pulse, Ma Huang Tang (Ephedra Decoction) or Jiu Wei Qiang Huo Tang (Nine-Ingredient Notopterygium Decoction) may be used. If body pain persists after sweating, with a Deep-Slow pulse, Gui Zhi Jia Ren Shen Tang (Cinnamon Twig Decoction with Added Ginseng) is indicated. In cases of Summerheat injuring Qi, presenting with spontaneous sweating, body pain, mental fatigue, and a Deficient pulse, Qing Shu Yi Qi Tang (Clear Summerheat and Benefit Qi Decoction) is appropriate. For Dampness causing body pain, heaviness preventing turning over, and a Fine-Slow pulse, Chu Shi Tang (Dampness-Expelling Decoction) is used if the pathogen is in the Exterior, while Wu Ling San (Five-Ingredient Poria Powder) is used if it is in the Interior. In Huoluan with vomiting, diarrhea, body pain, thirst, scanty urine, and a Hidden pulse, Wu Ling San is indicated. In Yindu, body pain feels like being beaten with a club, with a blueish complexion, sore throat, and a Deep-Fine-Rapid pulse, Sheng Ma Bie Jia Tang (Marsh Swallow Turtle Shell Decoction) is prescribed. When Cold and Damp contend, with only head sweating, back stiffness, body pain, and a Deep-Choppy pulse, Gan Cao Fu Zi Tang (Licorice and Aconite Decoction) is used. When Wind and Damp contend, causing pain throughout the body with a Vacuous-Floating-Choppy pulse, Chu Shi Juan Bi Tang (Dampness-Expelling and Bi-Syndrome-Relieving Decoction) is indicated. When Damp-Heat contends, causing vexing pain throughout the body with a Slippery-Rapid pulse, Dang Gui Nian Tong Tang (Angelica Ligustaci Pain-Relieving Decoction) is used. In Internal Injury from overwork, combined with Wind-Damp causing body pain, Bu Zhong Yi Qi Tang (Tonify the Middle and Benefit Qi Decoction) with added Qiang Huo (Notopterygii Rhizoma et Radix) and Fang Feng (Saposhnikoviae Radix) is employed. For alternating chills and fever with body pain and chest/hypochondriac discomfort due to Liver Blood Deficiency with Fire Stagnation, Jia Wei Xiao Yao San (Modified Free Wanderer Powder) is indicated. For wandering pain throughout the body or channel traction pain, regulating Qi and invigorating Blood is the method; San Bi Tang (Three-Bi Decoction) is used. Generally, joint Bi pain belongs to Fire, body heaviness to Damp, stiffness to Cold, swelling to Damp, wandering nature to Wind, fixed cold-like pain to Phlegm, and lower body pain with scanty urine requires separating and draining dampness via Wu Ling San. For lower body swelling and pain with a Floating pulse, spontaneous sweating, and aversion to wind, expelling Damp and consolidating the Exterior is suitable, using Fang Feng Huang Qi Tang (Saposhnikovia and Astragalus Decoction). One must especially examine accompanying symptoms for precise treatment.
In Shanghan, if all six pulses are Tight, it indicates a Taiyang Exterior Syndrome. Body pain like being beaten with a club, with a Deep-Tight pulse, indicates Yin Toxin. After sweating, if the pulse is Wiry-Slow and there is body pain, it indicates Disharmony of Qi and Blood. Pain throughout all joints with a Deep-Wiry pulse indicates Middle Dampness. Heavy painful limbs, slight swelling, sweating, aversion to wind, joint stiffness, inability to turn, and a Slow pulse indicate Wind-Damp. Pain throughout the body with a Wiry-Small or Slippery-Large pulse indicates Deficiency and Damage of Qi and Blood.
