Lu Xiaoyu treated the daughter of Chen Zhenyu, aged twenty-seven. She suffered from intermittent malaria (malaria occurring every other day) postpartum for over a month. Although the chills and fever were not severe, she experienced physical fatigue and reduced appetite. Pulse diagnosis revealed a balanced pulse on the left hand, while the right hand showed a weak and powerless pulse. Two doses of Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) were prescribed. The patient felt fullness and oppression in the chest and diaphragm, attributing this to Ren Shen (Radix Ginseng). A subsequent physician continued using Qingpi Yin (Green Peel Infusion) and Erchen Tang (Two-Cured Decoction), but the chills and fever worsened. Antimalarial drugs were used intermittently or stopped, yet symptoms recurred after a few days. Months later, the patient became emaciated with prominent bones. Upon re-examination, the pulse was extremely faint. Lu stated, "A dose of one liang of Ren Shen would have sufficed previously; now, it must be increased to one jin to take effect." Ten Complete Great Tonifying Decoction (Shiquan Dabu Tang) was administered for two doses, but the patient still felt chesty oppression. Suspecting the treatment, Lu advised, "Continue taking it until the bloating and oppression cease." The dosage of Ren Shen was doubled, leading to improved appetite. After dozens of doses, the patient recovered.
Xue Lizhai treated a postpartum woman who developed abdominal pain and a bluish-yellow complexion after eating eggs. After taking Pingwei San (Calm the Stomach Powder) and Erchen Tang, she developed dysentery and abdominal distension. Using Liqi Yinzi (Flowing Qi Infusion) caused a mass in the lower abdomen to attack upwards periodically, further reducing her appetite. This was attributed to deficiency-cold of the Spleen and Stomach, with Liver Wood (the functional system of the Liver in Five Element theory) overacting upon them. Bu Zhong Yi Qi Tang was modified with Mu Xiang (Radix Aucklandiae) and Wu Zhu Yu (Fructus Evodiae), leading to gradual recovery. Subsequently, Bazhen Tang (Eight-Treasure Decoction) was used for conditioning, resulting in stability. (Note: Since the mass is attacking upwards, regardless of excess or deficiency,昇麻 (Rhizoma Cimicifugae) and 柴胡 (Radix Bupleuri) should certainly not be administered again.)
Gong Zicai treated a postpartum woman suffering from bloody dysentery, urinary retention, and umbilical-abdominal pain. He crushed fresh Machixian (Herba Portulacae), extracted three he of juice, boiled it, added one he of honey, mixed well, and administered it in one dose, resulting in immediate cure.
Xue Lizhai treated a postpartum woman who passed blood after defecation. Various medications failed. She had little desire for food and felt limb fatigue. This was diagnosed as deficiency of Middle Qi. Bu Zhong Yi Qi Tang was modified with Wu Zhu Yu and five fen of stir-fried Huang Lian (Rhizoma Coptidis). After four doses, bleeding stopped. However, she still suffered from palpitations, insomnia, and night sweats. Guipi Tang (Restore the Spleen Decoction) was prescribed, leading to recovery.
Sun Wen Yuan treated his clan's daughter, who had undergone an abortion twenty days prior. She suffered from red dysentery (dysentery with blood), occurring more than ten times a day, accompanied by aversion to cold, anorexia, and lower abdominal distending pain. Pulse diagnosis showed a slippery and large pulse at the Right Cun position, indicating heat within deficiency. This was due to incomplete expulsion of lochia (postpartum residual blood and fluid), causing lower abdominal pain. Specialists adhered rigidly to Danxi's (Zhu Zhenheng) principle of greatly tonifying Qi and Blood postpartum, applying tonification indiscriminately, which induced dysentery. Her father asked, "If she is averse to cold, how can there be heat?" Sun replied, "Aversion to cold does not necessarily mean cold syndrome; it can be a heat syndrome, as extreme heat mimics water (cold signs). (Most contemporary physicians fail to understand this principle.) You will know after taking the medicine." The prescription used Bai Shao (Radix Paeoniae Alba), Dang Gui (Radix Angelicae Sinensis), and Hua Shi (Talcum) as Sovereign herbs; Tao Ren (Semen Persicae), Jiu Huang Lian (Wine-processed Rhizoma Coptidis), and Jiu Huang Qin (Radix Scutellariae processed with wine) as Ministers; Mu Xiang (Radix Aucklandiae), Gui Zhi (Ramulus Cinnamomi), and Bing Lang (Areca Seed) as Assistants; and Qing Pi (Pericarpium Citri Reticulatae Viride) as Envoy. After administration, a large amount of dark stasis blood was expelled, and the lower abdomen relaxed immediately. Symptoms remaining included dry mouth, scanty urine, nausea, fear of food, fatigue, and tenesmus (straining during defecation). The next prescription contained one qian each of Ren Shen (Radix Ginseng), Chuan Xiong (Rhizoma Chuanxiong), and Bai Shao; one qian five fen of Dang Gui; six fen each of Jiu Huang Lian and Chen Pi (Pericarpium Citri Reticulatae); and two fen of Mu Xiang. Additionally, Qingning Wan (Clearing Peace Pill) was given externally/orally. After taking it, heat cleared, and dysentery decreased by eighty percent. However, stools remained loose, with nausea and weakness. Si Jun Zi Tang (Four Gentlemen Decoction) was modified with Jiu Bai Shao, Chen Pi, Mu Xiang, Rou Guo (Nucis Arecae Semen Preparatum), Jiu Huang Lian, and Dang Gui for nourishment, leading to stabilization.
Lu Yang Yu treated Li Zheng, son of Li Shangtian, who suffered from postpartum dysentery for over a year. His limbs and muscles were emaciated, and his complexion was dark black. Everyone considered it incurable. Pulse diagnosis showed minute pulses on both hands, but between the Right Guan and Chi positions, the pulse felt forceful like a bead. Black coating was often present in the center of the tongue. Lu stated, "The minute pulse is a result of chronic dysentery. A slippery pulse with black coating indicates accumulation in the intestines that has persisted for a long time. If vigorously purged, the disease should be cured." Li Zheng argued, "I frequently took purgatives when the illness began. I haven't eaten for months; how could there be accumulation?" Reviewing previous prescriptions, they mostly focused on greatly tonifying Qi and Blood postpartum. Even when using digestive/promoting agents, they were mixed with Ren Shen, Huang Qi (Radix Astragali), Dang Gui, and Bai Shao. Thus, tonification failed to work, and digestion failed to clear, leading to daily decline of Yuan Qi and solidification of stagnation. Reaching the stage of astringency and warming, worsening was inevitable. Run Zi Wan (Moisten Character Pill) was prescribed, one liang divided into three doses, to be taken within one day and night. Several jars of purple-black paste-like stool were discharged. Severe thirst ensued. Sheng Mai San (Generate the Pulse Powder) was decocted and drunk as tea. Stomach function gradually opened. Run Zi Wan, one qian daily, was continued, discharging about one jar of thick accumulation daily. After ten days, tonifying treatments were started, and recovery occurred within one month.
Zhang Lu Yu examined a woman on a military vessel. She had suffered from dysentery before pregnancy, which persisted for three days postpartum. Lochia did not flow, accompanied by fever, wheezing, and distension. By standard protocols, this was untreatable. After one dose of medication, hiccups worsened. Pulse diagnosis showed three beats per respiration with Dai pulse (intermittent pulse with regular pauses, indicating衰竭 of organ qi). Zhang intended to refuse treatment. However, since the previous physician was detained, refusing might cause significant loss of face. Zhang said, "Although critical, there is a sliver of hope. We must plan carefully to attempt treatment." Hearing this, the family released the doctor to seek medicine. Zhang gave a cup and a few coins, instructing them to buy sugar, roast it until charred, mix with plain water, and administer. This could treat dysentery, expel stasis, and not damage Yuan Qi. The medicine was administered urgently. The family left happily, and the doctor escaped, returning only after the troops departed. (Note: His conscientiousness is admirable.)
Xue Lizhai treated a postpartum woman with dysentery, less than a month postpartum. Due to consuming cold food and alcohol, cold and heat attacked the blood, resulting in pure bloody dysentery with severe dragging pain. The pulse was large and powerless, with dry mouth. Huang Qin Shao Yao Tang (Scutellaria and Peony Decoction) was prescribed. After three doses, she stabilized.
