Wan Mizhai's nephew, aged seven, suffered from chronic dysentery that would not cease. Wan prepared a pill formulation for him. Pills act slowly, suitable for treating chronic diseases. The main prescription was Qian Shi Yi Gong San (Dr. Qian's Extra-ordinary Efficacy Powder) combined with Xiang Lian Wan (Cinnamon and Coptis Pill), supplemented with Zhu Ling (Polyporus), Ze Xie (Alisma), and Che Qian Zi (Plantago Seed) to promote urination; Shen Qu (Medicated Leaven) and Mai Ya (Barley Sprout) to resolve food stagnation; He Zi (Chebula Fruit), Rou Dou Kou (Nutmeg), and Chao Gan Jiang (Fried Dried Ginger) to stop dysentery. This combination was named He Zhong Wan (Middle-Harmonizing Pill). Approximately two taels were prepared. Before the entire dose was consumed, the dysentery ceased. This is a family secret formula for treating persistent chronic dysentery.
Mr. Wang, aged sixty, had a son who contracted dysentery at age three. A physician administered excessive purgatives, damaging the Spleen and Stomach, causing Middle Qi to sink. Another physician used Rou Dou Kou (Nutmeg), Mu Xiang (Aucklandia), Huang Lian (Coptis), and Ying Su Ke (Poppy Shell) to arrest the dysentery, but the condition worsened, presenting with tenesmus and scanty stool. Wan examined the child and said: "This is a late-born child with weak congenital constitution. Excessive purging has caused Qi sinking; the treatment should be to lift the Qi. Moreover, the previous medications have not cleared, and astringent agents that forcibly bind are contraindicated." He prescribed Qian Shi Yi Gong San (Dr. Qian's Extra-ordinary Efficacy Powder) added with Mu Xiang (Aucklandia), Huang Lian (Coptis), Dang Gui (Angelica Sinensis), Bai Shao (White Peony Root), Shan Yao (Dioscorea), Lian Rou (Lotus Seed Embryo), and Shen Qu (Medicated Leaven), made into paste pills. After ten days, the dysentery stopped. However, Yuan Qi (Original Qi) had not yet recovered, so he advised continuing with the same formula for regulation. Someone mentioned: "I have Asafoetida (A Wei), which is very effective for dysentery." It was made into five pills of 5 fen each for the child to take. Others thought it inadvisable. Mr. Wang said: "He took one pill this morning, and another after drinking water; he has fallen into a deep sleep since taking them." Wan said: "The dysentery has stopped; why continue medication? This drug is too harsh and may damage Yuan Qi (Original Qi); this may not be normal sleep." Upon calling out, there was no response; pushing yielded no reaction. Entering the room, they found the eyes rolled back, and breath had ceased. This case is recorded in detail as a warning against reckless medication.
Nie Jiuwu's son, just over one year old, developed accumulation-dysentery due to insufficient breast milk, food stagnation from porridge/rice, and excessive consumption of wheat products. Initially watery diarrhea, progressing to pus and blood. Weaning was in progress, food intake was low, eyes remained open during sleep, anus was flaccid like a bamboo tube, and the fingerprint extended beyond the Life Gate (Ming Guan), indicating critical severity. Clearing heat and resolving accumulation formulas were administered slowly via a tea spoon. When mental spirit appeared extremely exhausted, Ren Shen (Ginseng) and Mai Dong (Ophiopogon Decoction) were given in small amounts to preserve Yuan Qi (Original Qi). After several days, dysentery gradually ceased. However, the flesh became emaciated; it took half a year of nourishment to recover.
Wan Mizhai treated the daughter of Governor Sun, who contracted dysentery in May and had not recovered by July. Upon Wan's arrival, the condition was critical. He prescribed Ren Shen (Ginseng), Fu Ling (Poria), Gan Cao (Licorice), Dang Gui (Angelica Sinensis), Bai Shao (White Peony Root), Huang Qin (Scutellaria), Che Qian Zi (Plantago Seed), and Chen Pi (Tangerine Peel) in equal parts, with a small amount of Chao Gan Jiang (Fried Dried Ginger). Decocted and taken, slight improvement occurred; after five days, significant peace was restored. Governor Sun asked: "All physicians use Mu Xiang (Aucklandia) and Huang Lian (Coptis); you use none of these, nor typical dysentery drugs, yet efficacy is achieved. Why?" Wan replied: "This is Liu Hejian's Huang Qin Shao Yao Tang (Scutellaria and White Peony Decoction). It regulates Qi to relieve tenesmus and nourishes Blood to stop dysentery."
The Daoist Zhu's son, aged seven, suffered from dysentery for half a year without cure. Wan prescribed a formula containing Ren Shen (Ginseng), Bai Zhu (Atractylodes Macrocephala), Fu Ling (Poria), Gan Cao (Licorice), Huang Qi (Astragalus), Jie Geng (Platycodon), Mu Xiang (Aucklandia), Huang Lian (Coptis), He Zi (Chebula Fruit), Rou Dou Kou (Nutmeg), Che Qian Zi (Plantago Seed), Chao Gan Jiang (Fried Dried Ginger), Ze Xie (Alisma), Shen Qu (Medicated Leaven), Dang Gui (Angelica Sinensis), Mai Ya (Barley Sprout), and Bai Shao (White Peony Root). Pulverized, formed into water pills, and taken with rice drink. Recovery occurred within one month. Named He Zhong Wan (Middle-Harmonizing Pill).
A ten-year-old girl suffered from persistent dysentery with a洪数 (Surging-Rapid) pulse. Some said: "In dysentery, the pulse should be small; a Surging-Rapid pulse suggests difficult treatment." Wan said: "No harm. The *Yu Han Jing* states: 'To distinguish boys and girls, decide by the Cun, Guan, and Chi positions; naturally tight and rapid is clear, because essence and Qi are not yet lost.' This girl's pulse indicates strong Stomach Qi; she will heal soon." Indeed, dysentery ceased after several days.
The daughter of County Magistrate Zhang, aged six months, suffered severely from red-and-white dysentery. Wan used Huang Lian (Coptis) 1 qian, Mu Xiang (Aucklandia) 5 fen, Shi Lian Rou (Stone Lotus Seed) 5 fen, Chen Pi (Tangerine Peel) 7 fen, and Chao Gan Jiang (Fried Dried Ginger) 2 fen. Pulverized, formed into mustard-seed-sized pills with Shen Qu (Medicated Leaven), and taken with aged rice soup. She recovered.
Zhang Jingyue treated the eldest son of Commander Qian Xuyang, aged two years. In mid-summer, raw fruit damaged the Spleen, causing initial diarrhea then dysentery. Knowing his own illness was due to cold/raw foods, he self-prescribed Ren Shen (Ginseng), Bai Zhu (Atractylodes Macrocephala), Gan Jiang (Dried Ginger), and Gui Zhi (Cinnamon Twig) to warm the Spleen, but it failed. Sores appeared on the lips. He consulted Zhang: "Clearly injured by cold/raw foods, why not use warming drugs?" Zhang said: "Diarrhea has injured Yin; sudden introduction of pungent-hot herbs causes Deficient Fire to flare upward. Only Fuzi (Aconite) can guide fire back to its source." Two doses were given, resulting in doubled pain from lip/oral sores and severe throat swelling. (Why not administer hot medicine cold, or add human urine and pig bile?) Symptoms manifested on the head and face, worsening. Re-examining: "Medication fails so drastically; could it truly be dampness generating heat?" Zhang diagnosed: "Upper pulses and lower discharges are not true heat; originally Yin deficiency. Heat therapy failed, raising doubt, but investigating the outcome, perhaps the dosage was insufficient?" Qian said: "There is another sign: extreme craving for hot drinks, swallowing what others cannot bear despite throat swelling. This verifies True Cold." Zhang agreed: "Precisely." Added Fuzi (Aconite) 1.5 qian, plus Gan Jiang (Dried Ginger), Rou Gui (Cinnamon Bark), Rou Dou Kou (Nutmeg), Ren Shen (Ginseng), and Shu Di Huang (Prepared Rehmannia). Diarrhea ceased, and throat/oral symptoms resolved in days. Such ambiguity requires utmost caution. (Ye Tianshi noted: "For Spleen/Stomach disorders, tonifying Kidney is inappropriate; Kidney herbs are rich and cloying, hindering movement, especially in vomiting/diarrhea cases. Zhang Jingyue often uses Dang Gui (Angelica) and Shu Di (Rehmannia) for Spleen issues, claiming miraculous results, but this is unreliable.")
Yu Jiayan treated a young Ye boy with dysentery, presenting as Jin Kou Li (Jin Kou Dysentery/Anorexic Dysentery) with fever, continuous vomiting and hiccups. Guan pulse surged upward without root. Foot pulses also surged without root. Yu stated: "This is Jin Kou Dysentery, a sign of impending Stomach Qi exhaustion. Jin Kou Dysentery involves Deficient Heat obstructing the Stomach, preventing appetite. Treatment should tonify deficiency and clear heat. However, this child was injured by bitter-cold drugs, rejecting food; only warming tonification is suitable." Administered Lizhong Tang (Regulate the Middle Decoction) continuously for two doses. Within a short time, more than ten stools passed. Ye feared error and requested a new prescription. Yu said: "My intent was to save the failing Stomach Qi, not specifically treat dysentery. Even if treating dysentery, the intestines are deeply coiled; even divine medicine cannot instantly alter feces. Now, using drug power to accelerate evacuation is beneficial. Why doubt it?" Continued the same medicine for two days. Condition improved significantly; appetite returned, vomiting ceased. After four days, only residue passed. Regulated with Buzhong Yiqi Tang (Tonify the Middle and Augment Qi Decoction) for ten days until full recovery. This shows pediatric dysentery is mostly due to dietary injury to the Stomach, rarely internal accumulated heat; thus, standard dysentery protocols are unsuitable.
Chen Qingzhang, Prefect Zu Yongyun, reported: "While serving in Nankang, my ten-year-old son suffered Jin Kou Dysentery, unable to take fluids for days, accepting only medicine. A colleague's medical book contained a method. One dose slightly reduced dysentery; three doses induced desire for porridge; one meal of half a cup was consumed. Dysentery ceased thereafter. The method uses dried Shan Yao (Dioscorea): half fried yellow, half raw, pulverized, taken with rice soup."
A man's young daughter suffered dysentery with rectal prolapse. Shu Quan transmitted a formula: one handful of grass tea leaves, seven slices of ginger, decocted and taken. Cure achieved. Source unknown. Later reading Su Shi's writings, it was discovered that ginger-chopped tea was Su Dongpo's formula for treating Wen Lugong's dysentery.
Chen Liangfu stated: "In summer-autumn of the Jiazi year, residing at Marshal Zhao's hall, a nine-year-old nephew suffered severe dysentery. Pediatrician delayed; I examined. Six pulses were flat and fine. Clinically appearing as bloody dysentery, but actually merely bloody water. I recalled *Tiao Zhong Tang* indications: Early autumn following sudden cold during peak summer heat; heat binds limbs causing high fever/headache; cold injures Stomach causing dysentery with blood/water/redness, high fever/confusion, rapid pulse. Suitable for this formula. Removed Da Huang (Rhubarb) and administered; cured."
Yao Gongyuan's young son suffered dysentery. A physician mistakenly purged, leading to pure blood, asthma, fever, and anorexia. Miao Zhongchun arrived, urgently prescribing Ren Shen (Ginseng) 4-5 qian, Shi Lian Zi (Stone Lotus Seed), Bai Shao (White Peony Root), Sheng Ma (Cimicifuga), Ju Hong (Tangerine Pith), Cao Shi Can (Herba Cynanchi Stauntoniae), Bian Dou (Lablab Bean), Hua Shi (Talcum), and Zhi Gan Cao (Honey-fried Licorice). One dose stopped asthma and bleeding; several doses stopped dysentery. On departure, advised: "If the child survives hundred days without smallpox, he lives, as post-purge Yuan Qi (Original Qi) is unrecovered." Soon after, smallpox erupted; he died. My younger brother, Zhiduan, suffered severe dysentery in childhood, dozens of times daily. Several doses cured. Formula: Ren Shen (Ginseng) 3 qian, Wu Zhu Yu (Evodia) soaked seven times 1 qian, Chuan Huang Lian (Coptis) stir-fried with ginger juice 1 qian. Last two steamed over rice, decocted to 80%, taken warm. If intolerant, alternate one spoon medicine with one spoon rice soup, gradually administering. Stomach Qi recovers. If headache/fever occur, add Shi Gao (Gypsum) 6 qian, Gan Ge (Kudzu Root) 1 qian, mix separately Liu Yi San (Six-to-One Powder) 4 qian, taken cold.
