In a woman's life, nothing is more important than childbirth; at the time of labor, nothing is more urgent than promoting delivery; after delivery, no condition is more severe than retained placenta. The reasons for retained placenta include: some cases are due to blood deficiency causing dryness and roughness, or uterine emptiness leading to adhesion and failure to expel. Other cases are due to Qi and Blood deficiency, where the mother lacks strength, Qi fails to circulate, and cannot transport the placenta, resulting in stagnation. The symptoms manifest only as weakness, without abdominal pain or distension. Treatment should focus on tonifying Qi and assisting Blood. Quickly decoct a large dose of Shenghua Tang (Life Generating Decoction) and administer two to three bowls rapidly, or concurrently administer Yimu Wan (Motherwort Pill), so that Blood becomes abundant and Qi harmonizes, allowing the placenta to fall naturally. Alternatively, use Baosheng Wuyou San (Protect Life and Ensure Peace Powder) to consolidate Yuan Qi. As for Wan Mizhai's use of Wuling San (Five-Ingredient Powder with Poria), I fear it lacks the ability to nourish Blood, and thus dare not follow its usage. It is better to use Juejin Jian (Decoction for Crossing the Ford) from *Jingyue Quanshu* (Complete Works of Zhang Jingyue), or Huatai Jian (Decoction for Slippery Fetus), Wuyou San, or Heishen San (Black Divine Powder) from *Ju Fang* (Formulas of the Bureau). Another cause is lochia flowing into the uterus, causing distension that prevents expulsion. Generally, after the infant is born, the placenta and umbilical cord must descend; thus, the placenta within the abdomen takes the shape of an inverted lotus leaf. An inverted shape collects blood and fluid, causing distension that hinders expulsion. Secure the infant firmly, do not cut the umbilical cord immediately. Only experienced and knowledgeable midwives should insert their right index and middle fingers along the umbilical cord upwards until the end of the cord. Insert the fingers slightly upward, about half a cun, and if blood is felt upon palpation, it indicates the placenta. Press downwards gently; as the blood disperses, the placenta will naturally detach. Alternatively, use a finger to push against the fetal bottom to disperse accumulated blood. Or, feel the upper opening with a finger, open a corner, allowing lochia to drain out; once the abdomen is empty, the placenta will naturally descend. This method is very simple and should be considered the best technique for expelling the placenta. Some people, fearing that delayed placental expulsion in cold weather harms the infant, hastily cut the umbilical cord and wash the baby, tying and suspending the cord with cloth. The method of tying was described in the previous section "Notes on Waiting for Placental Expulsion." However, if blood has seeped into the uterus preventing expulsion, and manual methods cannot be used, prolonged stagnation may gradually fill the chest and heart, causing distending pain or rapid breathing. In such cases, drugs that purge and break up blood stasis are indispensable. Ancients used Liangfang Duoming Dan (Miracle Drug Preserving Life Pill from Good Prescriptions) or Shixiao San (Pain-Stopping Powder), taken with hot wine to disperse blood, reduce distension, and expel the placenta; delay leads to untreatable conditions. There are also single-herb prescriptions for expelling the placenta, selected here for emergency use in rural areas lacking medicine. Secular practices include inserting a woman's own hair into her throat to induce vomiting, causing Qi to rise and blood to disperse, softening the placenta for natural expulsion. Others use heated soles of the mother's shoes to moxa-compress the lower abdomen, which can also expel the placenta. Other formulas include Xueji Gao (Blood Extreme Paste), Sannai Fang (Sanmae Prescription), Huaruishi San (Flower Rock Stone Powder), Liangfang Niuxi San (Good Prescription Achyranthes Powder), Shenying Dan (Divine Response Pill), Moxibustion on the Little Toe, Niuxi Tang (Achyranthes Decoction), Shetui Wujin Wan (Snake Slough Black Gold Pill), and Banxia San (Pinellia Powder), all treating retained placenta. I believe that since manual methods are simple, effective, and non-injurious, they should be preferred. Although vomiting methods are effective, if vomit does not occur but flows backward, death is certain. Among the various formulas for expelling the placenta, those using Niter (Xiao), Achyranthes (Niuxi), Flower Rock Stone (Huaruishi), and Realgar (Liuhuang) are not only ineffective but also severely damage Stomach Qi even if successful. Moreover, harsh mineral substances are likely unsuitable for those with severe deficiency of the intestines and stomach. These later formulas are recorded for reference only. Users must exercise caution, as drug efficacy may not match the speed of manual methods and carries risks of harm. If manual methods are impossible and drugs are necessary, Shixiao San (Pain-Stopping Powder) or Wanquan Tang (Ten-Thousand Complete Decoction) with Ginseng removed are most appropriate. Secondly, Juejin Jian (Decoction for Crossing the Ford) is also good.
Yimu Wan (also known as Fanhun Dan / Soul-Returning Pill. Includes Yimu Gao.) Treats leukorrhea (red and white discharge) in women, continuous postpartum lochia, and various strange pains during pregnancy, postpartum, and menstruation; it treats everything. Materia Medica states: This herb causes no stagnation before birth and no deficiency after birth, hence named "Beneficial to Mother."
Yimucao (Motherwort Herb) (Single ingredient. Also known as Chongweizi, also known as Yetianma. Square stem, opposite leaves, leaves resemble hemp flowers, opens purple flowers in April-May; white-flowered varieties are invalid.) Collect in May, dry completely including roots, stems, and leaves. Avoid iron contact. Grind into fine powder. Refine honey and form pills the size of a bullet. Take one pill each time, dissolve in hot wine and child's urine (Tongbian). Or take with decoction according to syndrome differentiation. Another prescription: Use this powder, take two qian each time, with wine or child's urine, or with syndrome-specific guiding herbs. Another prescription: In case of sudden labor preparation, use fresh Motherwort herb, crush and extract juice, add a little honey, and take orally; the effect is significant. One Yimu Gao (Motherwort Paste) formula: Follow the above collection method, crush, filter through cloth to obtain thick juice. Boil in a clay pot over moderate heat until it becomes a paste, dark brown like black sugar candy. Store in porcelain jars. Take two to three spoons each time, mixed with wine and urine. Or add one spoon to blood-regulating decoctions for better effect.
Liangfang Duoming Dan (Miracle Drug Preserving Life Pill from Good Prescriptions) Treats blood stasis entering the uterus, causing fullness and difficulty in expulsion. Take urgently; blood dissipates, and placenta falls naturally. I consider this formula harsh and advise against light use.
Fuzi (Prepared Aconite Root, 5 qian) Ganqi (Dry Resin, crushed, stir-fried until smoke ceases) Mudanpi (Tree Peony Bark) Danggui (Angelica Sinensis, 1 liang each)
Grind into powder. Separately, take Dahuang (Rhubarb) powder 1 liang, boil with 1 sheng of good vinegar into a paste. Mix with the aforementioned powder to form pills the size of tung tree seeds. Swallow 5-7 pills with warm wine.
Shixiao San (Pain-Stopping Powder) Treats unbearable stabbing heart pain in women, postpartum "child pillow" blood stasis, and pain caused by evil blood attacking upwards. Also treats intestinal Qi pain, and specifically treats retained placenta due to blood stasis distension.
Wulingzhi (Trogopterus Dung, raw, ground with wine, settled to remove sand) Puhuang (Lygodium Spore, raw, sifted clean)
Grind into powder. Take 2-3 qian each time, mix with scallion decoction. Another prescription: Boil with wine and take hot. Another prescription: Use one ladle of good vinegar, boil into paste, add one bowl of water, boil to 70%, take hot. Another prescription: Mix with vinegar paste to form pills the size of longan, take one pill each time, boil in half a bowl of child's urine and half a bowl of water to 70%, take warm. Note: For moving blood, use equal parts raw. For stopping bleeding, use fried versions, or halve Wulingzhi. For pain relief, halve Puhuang. Another prescription: Half raw, half fried Puhuang.
Xia Taiyi Danfang (Single Prescription for Expelling Placenta) Treats retained placenta. (This formula is identical to the one for expelling dead fetus, but treatment differs, so it is re-recorded.)
Heiniufen (Black Cow Manure) (Any amount)
Lightly roast while moist, wrap in cloth, bind to the upper abdomen. Placenta will expel immediately.
Another Formula for Retained Placenta
Mangxiao (Natrii Sulfas, 3 qian) Niuxi (Achyranthes Bidentata) Danggui (Angelica Sinensis, 5 qian each)
Decoct in yellow wine and take. Do not use lightly unless the woman is robust.
Xueji Gao (Blood Extreme Paste, also known as Cudahuang Wan) Treats retained placenta, evil blood rushing to the heart, and painful abdominal blood masses.
Jinwen Dahuang (Striped Rhubarb, 1 liang)
Grind into powder. Boil with half a sheng of good vinegar into a paste. Form pills the size of tung tree seeds. Dissolve five pills in wine and take. Expels quickly. Also treats women with dry blood and heat, wiry-rapid pulse. Take with warm wine before sleep. Promotes bowel movement 1-2 times, meridians clear naturally. Also treats amenorrhea, generally applicable.
Sannai Xiabao Fang (Sanmae Prescription for Expelling Placenta)
Sannai (Sanmae Rhizome, 1-2 jin)
Hold in mouth, swallow saliva when produced. Placenta will fall naturally.
Huaruishi San (Flower Rock Stone Powder) Treats retained placenta, dead fetus in uterus. Also treats incomplete postpartum lochia, evil blood rushing to heart, syncope due to blood loss, etc. If dead but heart still warm, urgently take 1-2 qian mixed with child's urine. Expels foul matter resembling chicken liver slices; lifelong prevention of blood deficiency syncope. If blood above diaphragm turns to yellow fluid, expectorate immediately or pass via urine. Also treats all blood stagnation, near-death Qi collapse. For weak patients unable to withstand strong purgatives, this converts clotted blood to liquid.
Huaruishi (Flower Rock Stone, 1 liang) Liuhuang (Realgar, 4 qian)
Grind into coarse powder. Place in earthenware bottle, seal with salt mud, dry. Roast over charcoal fire for one day. Cool, grind finely. Take 1 qian, mix with child's urine. This is a mineral drug; do not use lightly. Recorded for reference only. Postpartum intestine-stomach deficiency cannot tolerate this.
Liangfang Niuxi San (Good Prescription Achyranthes Powder) Treats retained placenta, abdominal distension and pain. Take urgently to rot and expel.
Niuxi (Achyranthes Bidentata) Chuanxiong (Ligusticum Chuanxiong) Puxiao (Mirabilite) Puhuang (Lygodium Spore, 3 liang each) Danggui (Angelica Sinensis, 1.5 liang) Guixin (Cinnamon Twig, 5 qian)
Take 5 qian each time, add 3 slices of ginger and 1 qian of Shengdihuang (Rehmannia Root), decoct in water and take.
Jiu Zu Xiaozhi Fa (Moxibustion on Little Toe Method) Treats difficult labor and retained placenta.
Moxa-burn three cones on the tip of the mother's right little toe. Cone size like wheat grain. Facilitates easy delivery. This method may suit rural women who do not bind feet. Not suitable for bound-footed women.
Qianjin Niuxi Tang (Thousand Gold Achyranthes Decoction) Treats retained placenta after delivery.
Niuxi (Achyranthes Bidentata) Qumai (Dianthus Superbus, 1 liang each) Danggui (Angelica Sinensis) Tongcao (Tetrapanax Papyriferus, 1.5 liang each) Huashi (Talcum, 2 liang) Kuizi (Abelmoschus Manihot Seeds, half sheng)
Decoct these six ingredients in water, divide into three doses. Another prescription: Without Huashi, add Guixin 1 liang.
Shetui Wujin Wan (Snake Slough Black Gold Pill) For retained placenta, ancient method uses one snake slough, burn over sesame oil lamp, grind to powder, add Shexiang (Musk), mix with child's urine. Or add Qiai (Artemisia argyi), Ejiao (Donkey-hide Gelatin), Sumu (Sappan Wood) 1 qian each, malt powder paste to form pills, named Wujin Wan. For difficult labor or dead fetus, take with child's urine. Some use only snake slough, roasted in butter, ground to powder, take one teaspoon with child's urine.
Banxia San (Pinellia Powder) Treats obese parturient women with excessive phlegm obstructing airways causing difficult labor, dead fetus with dried membranes, or placenta retention after fetal expulsion.
Banxia (Prepared Pinellia Tuber) (Any amount)
Grind into powder. Take one cun-sized spoon with child's urine, continue for three doses. Also blow powder into nose to induce sneezing, greatly activating orifices.
