Pregnancy-related "Zhuanbao" (bladder displacement) refers to urgent pain below the umbilicus and difficulty in urination. This condition arises from forcibly holding urine, walking hastily when the urge to urinate is strong, holding urine after a full meal, or engaging in sexual intercourse while holding urine. These actions cause water qi to rebel upwards, compressing the bladder, causing it to become twisted and unable to expand. It is not a disease of the small intestine or bladder itself, nor can it be treated with diuretic drugs; the treatment method should focus on regulating qi, which will lead to recovery. This condition frequently occurs in women who have weak constitutions, are prone to worry and depression, have irritable temperaments, or consume rich and heavy foods. Because the bladder cannot rotate normally and is compressed by the fetus, if the fetus is lifted up, the bladder's ligaments will naturally relax, and the urinary pathway will clear. Use Erchen Shengti Yin (Two-Olds Lifting Decoction). Another method involves administering Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), followed by inducing vomiting to lift the qi; once the qi flows freely, immediate supplementation with Ren Shen (Ginseng Radix et Rhizoma) and other tonics is necessary to prevent miscarriage. If the medicinal effect has not yet arrived and the distending pain is unbearable, an elderly woman should apply sesame oil to her hands and manually lift the fetus through the vaginal opening; the urine will flow out naturally, relieving the distension and urgency, but this must be followed by large doses of Ren Shen (Ginseng Radix et Rhizoma) and other tonics. However, Zilin (pregnancy strangury) is similar to Zhuanbao. Frequent, scanty, and painful urination is called Zilin, caused by deficiency-heat in the bladder and small intestine. Deficiency prevents control over water, and heat prevents free flow, resulting in strangury. If urination is frequent and scanty but painless, it is Zhuanbao; although there may be slight pain, it is ultimately different from the stinging pain of Zilin. (The "urgent pain" mentioned in the discussion refers to pain felt when urgently needing to urinate, which stops after voiding. Slight pain differs from the stinging pain of Zilin.)
Erchen Shengti Yin (Two-Olds Lifting Decoction): Treats abdominal pain in pregnant women, strangury and retention of urine, or slight pain, distinct from strangury, caused by qi deficiency and fetal pressure on the bladder.
Ren Shen (Ginseng Radix et Rhizoma) (1 qian), Bai Zhu (Atractylodis Macrocephalae Rhizoma, stir-fried with soil) (1.5 qian each), Sheng Di Huang (Rehmanniae Radix) (1.5 qian each), Dang Gui (Angelicae Sinensis Radix) (2 qian), Chuan Xiong (Ligustici Chuanxiong Rhizoma) (0.8 fen), Ban Xia (Pinelliae Tuber, processed or stir-fried with sesame oil) (0.6 fen), Chai Hu (Bupleuri Radix), Sheng Ma (Cimicifugae Rhizoma), Chen Pi (Citri Reticulatae Pericarpium), Zhi Gan Cao (Glycyrrhizae Radix et Rhizoma Preparata) (0.4 fen each).
Add one slice of Sheng Jiang (Zingiberis Rhizoma Recens), decoct in water and take orally. Alternatively, drink salt water on an empty stomach to induce vomiting, thereby lifting the qi, allowing urine to pass spontaneously; this is another method. (I believe inducing vomiting is only for emergency use; afterwards, lifting and tonifying medicines must be taken.)
