Hypochondriac pain after childbirth is caused by retained blood (lochia) flowing into the Liver Meridian. The Foot-Jueyin Liver Meridian traverses the hypochondriac and costal regions, thus causing pain. Xiong Gui Xie Gan San (Cnidium and Angelica Decoction to Drain the Liver) is the principal formula.
Angelica Caudex (Gui Wei), 2 qian; Chuanxiong Rhizome (Chuanxiong), 2 qian; Green Tangerine Peel (Qing Pi), 2 qian; Cyperus Rhizome (Xiang Fu), 2 qian; Aurantium Fruit (Zhi Ke), 2 qian; Safflower (Hong Hua), 2 qian; Peach Kernel (Tao Ren), 2 qian.
Decoct in water. Take with one cup of fresh urine from a healthy child (Tong Bian) and one cup of wine.
Ruxiang Gao (Frankincense Ointment): Treats severe lower back and hypochondriac pain after childbirth that is unbearable. These pains are all due to retained blood flowing into these two meridians. Use Frankincense Resin (Ruxiang) and Myrrh (Moyao), each 5 qian, ground into fine powder. Add one cup of wine and one cup of vinegar, boil down to an ointment consistency, spread on cloth, and apply as a patch.
Inability to speak after childbirth: This condition occurs because retained blood is not fully expelled, stagnating and blocking the Heart orifices, leading to unclear spirit and consciousness, often resulting in confusion. The Qi of the Heart connects to the tongue. When Heart Qi is blocked, the tongue becomes stiff and speechless. Shizhen San (Seven Treasures Powder) is the principal formula.
Ginseng Radix et Rhizoma (Renshen), 2 qian; Acori Tatarinowii Rhizoma (Shichangpu), 2 qian; Rehmanniae Radix (Shengdi), 2 qian; Chuanxiong Rhizome (Chuanxiong), 2 qian; Angelicae Sinensis Radix (Danggui), 2 qian; Asari Radix et Rhizoma (Xixin), 6 fen.
Decoct in water, mix with 5 fen of Cinnabar (Chensha).
Slurred speech after childbirth, also treating palpitations (Zhengchong). Generally, the Heart governs Blood. If too much blood is lost, Heart Blood becomes deficient. The tongue is the sprout of the Heart. When its Blood cannot nourish the tongue upwards, the tongue atrophies, curls, and shortens, preventing clear speech. Jiawei Shengmai San (Modified Replenishing the Pulse Powder) is the principal formula.
Ginseng Radix et Rhizoma (Renshen), 1.5 qian; Ophiopogonis Radix (Maidong), 1.5 qian; Schisandrae Fructus (Wuweizi), 15 grains; Rehmanniae Radix (Shengdi), 1 qian; Angelicae Sinensis Radix (Guishen), 1 qian; Glycyrrhizae Radix et Rhizoma Praeparata cum Melle (Zhigancao), 1 qian; Acori Tatarinowii Rhizoma (Shichangpu), 1 qian; Porcine Heart (Zhuxin), 1 piece, split open.
Decoct in water until 2.5 zhan remain. Remove the heart tissue/internal parts, add to the decoction, and simmer until 0.7 zhan remains.
If palpitations occur, take with 3 fen of Cinnabar (Chensha).
Abdominal masses (Jia) after childbirth: Caused by incomplete expulsion of lochia and deficiency of Spleen Qi, which loses its function of transportation and transformation, leading to accumulation and formation of a mass. Over time, phlegm-fire and blood accumulate and grow larger, reducing appetite, causing progressive emaciation, and intermittent fever/chills. Treatment focuses on tonifying deficiency. Jianpi Xiaoji Wan (Spleen-Strengthening and Mass-Resolving Pill) is the principal formula.
Ginseng Radix et Rhizoma (Renshen), Atractylodis Macrocephalae Rhizoma (Baizhu), Poria (Fuling), Glycyrrhizae Radix et Rhizoma Praeparata cum Melle (Zhigancao), Pinelliae Rhizoma (Banxia), Citri Reticulatae Pericarpium (Chenpi), Cyperus Rhizome (Xiang Fu), Peach Kernel (Tao Ren), Trogopteri Dung (Wulingzhi), Calcitum (Fuhaisshi), Corydalis Rhizoma (Yanhusuo), Sparganii Rhizoma (Sanleng), Curcumae Rhizoma (Ezhu), Curcumae Aromatica Rhizoma (Yujin).
Boil red dates until cooked, remove skin and pits. Use Shenqu (Medicated Leaven) and rice drink to make a paste for binding the pills.
The above conditions could not be fully recorded in the secret manuscript. Therefore, they are excerpted from Mr. Wan Mizhai's Gynecology texts to supplement what was missing. However, one must not rigidly adhere to fixed decoctions. The key lies in carefully observing the pulse and symptoms at the time of treating the patient and thoroughly examining the properties of the herbs. In my opinion, it is generally advisable to follow Danxi's Shenghua Tang (Postpartum Recovery Decoction) as the base formula, then add specific herbs according to the presenting symptoms. This approach ensures success every time and avoids any risk.
