The Master said: Leukorrhea in women is one of the "Six Extremes" (six types of extreme deficiency syndromes). A floating pulse indicates intestinal murmurs and abdominal fullness; a tight pulse indicates abdominal pain; a rapid pulse indicates itching in the yin region, potentially leading to sores; a string-taut pulse indicates pulling pain in the yin region.
The Master said: Leukorrhea is categorized into three gates: one is the Bao Gate (Uterine Gate), two is the Long Gate (Dragon Gate), and three is the Yu Gate (Jade Gate). Women who have given birth belong to the Bao Gate; those who have not given birth belong to the Long Gate; and unmarried girls belong to the Yu Gate.
Questioned: What are the three diseases affecting unmarried women who rarely go out? The Master said: The first disease occurs when menstruation first begins, with heat in the yin region, caused by exposure to wind or fanning. The second disease is caused by washing with cold water. The third disease arises from fright upon seeing menstrual blood, which falls under the category of leukorrhea.
The Master said: There are nine types of excess patterns in leukorrhea. If one contracts the disease known as "Shu Ru" (Mouse Breast), which is severe and changes rapidly, the prognosis should be judged on Geng or Xin days. Other cases follow this pattern.
Questioned: A woman around fifty years old suffers only from back pain, frequent abdominal pain, loss of appetite with aversion to food, and a preference for abdominal distension. Her pulse is faint at the Cun position, and small and tight at the Guan and Chi positions. The physical symptoms do not correspond with the pulse. What does this mean? The Master said: One should ask about the patient's diet. If the patient says she has no desire to eat and feels nauseous smelling grain, the disease is in the Upper Jiao. If the patient says she sometimes desires food but can manage without it, the disease is in the Middle Jiao. If the patient says her diet remains unchanged, the disease is in the Lower Jiao, belonging to leukorrhea, and should be treated accordingly.
For leukorrhea accompanied by irregular menstruation, lower abdominal fullness and pain, and menstruation occurring twice a month, Tu Gua Gen San (Trichosanthes Root Powder) is the primary prescription.
For leukorrhea with a floating pulse, aversion to cold, and metrorrhagia (non-cyclic vaginal bleeding), the condition is difficult to treat.
The Master said: A woman brought her fifteen-year-old daughter for consultation. She stated that her daughter's menstruation started normally at fourteen but has now ceased, causing great fear. The Master asked: Is this girl your biological daughter? If so, I will explain. The woman replied: Yes, she is my biological daughter. The Master said: I ask this for no other reason than that you also experienced cessation of menstruation after it began at fourteen. This is called "Bi Nian" (Avoiding the Year), do not be alarmed; menstruation will resume naturally.
Cold in the lower abdomen with long-term aversion to cold leads to infertility in young women and sterility in older women.
The Master said: A faint and choppy pulse in a young woman indicates infertility; in a middle-aged woman, it indicates sterility.
The Master said: A floating and tight Shao Yin pulse indicates Hernia-Jia (abdominal masses) and abdominal pain, resulting from miscarriage or abortion injury. Floating indicates blood loss, sterility, and aversion to cold.
The Master said: In obese individuals, a fine pulse indicates Cold in the Bao Palace (Uterus), leading to few offspring. Yellowish complexion indicates Cold in the Chest.
In women, rolling and striking pain in the lower abdomen that resolves spontaneously, with irregular onset and cessation of menstruation, combined with hard, urgent pain in the bladder radiating to the yin region, will eventually lead to stiffness and tension in both hypochondriac regions if prolonged.
Questioned: A woman around fifty suffers from diarrhea lasting dozens of days without cessation, evening fever, urgency and pain in the lower abdomen, abdominal fullness, hot palms, and dry lips and mouth. What is the cause? The Master said: This disease belongs to leukorrhea. Why? Because there was a previous miscarriage, leaving Blood Stasis in the lower abdomen. How do we know? The symptom of dry lips and mouth confirms this. Wen Jing Tang (Warming the Menses Decoction) should be administered.
Questioned: Why does a woman suffering from diarrhea experience the cessation of menstruation? The Master said: Merely stop the diarrhea, and menstruation will return naturally; do not be surprised. The reason diarrhea persists while menstruation ceases is the loss of Body Fluids due to diarrhea. Once diarrhea stops and fluids recover, menstruation will resume. If a woman experiences vaginal bleeding with dry throat but no thirst, menstruation must cease because Ying Blood is insufficient and there is inherent slight Cold, hence no desire to drink. If there is thirst and desire to drink, it indicates that Body Fluids are flowing freely, Ying and Wei are harmonized, and menstruation will surely resume. The Master said: A faint and choppy Cun Kou pulse indicates insufficiency of Wei Qi (Defensive Qi) and deficiency of Blood Qi. Insufficient Wei Qi causes shortness of breath and dry form; deficient Blood causes physical disorder. When both Ying and Wei are deficient, speech may be erroneous. A floating and choppy Zu Yang pulse indicates deficiency of Stomach Qi, leading to shortness of breath, dry throat, bitter taste, and loss of fluids due to stagnant Stomach Qi. A faint and slow Shao Yin pulse indicates lack of Essence (slow implies Cold in the yin); choppy implies blood does not flow. This is called Ju Jing (Residing Menstruation), occurring once every three months. The Master said: Faint pulses indicate dual deficiency of Blood and Qi. In young women, this is Blood Loss. Diarrhea during lactation is acceptable; otherwise, it is Ju Jing, occurring once every three months.
Questioned: A woman at three months of pregnancy consults the Master, who diagnoses that she is not pregnant and that her monthly menses should arrive. What is the pulse like? How to distinguish it? The Master said: At the Cun Kou, the Wei pulse is floating and large, while the Ying pulse is surprisingly weak. Floating and large indicate strong Qi; surprisingly weak indicates little Blood. Solitary Yang exhalation cannot be absorbed by Yin; the two energies do not interact. Wei descends, Ying exhausts; Yin becomes accumulated Cold, Yang becomes gathered Heat. Excess Yang fails to moisten, meridians and collaterals are insufficient. Yin deficiency allows Yang to advance (or excess), leading to scant blood. Occasional chills, dry throat, sweating, thick frequent urination, and excessive saliva indicate double deficiency. Leakage of Body Fluids confirms non-pregnancy. Accumulated irritability and fullness, receiving one channel's energy per month, results in menstruation every three months. Excess Yin causes discharge, named Ju Jing.
Questioned: A woman around fifty experiences sudden clear blood discharge for a day, continuing for two or three days. How to treat? The Master said: This woman previously had sterility and amenorrhea. Now she suddenly discharges clear blood; this is Ju Jing. No treatment is needed; it will stop spontaneously. Normal menstruation lasts five days; here, five days will bring recovery. If a woman's menstruation comes twice a month, upon arrival, the pulse should be normal. If it is surprisingly faint, unfavorable, and without sweating, her menstruation will definitely come in two months.
