Upon the initial contraction of Cold Damage (Shanghan), the pathogenic factor first invades the Taiyang (Greater Yang) channel; therefore, treatment prioritizes diaphoresis (sweating). If sweating occurs profusely like rain and the pathogen dissipates, the disease will be cured. However, if the condition remains unresolved after sweating, it may be because the exterior pathogen is not yet fully cleared, or the heat pathogen has transmitted inward, or the pathogenic factor takes advantage of deficiency to invade internally. Thus, there are cases where the disease persists despite sweating.
If the disease remains unresolved after sweating and aversion to cold appears instead, this indicates a deficiency pattern; Shaoyao Gancao Fuzi Tang (Peony, Licorice, and Aconite Decoction) should be used.
In Taiyang disease, if fever persists with sweating but the heat does not subside, accompanied by palpitations in the epigastrium and trembling desire to fall, Zhenwu Tang (True Warrior Decoction) is indicated.
After sweating, if body pain persists and the pulse is deep, Guizhi Ji Shaoyao Renshen Xinjia Tang (Cinnamon Twig Decoction with Added Peony and Ginseng) should be administered.
In Taiyang disease, excessive sweating leads to continuous perspiration, aversion to wind, difficulty in urination, and spasmodic stiffness of the four limbs making flexion and extension difficult; Guizhi Jia Fuzi Tang (Cinnamon Twig Decoction with Added Aconite) is appropriate.
After massive sweating, if the heat does not abate, accompanied by abdominal cramping, limb pain, diarrhea, and aversion to cold, Sini Tang (Recovery from Collapse Decoction) should be used.
Excessive sweating leading to dizziness (mao), palpitations in the epigastrium that are relieved by pressure, requires Guizhi Gancao Tang (Cinnamon Twig and Licorice Decoction).
After sweating, if abdominal fullness occurs, Houpo Shengjiang Renshen Tang (Magnolia Bark, Fresh Ginger, and Ginseng Decoction) is indicated. *(Note: Text says "Nongpu Shengjiang Renshen Tang", likely referring to Houpo Shengjiang Banxia Renshen Tang contextually, but strictly translating text provided)* -> Houpo Shengjiang Renshen Tang (Magnolia Bark, Fresh Ginger, and Ginseng Decoction).
After Taiyang disease sweating, massive sweating causes dryness in the stomach, insomnia, and thirst. Giving small amounts of water frequently will lead to recovery. If the pulse is floating, urination is difficult, there is slight fever, and excessive thirst (xiaoke), Wuling San (Five-Ingredient Powder with Poria) should be used.
After sweating, if the pulse is floating and rapid with irritability and thirst, Wuling San (Five-Ingredient Powder with Poria) is indicated.
After taking Guizhi Tang, if the pulse becomes surging (hongda) and follows the previous method, or if symptoms resemble malaria occurring twice daily and resolve with sweating, Mahuang Tang (Ephedra Decoction) should be used.
If fever and sweating persist without resolution, accompanied by epigastric stuffiness and hardness, and persistent vomiting, Da Chaihu Tang (Major Bupleurum Decoction) is indicated.
After taking Guizhi Tang, if massive sweating leads to severe irritability and unquenchable thirst with a surging pulse, Baihu Jia Renshen Tang (White Tiger Decoction with Added Ginseng) should be used.
After sweating, if there is palpitation below the navel suggesting impending Ben Tun (Rushing Pig) syndrome, Fuling Guizhi Gancao Dazao Tang (Poria, Cinnamon Twig, Licorice, and Jujube Decoction) is appropriate.
After sweating, if there is no longer aversion to cold but only aversion to heat, with steaming fever, this indicates an excess pattern; Tiaowei Chengqi Tang (Regulating the Stomach and Directing Downward Decoction) *[Text reads "Wei Wei Chengqi Tang", corrected to standard name]* should be used.
After sweating, Guizhi Tang should not be used again.
If there is sweating and wheezing without significant high fever, Mahuang Xingren Gancao Shigao Tang (Ephedra, Apricot Kernel, Licorice, and Gypsum Decoction) *[Text reads "Mahuang Xingren Gancao Tang", missing Gypsum]* is indicated.
The methods to eliminate Cold Damage pathogens are limited to diaphoresis, emesis, and purgation. If these three methods are applied correctly, the pathogenic force declines easily, and the disease heals; if misapplied, the condition becomes critical with endless transmission, which is indeed worrying. Moreover, after diaphoresis, emesis, or purgation, if the pathogen remains due to deficiency or stagnation, one must assess deficiency and excess to treat accordingly. Ancients stated that knowing the deficiency/excess of the pathogen and appropriately using warming tonification or acupuncture/moxibustion when diaphoresis, emesis, or purgation fail ensures complete success.
If the disease passes its cycle (transmitting through the six channels over six days, with seven days as one qi period), and does not heal by the thirteenth day, it transmits through the channels again, termed "Guojing" (Passing the Channel).
In Cold Damage at five to six days, after severe purgation, if body heat persists and there is painful knotting in the heart region, the disease is not yet resolved; Zhizi Chi Tang (Gardenia and Fermented Soybean Decoction) should be used.
After purgation, if there is irritability, abdominal fullness, and restlessness lying down and getting up, Zhizi Houpo Tang (Gardenia and Magnolia Bark Decoction) is indicated.
In Yangming disease, after purgation, if there is profound discomfort and irritability, Zhizi Chi Tang (Gardenia and Fermented Soybean Decoction) should be used; if dry feces are present, Dachengqi Tang (Major Order the Qi Decoction) is appropriate.
After Taiyang disease purgation, if the pulse is hurried and the chest is full, Guizhi Qu Shaoyao Tang (Cinnamon Twig Decoction without Peony) should be used; if there is slight aversion to cold, add Fuzi (Aconite) to the formula without Peony.
If a physician mistakenly uses pill-based purgatives, resulting in persistent heat and slight irritability, Zhizi Ganjiang Tang (Gardenia and Dried Ginger Decoction) is indicated.
In Taiyang disease presenting with Guizhi syndrome, if a physician erroneously applies purgation, causing persistent diarrhea with a hurried pulse indicating the exterior is not resolved; if there is wheezing and sweating, Gegen Huanglian Huangqin Tang (Kudzu, Coptis, and Scutellaria Decoction) should be used.
After taking Guizhi Tang or undergoing purgation, if headache, neck stiffness, absence of sweating, mild fever, epigastric fullness, slight pain, and difficulty urinating persist, Guizhi Qu Gui Jia Fuling Baizhu Tang (Cinnamon Twig Decoction without Cinnamon Twig plus Poria and Atractylodes Macrocephala) is indicated.
In Taiyang disease, after passing the channel for more than ten days and undergoing two or three purgations, if vomiting persists and there is slight irritability, Da Chaihu Tang (Major Bupleurum Decoction) should be used.
After purgation, followed by another severe purgation six to seven days later, if the cun pulse is deep and slow, hands and feet are cold, there is diarrhea, the pulse is imperceptible, throat is obstructed, pus and blood are vomited, and diarrhea is uncontrollable, it is a difficult-to-treat case; Mahuang Shengma Tang (Ephedra and Corydalis Decoction) is indicated.
