[General Principles] Generally, childbirth has its own appropriate timing. Before the signs of labor appear, one must absolutely not forcibly administer drugs for inducing labor or facilitating delivery; only when circumstances leave no other choice should they be taken. It is also said: One must absolutely not sit on the birthing bed too early, nor allow the midwife to handle the mother recklessly. (All labor-inducing drugs must only be administered after severe lower back pain occurs, the fetus descends, and the amniotic fluid breaks.) The general treatment principle is: Use slippery agents to unblock stagnation, use bitter agents to expel obstruction, and use aromatic agents to open orifices and dispel blood stasis. For qi stagnation, promote qi movement; if the amniotic sac ruptures first leading to exhaustion, consolidate the blood. Danxi stated: For inducing labor, only Foshou San (Buddha's Hand Powder) should be used, which is the safest and most effective. (See General Principles of Fetal and Postpartum Care for the formula.)
[Slippery Agents]
Cui Sheng Ru Sheng San (Induction Like-Sage Powder)
Malvae Seed (Half a small measure. Another version specifies two qian weight.) Grind until smooth, filter through wine to remove dregs, and take warm. The effect is miraculous. For cases with bleeding leading to dry fetus and difficult labor with extreme pain, take three doses consecutively. After a while, abdominal qi will feel relaxed, and the fetus will slide out easily. However, it must only be taken when normal labor signs are present. The song says: "Seventy fried Malvae seeds, ground fine and mixed with wine to relieve your urgency; when facing life-threatening dystocia, it saves the whole family from weeping."
Another prescription: Mix sesame oil, white honey, and urine evenly, half a cup each, and take with Motherwort powder. Another version uses only oil, honey, and urine, which can facilitate difficult labor.
Another prescription: For transverse or breech presentation.
Ming E Jiao (Colla Corii Asini, processed by stir-frying) 1 liang, Hua Shi (Talcum) powder 1 liang, Kui Zi (Malvae Seed) 2 liang.
Add one and a half cups of water, decoct to one cup, remove dregs, and divide into two doses.
For difficult labor lasting five to six days without delivery, accompanied by physical exhaustion and deficiency:
Guang Ming Shui Jiao (Light-bright Water Gelatin) 2 liang, bake over low fire, add one and a half sheng of good wine, boil, add gelatin until melted, then add one fresh chicken egg and one qian of salt, mix well, let cool to warm temperature. Have the parturient sit on a chair, extend her waist, and drink in two large gulps. Delivery will occur when she feels heaviness in urination, as this is due to premature sitting on the birthing bed causing disturbance.
[Bitter Agents]
Cui Sheng Zuo Mu Yin Zi (Induction Oak Wood Drink)
Treats dystocia, whether transverse or breech, or retained dead fetus causing abdominal distension and rotting. This formula has proven miraculous repeatedly.
Large Zuo Mu Branch (A large bunch, one chi long, washed clean, cut into cun segments, used raw) 5 chi of large Gan Cao (Licorice Root), cut into five segments.
Place in a new porcelain jar with three and a half sheng of newly drawn water. Seal tightly with triple-layered paper. Decoct over mild heat until reduced to one and a half sheng until fragrant. When the parturient feels heavy, painful lower back and desires to sit on the birthing bed, take one small cup warm. (Do not take if there is no heavy/painful sensation in the lower back.) She will feel relief in the chest/epigastrium. If thirsty, drink another cup up to three or four cups. When she feels heaviness below, delivery will occur. This formula is highly effective.
[Mei] Treats difficult labor where the fetus obstructs the abdomen, such as when the head is visible but the placenta does not deliver, or if the fetus is dead.
Ji Li Zi (Tribulus Fruit) and Bei Mu (Fritillaria Bulb) each 4 liang, grind into powder. Take one spoonful with rice water. If not delivered after walking for about four to five li, take another dose.
Note: Bitterness can direct qi downward. Zuo Mu, Ji Li, and Bei Mu are not solely bitter; they possess the merit of benefiting orifices and promoting blood circulation. They are listed here merely following Lou's compilation.
[Aromatic Agents]
Ru Zhu Dan (Milky Cinnabar Pill)
Use transparent Ru Xiang (Frankincense), grind finely, form pills with pig heart blood, size of Sophora seeds, coat with Cinnabar, and dry in the sun. Take one pill per dose, dissolve in cold wine. If not delivered after a while, take another pill. For severe dystocia, use seven Lotus Leaf Heart Stems, decoct in two cups of water to one cup, warm, and dissolve one pill. If not delivered after a while, take another. The efficacy is miraculous. If the fetus is delivered but the placenta remains, taking this will cause the placenta to deliver. If the fetus is in a transverse or inverted position, first take Shen Ying Hei San (Divine Response Black Powder), then take this drug to induce labor. The best time to prepare the medicine is noon on the fifth day of the fifth lunar month, or the seventh day of the seventh lunar month, the third day of the third lunar month, or the Chen hour of the first Shang day.
Another prescription: Transparent Ru Xiang (grind to powder the size of a Gleditsia pod). When lower back pain occurs, take one small cup of newly drawn water, add a little vinegar, decoct together. Have the parturient hold two Shi Yan (Stone Swallows) in both hands. The midwife prepares and administers the medicine. Within moments, she will sit on the birthing bed and deliver without pain. Excellent effect.
Kai Gu Gao (Bone-Opening Ointment) (Prepare at noon on the fifth day of the fifth lunar month.)
Grind Ru Xiang finely, drop water to form pills the size of a Chinese Quince seed. Take one pill per dose, swallow with non-grain alcohol.
Another prescription: Ru Xiang and Zhu Sha (Cinnabar) in equal parts, grind to fine powder, take with wine.
Cui Sheng Dan (Induction Pill) (Also known as Rabbit Brain Pill.)
Treats abnormal physiology during childbirth, difficult delivery, or transverse/inverted presentations.
Rabbit brain marrow from the twelfth lunar month (remove skin/membrane, grind to paste) 1 qian of Mu Ding Xiang (Cloves), fine powder; 2.5 qian of Ru Xiang (Frankincense), separately ground; She Xiang (Musk), separately ground, one zi.
Mix these three ingredients, bind with rabbit brain marrow into pills the size of a cockle shell, dry in shade, wrap in oiled paper. Take one pill per dose with warm water. The infant will be born holding the medicine.
Ru Yi San (As-You-Wish Powder)
Only take when experiencing lower back pain during labor.
Ren Shen (Ginseng Root), ground; Ru Xiang (Frankincense) each 1 qian; Zhu Sha (Cinnabar) 2 qian.
Grind together. At urgent labor, mix with one egg white, then dilute with fresh ginger juice and take cold. If transverse or inverted, it will correct itself, ensuring safety for mother and child.
Sheng Jin San (Victorious Gold Powder)
Guo Jizhong stated: Dystocia is caused by the fetus turning, breaking the blood clots of the fetal pillow, which stagnates with decidual blood in the uterus, preventing the fetus from moving freely, thus causing difficult labor. Urgently take Sheng Jin San to dissipate the blood, making it easier for the fetus to be born. Chen Wuyue stated: Often due to the fetus not turning smoothly, sitting on the birthing bed too early, or pushing too hard, causing the amniotic sac to rupture and blood/fluid to dry up, making the birth canal rough and the fetus difficult to pass. First take Cui Sheng Ru Shen San (Induction Like-God Powder) to consolidate blood. If transverse or inverted birth occurs, use acupuncture as previously described.
She Xiang (Musk) 1 qian, ground; Yan Chi (Salted Fermented Soybeans) 1 liang, wrapped in green cloth, heated red-hot, quickly ground fine.
Heat a lead weight until red-hot, quench it in wine, mix with wine, and take one qian.
[Promoting Qi Movement]
Cui Sheng Tang (Induction Decoction)
Treats pregnancy nearing term with sparse contractions, difficult labor lasting two to three days without delivery, intrauterine fetal death, or maternal qi deficiency and fatigue, dry birth canal. Can be taken as soon as contractions become dense and waters break.
Cang Zhu (Atractylodes Rhizome) 2 liang, soaked/washed in rice water, cut, stir-fried yellow; Xiao Nong Zhi Ke (Bitter Orange Peel) remove pith, bran-fried; Bai Jie Geng (Platycodon Root); Bo Chen Pi (Tangerine Peel) remove white pith; Yang Shao Yao (Peony Root); Chuan Bai Zhi (Angelica Dahurica); Da Chuan Xiong (Sichuan Lovage Rhizome); Da Dang Gui (Chinese Angelica Root) remove tail, each 1 liang; Jiao Zhi Gui (Cinnamon Bark); Ban Xia (Pinellia Tuber) washed in hot water; Fen Cao (Licorice Root); Ma Huang (Ephedra Stem) remove joints; Jun Jiang (Ginger) peel off; Nong Po (Magnolia Bark) remove rough bark, cut, stir-fry with ginger juice; Nan Mu Xiang (Costus Root) avoid direct fire; Xing Ren (Apricot Seed) remove skin/tips, separately grind; Bai Fu Ling (Poria) each 5 qian.
Grind into powder. Take 2 qian per dose, mix with warm flowing water. If feeling hot/stuffy, take with white honey water, or decoct the powder with one he of true rice vinegar.
Yang Shao Yao and Rou Gui (Cinnamon Bark) in the formula can open the uterus; the rest boost qi, naturally opening orifices. Ma Huang internally promotes yang qi; abundant yang qi promotes blood circulation, leading to delivery. Externally expels cold pathogens and removes accumulations. Very suitable for winter months; not recommended for summer. Alternatively, use Wu Ling San (Five-Ingredient Powder with Poria) with Kui Zi and Deng Xin Cao (Juncus Effusus) decocted in water to take, which clears summer heat and soul, facilitating slipperiness and easy delivery. If amniotic fluid breaks first, causing dry fetus and difficult labor, use white honey and clear oil, soak in hot wine, take at once. Once fetal qi is moistened, delivery occurs.
Dystocia Formula: Suo Sha (Amomum Fruit), Xiang Fu (Cyperus Rhizome, vinegar-fried), Zhi Ke (Bitter Orange Peel), Gan Cao (Licorice Root), Hua Shi (Talcum).
Grind into powder, take with warm water.
[Promoting Blood Circulation]
Ben Cao (Compendium of Materia Medica) indicates it treats difficult labor. Crush Yi Mu Cao (Motherwort Herb) to extract juice (seven da he), decoct until halved, take at once for immediate delivery. If fresh is unavailable, use a large handful of dried herb, decoct in seven he of water.
[Dan] Yi Chan Tian Ma Wan (Easy-Birth Gastrodia Pill)
Tian Ma (Gastrodia Rhizome - Note: Text identifies this as Yi Mu Cao/Motherwort, June harvest with roots/flowers/leaves, dried) grind into powder, bind with refined honey into pills the size of a dragon eye. Chew one pill with warm water at labor onset; it can cure hundred postpartum diseases.

Take as one dose, decoct in two zhong of water to one zhong. Add Ru Xiang and Xue Yu, mix well, take anytime.
[Consolidating Blood]
When amniotic fluid breaks first, excessive lochia flows, and the dry fetus cannot descend, first administer Si Wu Tang (Four-Substance Decoction) to nourish blood and qi. Then, decoct scallion soup strongly, let cool, have the midwife wash the vulva carefully to ensure qi flows up and down smoothly. Apply Hu Su (Butter) mixed with Hua Shi (Talcum) powder to the inside of the vulva. Then take Shen Miao Ru Zhu Dan (Miraculous Milky Cinnabar Pill) or Kui Zi Ru Sheng San (Malvae Seed Like-Sage Powder).
Cui Sheng Ru Sheng San (Induction Like-Sage Powder)
Treats inverted/transverse birth, thin fetus.
Bai Cao Shuang (Soot from Hundred Herbs) and Xiang Bai Zhi (Aromatic Angelica Dahurica) avoid direct fire, grind to powder.
Equal parts, grind uniformly. Take two large qian per dose. At labor onset, mix with child's urine and a little rice vinegar to form a paste, take with boiling water.
Ji Yan (Collected Proven Prescriptions): Decoct wine and child's urine (half cup each), take hot immediately upon boiling. Usually delivers after one more dose. (Danxi uses Xiong Gui Tang (Liquorice and Angelica Decoction) to regulate blood; blood stops when blackened. This drug greatly consolidates blood, preventing dry birth.)
For women who have labored for days, exhausted qi, unable to deliver, with lochia fully discharged and uterus dry, finally failing to deliver: Use one dou of Chi Xiao Dou (Adzuki Bean), nine sheng of water, cook until soft. Remove bean residue. Add three liang of E Jiao (Donkey-Hide Gelatin), melt in bean juice. Take half a cup per dose. If not delivered, take again, and it will come out.

Grind into powder. During labor contractions, heat a plowshare iron until red-hot, quench in wine, mix and take three qian. After drinking one or two cups, delivery occurs immediately.
Sheng Jin San (Victorious Gold Powder)
Treats difficult labor, expels stale blood, allowing natural birth. Corrects transverse/inverted positions; softens and expands the uterus if fetus is dead, leading to delivery. Secret transmission from ancestors, priceless.
Wang Bu Liu Xing (Vaccaria Seed); Suan Jiang Cao (Persicaria Hydropiper); Chong Wei Zi (Leonurus Fruit); Bai Ji Li (Tribulus Fruit) remove spines; Wu Ling Zhi (Trogopterus Dung) use raw for blood-moving, all in equal parts.

[External Application]
Ru Sheng Gao (Like-Sage Ointment)
Treats difficult labor, retained placenta, and dead fetus.
Bi Ma Zi (Castor Seed) 7 pieces, remove shell, grind finely into ointment. Apply to sole of foot. Placenta will deliver; wash off immediately, otherwise intestines may prolapse. Apply this ointment to the crown of the head instead, and intestines will retract. Another version: 100 Bi Ma Zi (Castor Seeds), 1 qian Xiong Huang (Realgar), grind finely, apply as above.
Cui Sheng Wan Jin Bu Chuan Yu Xian Dan (Induction Ten-Thousand-Gold Non-Transmitted Immortal Pill)

Once the head is born, urgently remove the medicine. One pill can be used three times.
"Three Castor, Four Croton, strip clothes; grind fine, add Musk. If a woman faces dystocia, paste on umbilicus split apart." (Use 3 Castor Seeds, 4 Croton Seeds, remove shells, add Musk, grind fine, paste on umbilicus.)
Li Sheng Dan (Establishing Sage Pill)
Treats critical difficult labor.
Han Shui Shi (Calcitum) 4 liang, 2 liang raw, 2 liang calcined/red-hot.
Grind together finely, add Zhu Sha (Cinnabar), grind until deep peach-blossom colored. Take 3 fen per dose, mix with well-water to thin paste. Cut paper flower like apricot leaf, spread paste on it, apply to umbilicus. Change when dry. Not more than three applications, and delivery will occur. Treats transverse/inverted bad omens and retained dead fetus. Miraculous. (Note: Han Shui Shi is not Calcite, but what people now call Soft Gypsum. This formula was transmitted by an extraordinary person, very subtle.)
[Miscellaneous Prescriptions]
Tu Jing (Illustrated Classic) states: Have the parturient hold one Shi Yan (Stone Swallow) in each hand; within moments, the baby will be born.
Jiu Chan Nan Jing Ri Bu Sheng (Rescue Difficult Labor Lasting Days Without Birth): Yun Mu Fen (Mica Powder) 0.5 liang, take with warm wine. Delivers upon entering the mouth, ten thousand times never missing one. (Lu Shi states: Has saved three to five hundred people, effective.)
Treats transverse/inverted abnormal labor mechanics: Use Fu Long Gan (Dragon's Liver/Logwood Earth) finely ground, take 1 qian with wine. The earth will emerge wearing the baby's head, wonderful.
[Shi] Ru Shen San (Like-God Powder)
Treats induction with cumulative effectiveness. The spiritual mechanism is hard for people to understand, but the actual use is extremely effective.
At labor onset, have someone find a pair of broken straw sandals on the road, take the ear, burn to ash, take 3 qian bi with warm wine. Left foot yields male, right foot yields female, upside down yields death, sideways yields fright. Truly magical. Using this to send Induction Pills is especially wonderful.
Sheng Jin Dan (Victorious Gold Pill)
Treats difficult labor miraculously. Burn one failed rabbit-hair brush head to ash, grind fine, take with one cup of fresh lotus root juice. If the parturient is weak or has chronic cold disease, fearing lotus root's coldness moves qi, warm it in a double boiler before taking. Immediate delivery.
[Acupuncture]
[Dong] Women about to give birth, with premature rupture of membranes, continuous flow of foul water, sitting on birthing bed too early, no blood to nourish, depletion. Du Yin (0.5 cun, between the third joint of the little toe) Cheng Yin (1.5 cun).
[Yu] Induction, difficult labor, and dead fetus. Tai Chong (0.8 cun, tonify 100 breaths) He Gu (Tonify) San Yin Jiao (0.5 cun, sedate, immediate resolution). Zu Xiao Zhi Jie (3 moxa cones. Xin Shu adds this acupoint). Fetus rising upward pressing on heart. Ju Que (Have sit upright, embrace head and waist slightly leaning back, needle 0.6 cun, retain 7 breaths, sedate upon obtaining qi, immediate revival. If fetus grasps mother's heart, needle mark on palm after birth. If fetus presses heart, mark on Ren Zhong. Mark on occipital bone backward. These are signs. Divine efficacy.) He Gu (0.3 cun, retain 3 breaths, tonify) San Yin Jiao (0.5 cun, sedate inhale at Cun position).
[Zhang Zhongwen] Transverse birth, difficult labor. Tip of the little toe of the right foot (Moxibustion 3 cones, immediate delivery).
[Ji] Another method: Du Yin (Same method as above, moxibustion 7 cones, prohibit needling) He Gu (Tonify) San Yin Jiao (Sedate).
[Sang] Difficult labor. San Yin Jiao.
[Treatment Cases] Chunyu Yi treated Wang Meiren of Chuan who carried a child but did not lactate. Summoned Yi, who gave her a pinch of Lang Ku Ti (Cocklebur) medicine, drunk with wine, and lactation resumed. Yi examined her pulse again, finding it rapid. Rapid pulse indicates excess disease, so he gave her one dose of Xiao Shi (Niter), resulting in bleeding of five or six bean-sized clots.
Hua Borin treated a woman with difficult labor, not lactating for seven days, eating very little. Borin observed, gave her a bowl of cool porridge, decocted Maple Leaf tea to mix, she ate it and lactation resumed. When asked for the rationale, Hua said: This woman eats very little; one cannot give birth without grain qi. Maple leaves fall first if born first, last if born last, hence used as tea.
Pang Anchang treated a woman who had not delivered after seven days, hundred treatments ineffective. Pang observed, ordered family to warm her waist and abdomen with hot water, personally massaged up and down. The pregnant woman felt slight intestinal pain, groaned, and gave birth to a boy. Family was surprised and delighted, not knowing why. Pang said: The child has exited the sac, but one hand mistakenly grabbed the mother's intestine and could not release. Talismans and medicines cannot do this. I palpated the child's hand through the abdomen, needled his Tiger's Mouth (He Gu point), pain caused hand withdrawal, hence rapid birth. No other technique. Examining the child, the needle mark remained on the right Tiger's Mouth. A woman had repeated failures to deliver, induction drugs ineffective. Pang said: This must be due to sitting on the birthing bed too early, fear in the heart, qi knotting and not moving, not obstruction. Su Wen states: Fear causes qi to sink. Fear makes spirit timid; timidity closes Upper Jiao; closure causes qi rebellion; rebellion causes Lower Jiao distension; qi fails to move. One dose of Zi Su Yin (Perilla Drink) resulted in delivery. Also treated Zi Xuan Syndrome (Fetal Distress) in women. (Zi Su Yin see Fetal Distress Pressing Heart.)
Wu Jiaoshan treated a woman with difficult labor, not delivering for three days, various blood-breaking and menstruation-promoting drugs ineffective. Wu formulated a prescription using Che Qian Zi (Plantago Seed) as Sovereign, Dong Kui Zi (Wintermallow Seed) as Minister, Bai Zhi (Angelica Dahurica) and Zhi Ke (Bitter Orange Peel) as Assistant/Envoy. Taken at Si hour, delivered at Wu hour. Physicians were amazed. Wu said: Ben Cao states these are sovereign for induction; Mao Shi (Book of Songs) mentions harvesting Yi Yi (Coix Seed) to prevent dystocia.
Liu Fuzhen encountered the daughter of a Prefect who failed to deliver, already prepared for burial. Liu took concentrated Hong Hua (Safflower) decoction, supported the girl on a stool, dipped cotton cloth in the hot liquid, poured continuously onto the cloth, kept water vessel warm and poured. After a while, she revived and gave birth to a boy. Due to severe winter, blood was cold and stagnant; warmth induced delivery. Also miraculous.
A doctor staying at an inn found the owner's wife had not delivered for days, lower body cold, no medicine available, very distressed. Decocted Pepper, Orange, Evodia, etc., washed the umbilicus, abdomen, and entrance area with the warm liquid when possible. Warmth promoted qi and blood circulation, resulting in delivery.
[Pubic Symphysis Separation]
[Xue] Pubic symphysis separation (Jiao Gu Bu Kai) and failure of the birth canal to close are both due to inherent weakness of primordial qi, lack of regulation during pregnancy, leading to inability of blood and qi to circulate. Pubic symphysis separation is Yin qi deficiency, use Jia Wei Xiong Gui Tang (Modified Liquorice and Angelica Decoction) and Bu Zhong Yi Qi Tang (Tonify Middle and Augment Qi Decoction). Failure of birth canal to close is Blood and Qi deficiency, use Shi Quan Da Bu Tang (Ten Substance Great Tonifying Decoction). Official Li Mengqing married a 35-year-old woman as second wife, worried about difficult labor during pregnancy, requested four doses of Jia Wei Xiong Gui Tang for backup. At term, indeed birth canal did not open, took only one dose, and delivered instantly. Official Fei Huaide's wife, birth canal did not open, two days without delivery, took one dose of previous medicine, delivered immediately. Official passed on this formula, users found it infallible. A woman who delivered easily before, at forty years old, had heavy vaginal bleeding during pregnancy, birth canal did not open. Given one dose of previous decoction, plus one jin of Wu You San (Carefree Powder), decocted and sipped frequently to assist blood and enable delivery.
Jia Wei Xiong Gui Tang (Modified Liquorice and Angelica Decoction)
Mainly treats pubic symphysis separation, inability to deliver.
Chuan Xiong (Sichuan Lovage Rhizome) and Dang Gui (Chinese Angelica Root) each 1 liang; Zi Si Gui Ban (Carapace and Plastron of Naturally Dead Turtle) one piece, roast with butter; Fu Ren Tou Fa (Women's Hair) from those who bore many sons/daughters, one handful, burn until carbonized.
Grind into powder. Take 5 qian per dose, decoct in water. Delivery occurs approximately after walking five li. Even if fetus is dead, it will deliver. Calcined turtle shell can also be used.
