The three Yin meridians of the foot all originate from the toes. If Qi and Blood stagnate and fail to circulate, condensing between the nail and the flesh, it can form Onychia (Jia Ju). This condition involves the flow and distribution of the meridians, not merely a disease of the muscles. It may result from trimming nails and injuring the skin, or from overgrown nails invading the flesh, or from ill-fitting footwear causing Qi and Blood obstruction and blockage, leading to putrefaction and ulceration into a carbuncle. Over time, this can rot the fingers and extend upwards to the shins and knees, resulting in sores. However, since the disease is located at the extremities, internal treatment is not necessarily required; rather, topical applications, washing, removal of necrotic tissue, and wearing comfortable shoes are sufficient.
Commentary: The toes are at the lowest part of the body, where Qi and Blood easily stagnate and medicinal efficacy is difficult to reach. In weak individuals, even minor injuries can lead to sores that fail to heal for a long time, let alone severe cases. Without substantial tonification of Qi and Blood, how can recovery be expected? The statement that diseases at the extremities do not require internal treatment is not a universal principle.
A woman injured her second toe while trimming her nails, developing pus but no rupture, with pain radiating to her hand. She mistakenly applied cold-natured external medicines, causing complete ulceration and loss of appetite. Her pulse was deep and fine, indicating that the pain had damaged Stomach Qi rather than toxic invasion. I performed acupuncture to drain the pus and administered Liu Jun Zi Tang (Six Gentlemen Decoction) with Huo Xiang (Patchouli Herb) and Dang Gui (Angelica Sinensis), which improved her appetite. Subsequently, Ba Zhen Tang (Eight Treasures Decoction) with Huang Qi (Astragalus Root), Bai Zhi (Angelica Dahurica), and Jie Geng (Platycodon Root) was given, leading to recovery after more than a month.
A man injured his thumb, which turned purple but was painless. He took formulas to support the interior and expel pus, along with fifty-plus moxa cones. Pain ensued, followed by suppuration and healing.
A young girl suffered frostbite on both feet, which ulcerated in spring, destroying all her toes. After debridement of the necrotic tissue, she recovered following administration of strong tonic herbs.
A woman suffered from an ingrown toenail for over a year, discharging several drops of pus daily. I stated: The big toe is the origin point of the Spleen Meridian. Moxibustion should be applied to the affected area to remove blood stasis and necrotic tissue, allowing Yang Qi to arrive and the wound to close naturally; otherwise, it cannot be cured. She ignored this advice and delayed treatment, eventually dying of consumptive disease. Generally, at the extreme Yin lower parts, Qi and Blood struggle to reach. Women often suffer from this due to tight binding causing meridian obstruction, or wind-pathogen invasion, leading to a lack of nourishment and formation of dead flesh. Treatment must strengthen the Spleen and Stomach, unblock the meridians, and generate Qi and Blood. Some cases develop Tetanus (Po Shang Feng), characterized by trismus and facial paralysis. Initially, Yu Zhen San (Jade True Powder) should be taken, followed by blood-generating and meridian-unblocking herbs.
A man injured his toe while trimming, turning black and painful, threatening to detach. I said: This is due to deficient Yang Qi failing to reach the site. The necrotic tissue must be removed immediately, and strong tonics taken to boost Yuan Qi; otherwise, the necrosis will spread up the leg, and death is inevitable. He did not believe me, and indeed, black rot spread to his shin, leading to death. Generally, the hands and feet are areas rarely reached by Qi and Blood. If sores or injuries occur and Yuan Qi is damaged, pathogenic factors intensify, and upward spreading ulceration leads to death. Non-ulcerated but dark-colored lesions also indicate death. If bones are broken but tendons and skin remain connected, immediate amputation is necessary.
[Essentials] To treat Onychia caused by nail trimming injury or overgrown nails invading flesh, resulting in swollen, painful sores: Further irritation from narrow boots grinding the sides causes swelling, yellow fluid discharge, and cross-infection, rotting all five toes and extending up the dorsum of the foot with blistering resembling burns. Day and night worsening, resistant to medical treatment. Use 5 liang of Green Vitriol (Lv Fan), which resembles Mirabilite (Pu Xiao) in shape but is green. Place on an iron plate, cover with charcoal, seal, and blow fire until boiling. True vitriol flows out as red liquid like molten gold. Wait for boiling to stop and liquid to dry, remove from fire, cool, grind finely, and store when color resembles Yellow Lead Oxide (Huang Dan). First wash the sore with salt water, dry with cloth, apply this powder, and it will heal.
Sheng Hui Toad Powder treats Onychia with thick, swollen, painful skin.
Toad Ash (Ban Liang), Coptis Rhizome (Lian Lian, ground), Calomelas (Ni Fen, half ban fen each), Musk (She Xiang, ground), Realgar (Xiong Huang, ground), Calcined White Alum (Ku Bai Fan, ground, half qian each), Apricot Kernels (Xing Ren, seven grains, roasted black, ground to paste), Deer Antler (Lu Jiao, seven cun, thoroughly burned, finely ground), Rattlesnake Gallbladder (Ran She Dan, half qian).
Mix and grind evenly. Blend with lard from the twelfth lunar month to form a paste. First wash the sore with decoction of Licorice Root (Gan Cao), Cnidium Fruit (She Chuang Zi), and Sophora Bark (Huai Bai Pi), then dry. Apply medicine wrapped in oil paper, then cotton, changing every two days. Remaining flesh and nails will fall off naturally. Change dressing every three days.
Pig Trotter Soup treats Onychia and various malignant ulcers with necrotic tissue.
Raw Pig Trotters (one pair), Akebia Caulis (Bai Lian), Angelica Dahurica (Bai Zhi), Wild Wolf's Tooth (Ye Lang Ya), Peony Root (Shao Yao) (each 3 liang), Coptis Rhizome (Huang Lian), Scutellaria Root (Huang Qin), Rhubarb (Da Huang), Notopterygium Rhizome (Du Huo) (each 2 liang).
Chop ingredients. Boil trotters in 3 dou of water until reduced to 1.5 dou. Remove trotters, add herbs, boil until reduced to 5 sheng. Warm and wash sores three times daily.
White Alum Powder treats blood-wind toxins attacking fingers/toes causing chronic Onychia, with necrotic tissue wrapping the nail, pain, and irregular bleeding. Use this to shrink flesh and dry sores.
White Alum (Bai Fan), Melanterite (Shi Dan) (each half liang), Musk (She Xiang), Dragon's Blood (Qi Lin Jie), Vermilion (Zhu Hong) (each one fen).
Calcine White Alum and Melanterite in an iron vessel over charcoal fire. Add the other three ingredients and grind finely. Sprinkle small amounts directly onto the sore, bind with cloth, changing two to three times daily.
White Alum Powder treats Onychia.
Calcined White Alum (Bai Fan, 1 liang), Musk (She Xiang), Aloe (Lu Hui), Rattlesnake Gallbladder (Ran She Dan) (each half ban fen, ground fine).
Wash sore with sour water, apply dry. Wash 2-3 times daily until healed.
Portulaca Powder treats Onychia.
Wild Purslane (Ma Chi Xian, dried in shade, 1 liang), Costus Root (Mu Xiang), Cinnabar (Dan Sha, ground fine), Salt (Yan, ground fine, roasted, each 2.5 qian).
Except for Cinnabar and Salt, chop and mix well. Roast in an ironing pan over wood ash fire, grind fine. Then stir-fry with Cinnabar and Salt powders. Apply fresh to sores 2-3 times daily.
Kushen Paste treats Onychia worsening day and night, with red flesh growing around the nail.
Kushen (Radix Sophorae Flavescentis), Astragalus Root (Huang Qi) (each 2 liang), Lard (Zhu Zhi).
