A question is asked: What is the condition of a carbuncle (ju, 疽) arising on the bone behind the shoulder? The answer is: This is named Shangshu Ju (Upper Rat Carbuncle), also known as Shangda Ju. It is caused by accumulated anger and depression, belonging to the Taiyang (Greater Yang) channel combined with the Shaoyang (Lesser Yang) channel. In the initial stage, moxibustion through garlic slices is appropriate, along with administering Huoming Yin (Life-Saving Decoction) augmented with Qianghuo (Notopterygii Rhizoma et Radix), Jiegeng (Platycodonis Radix), and Chaihu (Bupleuri Radix). Shengjin Dan (Victory Gold Pill), Zijin Dan (Purple Gold Pill), and Duoming Dan (Life-Extending Pill) may be selected. After ulceration, Shiquan Dabu Tang (Decoction for Ten Thousand Perfections and Great Tonification), Huangqi Muxiang San (Astragalus and Costus Powder), and Renshen Yangrong Tang (Ginseng Nourishing Nutritive Qi Decoction) are used.
A question is asked: What is the condition of swelling, hardness, and pain on both sides at the posterior shoulder axillary region (Jiazhi)? The answer is: This is named Jin Ju (Sinew Carbuncle), also known as Guoshan Ju. Upon onset, there are chills and fever resembling malaria, but with swelling and hardness without a head. Urgent moxibustion through garlic slices is required, and Huoming Yin augmented with Qianghuo should be taken. Shengjin Dan and Duoming Dan can be used to induce sweating. For those with robust constitutions, Yili Jin Dan (One-Pill Golden Pill) and Wanbing Jiedu Dan (Ten-Thousand-Disease Detoxifying Pill) may be selected.
A question is asked: What is the condition of nodules appearing in the neck, axilla, beside the breasts, or masses arising in the soft flesh of the groin? The answer is: This is named Luoli Yong (Scrofula Abscess), belonging to the Hand-Shaoyang Sanjiao (Triple Burner) channel. Its onset is slow, representing a cold syndrome rather than a heat syndrome. It is appropriate to apply Huiyang Yulong Gao (Return-Yang Jade Dragon Ointment) with hot compresses, and to administer Neibu Shixuan San (Internal-Nourishing Ten-Auspiciousness Powder), Heshouwu San (Polygoni Multiflori Radix Powder), or Shengjin Dan, selecting among them.
The Ling Shu states: When a lesion arises in the axilla that is red and hard, it is called Mi Ju (Rice Carbuncle). It is treated with bian stones (stone lancets), which should be fine and long, pricking sparsely, and smeared with shi gao (pig fat). It heals in six days; do not bind it. If the abscess is hard and does not ulcerate, it is called Madao (Horse Knife) or Jiaying (Accompanying Goiter), and must be treated urgently. The Gui Yi Ke states: Neijiu Ju arises in both axillae and arms, as well as in the centers of both palms. Symptoms include shaking chills and fever, dry throat, vomiting after drinking much water, vexation, and a full pulse. Diagnosis on the sixth, seventh, or eighth day indicates that if sweating can be induced, the patient lives; if not, the patient dies.

It belongs to the Hand-Shaoyin Heart channel and the Hand-Jueyin Pericardium channel, caused by wind-heat. It is appropriate to take Neituo Huangqi Chaihu Tang (Internal-Supporting Astragalus and Bupleurum Decoction). For those with robust constitutions, Bazhen San (Eight-Trigrams Powder) and Yili Jin Dan can be used to purge, and Zijin Dan or Shengjin Dan may be selected. For the elderly and weak, Huangqi Muxiang San, Renshen Yangrong Tang, and Shiquan Dabu Tang are the primary treatments.
