(Named "Zhuanbao" or Displaced Bladder)
This condition differs from Lin syndrome (strangury), which is characterized by frequent urination and is called Zilin (pregnancy strangury). In contrast, urinary retention presents as dripping drops of urine, or even complete inability to void, making the condition more severe than Zilin. This occurs because the fetus descends and compresses the bladder's mesentery, hence the name Zhuanbao. The danger arises rapidly; if not treated urgently, turbid pathogenic factors may reverse upward, causing vomiting of clear fluid, resembling Guange (obstruction syndrome), which is certainly critical. To elevate the fetal Qi, Mr. Zhu Danxi used Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction); after administration, inducing vomiting was employed, a method with profound rationale. My teacher, Mr. Zhong Ling, used Fuling Shengma Tang (Poria and Cimicifuga Decoction), achieving miraculous results every time. Ancient formulas include Siwu Tang (Four Substances Decoction) added with Huangqin (Scutellariae Radix) and Zexie (Alismatis Rhizoma), which is used for mild cases. Additionally, Mr. Zhang Zhongjing treated Zhuanbao using Bawei Tang (Eight-Ingredient Decoction). This addresses situations where the Lower Jiao (Lower Burner) occasionally contracts deficiency-cold, causing the fetal Qi to become Yin-cold, cutting off the flow of water; warmth allows Yang Qi to circulate freely. Another type of condition involves isolated Yang without Yin, failing to transform Qi; in such cases, one should nourish Yin, as done with Zishen Wan (Kidney-Nourishing Pill) from ancient texts. My teacher used Liuwei Dihuang Tang (Six-Ingredient Pill with Rehmannia) added with Cheqianzi (Plantaginis Semen) and Niuxi (Achyranthis Bidentatae Radix), often achieving success. These two conditions represent Yin and Yang respectively, involving the distinction between Water and Fire, requiring deep investigation to avoid error. One must examine whether the pulse is slow or rapid, observe whether the patient prefers cold or heat, and combine these findings for differentiation to avoid misdiagnosis.
