[Decoction for Detoxification: composed of Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), Huangbai (Phellodendri Cortex), and Zhizi (Gardeniae Fructus)]
If a pregnant woman contracts Shanghan (Cold Damage) or is infected with seasonal fire syndromes (warm-heat diseases), presenting with persistent fever and drowsiness, cool-natured herbs must be urgently used to clear heat and resolve the pathogen to protect the fetus. For example, Sanhuang Decoction (Three Yellows Decoction), Jiedu Decoction, Huangqin, Huanglian, and Lingyangjiao (Antelope Horn) may be used. However, Xijiao (Rhinoceros Horn) is a substance that damages the fetus and should not be used. If the condition does not resolve, leading to a dead fetus retained in the uterus unable to be expelled, and the pregnant woman falls into coma, treatment becomes extremely difficult at this stage. One must examine the mother's tongue. If the face is red but the tongue is cyanotic, the fetus has already died; urgently use Pingwei San (Regulate the Stomach Powder) added with Poxiao (Mirabilite). After ingestion, the dead fetus will be expelled immediately. However, the dead fetus must be expelled; yet, after expulsion, there is fear of Qi collapsing with the expulsion, while failing to expel it makes it difficult to save the mother. If the face is cyanotic and the tongue is red, or if both face and tongue are cyanotic accompanied by foaming at the mouth, both lives are difficult to save, and no cure is possible. If, upon expulsion of the fetus, the discharge turns red, indicating a remaining breath of life, one should immediately administer Shengmai San-like rescue with Renshen (Ginseng), Danggui (Angelicae Sinensis Radix), and Dihuang (Rehmanniae Radix); occasionally, one or two out of ten may survive.
When Heat enters the Blood level, Xijiao (Rhinoceros Horn) is not used. The classic states: "Where there is pathology, there is no injury [to the healthy body], nor injury to the fetus." It is merely necessary not to overdose.
[Pingwei San]
Cangzhu (Atractylodis Rhizoma) (2 qian) Nongpo (Magnoliae Officinalis Cortex) (1.5 qian) Chenpi (Citri Reticulatae Pericarpium) (1.5 qian) Gancao (Glycyrrhizae Radix et Rhizoma) (0.8 qian)
Add Poxiao (Mirabilite) (5 qian), decoct and take. Another prescription: Use only Poxiao (Mirabilite) 3-4 qian, mixed with one cup of Tongbian (Child's Urine) and warm wine, taken orally; the dead fetus will be expelled immediately. However, after expulsion, treatment should follow postpartum warming and tonifying protocols. If treated as a Heat syndrome again, it would be an error.
Treating fetal damage from Heat disease by using warming and tonifying methods immediately after expulsion of the dead fetus accelerates the mother's death. (Comment by Xu)
Whenever Shanghan (Cold Damage) is encountered, the fetus must be protected; do not guess casually as with ordinary cases. The most stable approach is to use Xiangsu Yin (Fragrant Perilla Infusion), varying it according to channel differentiation.

For Taiyang Channel (Taiyang Meridian) involvement, add Fangfeng (Saposhnikoviae Radix), Jingjie (Schizonepetae Herba), and Qinjiao (Gentianae Macrophyllae Radix). For Yangming Channel (Yangming Meridian) involvement, add Gegen (Puerariae Lobatae Radix) and Zhimu (Anemarrhenae Rhizoma). For Shaoyang Channel (Shaoyang Meridian) involvement, add Chaihu (Bupleuri Radix), Huangqin (Scutellariae Radix), and Renshen (Ginseng).
(This is a Shaoyang Syndrome, where the pathogen is between the exterior and interior.)
This syndrome presents with alternating chills and fever, with clarity of spirit returning after intervals. It may occur once daily or every other day. Initially, Xiangsu Yin (Fragrant Perilla Infusion) should be used to release the exterior. Wait until the chills and fever are distinct before using Xiaochaihu Tang (Minor Bupleurum Decoction) to harmonize. Do not use Chaihu (Bupleuri Radix) too early. If sweating is profuse, add Renshen (Ginseng). If Heat is severe with mild chills and sweating, add Shigao (Gypsum Fibrosum) and Zhimu (Anemarrhenae Rhizoma) to cool and dissipate Heat, preventing Heat from damaging the fetal Qi; attention must be paid early.
