Infantile startled crying refers to the sudden awakening from sleep due to fright. This occurs when wind-heat pathogenic factors invade the Heart, generating heat in the Heart organ and causing mental instability. Consequently, sleep is restless, leading to startled crying.
Infantile nocturnal crying is caused by coldness in the viscera. Nighttime is characterized by abundant Yin qi; when this interacts with internal cold, the cold stirs and combines with the Qi of the viscera, causing irritability or pain, thus resulting in nocturnal crying. However, some cases are caused by violating taboos, which can be stopped through magical methods.
During gestation, if the mother's care was compromised by wind-cold, pathogenic factors entered the uterus and damaged the fetus's viscera. After birth, these pathogens remain in the infant's abdomen. When the pathogens stir and contend with the healthy Qi, abdominal pain ensues, causing the infant to cry while stretching limbs and frowning.
If the mother excessively consumed cold substances during gestation, cold qi entered the uterus and damaged the fetus's intestines and stomach. After birth, the cold qi remains between the intestines and stomach. The presentation includes: cold intestines and stomach unable to digest milk and food, abdominal distension, or intermittent grain dysentery, leading to a pale complexion and frequent crying. This is termed fetal cold.
Infantile abdominal pain is mostly due to imbalance of cold and heat. The conflicting cold and heat Qi strike the viscera, causing pain. In heat-type pain, the face is red, possibly with high fever, restlessness in the four limbs, and heat in the palms and soles. In cold-type pain, the face may be cyanotic or pale; in severe cases, the face turns black, and the lips, mouth corners, and nails become cyanotic.
Infantile epigastric and abdominal pain occurs when chronic cold lingers in the intestines and stomach, compounded by an acute attack of cold Qi. The overlapping cold Qi contends with the visceral Qi, moving up and down with the Qi mechanism, striking the heart and abdomen, thus causing pain.
Craniolysis (splitting of the skull sutures) presents as delayed closure of fontanelles and splitting of cranial sutures in older infants. This is due to insufficient Kidney Qi. The Kidneys govern bone marrow, and the brain is the sea of marrow; insufficient Kidney Qi leads to inadequate marrow and brain development, failing to fuse properly, resulting in split cranial sutures.
Infantile bulging fontanelle is caused by irregular feeding, unregulated hunger and satiety, or exposure to heat or cold invading the Spleen and Stomach. This disrupts visceral harmony, causing Qi to rush upward. The presentation includes a bulging fontanelle filled with material, sweating, and yellow, short hair. If cold Qi rushes upward, the fontanelle feels hard; if heat Qi rushes upward, it feels soft. Additionally, subcostal accumulation with abdominal fullness and fever allows heat Qi to invade the viscera and rush upward to the brain and fontanelle, also causing bulging fontanelle. Coughing and prolonged crying cause Qi to invade the viscera and rush upward, also causing this condition. Feeding immediately after prolonged crying, when Qi has not yet settled, also causes bulging fontanelle. This is because during crying, Yin and Yang Qi rebel and rush upward.
