Whenever a ju (deep-seated abscess) occurs on the lower leg, it is called Tuni Chuang (Rabbit-Bite Sore). It is also known as Xueshi Chuang (Blood-Excess Sore). This condition varies with the phases of the moon (similar to lunar eclipses), but this does not refer to sores on the lower leg per se. Investigating this sore, it arises from the interaction of Wind-Dampness pathogenic factors with Blood and Qi, generated by excess heat in Blood and Qi; hence it is named Blood-Excess. It is also named Rabbit-Bite because its shape resembles a bite mark left by a rabbit.
Regarding Xue Chuang (Blood Sore), it is stated that various sores are generated when Wind-Dampness interacts with Blood and Qi. When the pathogenic heat dissipates and escapes, and the sore only bleeds, it is named Xue Chuang.
At the initial stage of any sore, it forms because Wind-Dampness interacts with Blood and Qi and manifests on the skin. If it persists for a long time without healing, it is prone to contracting Wind, Cold, or Water-Dampness pathogens. Contracting Wind leads to trismus and spasms; contracting Cold makes it difficult to heal; contracting Water-Dampness leads to edema.
For all types of sores and malignant sores, although they initially arise from Wind-Dampness interacting with Blood and Qi, causing accumulation and heat generation which steams the skin and flesh into a sore, if exposed to water and dew moisture, disturbing the meridians, they may fail to heal for over ten years. The sore becomes dark and purplish with scabs, resembling frost-bitten gourd skin, and the flesh inside appears severed; thus it is named Lubai Chuang (Dew-Rotting Sore).
All sores and carbuncles are caused by Wind-Dampness interacting with Blood and Qi, leading to accumulation and heat generation, manifesting in the muscles as sores. If they do not heal for a long time, malignant flesh (necrotic tissue) often grows, protruding at the four sides, while healthy flesh fails to regenerate. This is due to incomplete elimination of toxic heat, obstruction of the meridians, and inability of Blood and Qi to reach the area.
All malignant sores are generated by Wind-Dampness and toxic pathogens. Although they may heal initially, if the Wind-Toxic qi remains in the meridians, subsequent slight exertion with fever or consumption of toxic foods can cause recurrence.
Lacquer contains toxicity. Some individuals have a congenital sensitivity to lacquer; merely seeing lacquer can induce poisoning. They may experience itching on the face, followed by itching in the chest and arms. The face swells, and the area around the eyes becomes slightly red. At all itchy sites, scratching causes raised red streaks and nodules along the scratch marks. After the nodules subside, tiny millet-grain-like pustules appear, very subtle. These are the symptoms of mild poisoning. In severe cases, sores cover the entire body; small ones resemble hemp seeds or beans, large ones resemble dates or apricots. The pus-filled blisters are painful; after being broken, there is slight improvement, but new ones form subsequently. Burning lacquer intensifies its toxic vapors, causing acute and severe reactions in people. However, some individuals are naturally tolerant; even if they boil lacquer all day, they suffer no harm.
