A man who has engaged in prolonged and frequent sexual intercourse suddenly experiences uncontrollable seminal emission at the peak of pleasure and intense passion. After the semen is exhausted, blood follows. He suffers from panting, and his hands, feet, and entire body become cold. People generally believe that seminal depletion in men constitutes Yang collapse (Yang Tuo), while in women it constitutes Yin collapse (Yin Tuo). In reality, both men and women can experience collapse of either Yin or Yang, and treatment should not be differentiated by gender. Generally speaking, such collapse syndromes should be treated by addressing Yang. This is because after seminal depletion, the essence (Jing) is completely lost, which means there is no Yin. At this moment, Yang Qi is also on the verge of collapsing but has not yet fully dispersed. If Yang Qi is not urgently rescued, once it scatters, it will return to Yin (death) very quickly. Moreover, Yin nature is slow, while Yang nature is rapid. Merely tonifying Yin would be extremely sluggish and of little help in an emergency. If one adheres strictly to the theory of tonifying Yin, since the Yin essence has already been entirely drained and the root of True Yin inside is severed, where can the tonification begin? Therefore, tonifying Yang can sustain Yin, whereas tonifying Yin alone makes it difficult to guide Yang back.
However, when Yin essence is exhausted and bleeding follows, it seems hemostasis should be employed. But hemostatic herbs are essentially astringents used to close the leakage. Yet, with no Yin left inside, how can closure be achieved? It is better to use formulas that tonify Qi to assist Yang Qi. When Yang Qi becomes vigorous, Yin will naturally generate. Through the mutual assistance of Yin and Yang, and the smooth flow of Qi and Blood, semen will regenerate and bleeding will stop naturally. This achieves astringency without using astringent herbs.
Prescription: Xuyin Jiujue Tang (Continuing Yin Decoction for Saving the Desperate)
Ren Shen (Ginseng Radix et Rhizoma) 2 liang
Bai Zhu (Atractylodis Macrocephalae Rhizoma) 3 liang
Fu Zi (Aconiti Lateralis Radix Praeparata) 1 qian
Ba Ji Tian (Morinda Officinalis Radix) 1 liang
Decoct in water and take orally. Bleeding stops after one dose; Yin generates after two doses. Taking it continuously for four doses ensures survival.
This prescription is a holy medicine for tonifying Yang Qi. Ren Shen rescues the desperate state and reconnects life in the realm of nothingness; Bai Zhu promotes the free flow of Qi in the waist and umbilical region; Fu Zi retrieves scattered Yuan Yang (Original Yang); Ba Ji Tian tonifies the Yin of the Heart and Kidney. These are still Yang-tonifying herbs; thus, when Yang returns, Yin also returns. If Ren Shen is omitted and only Fu Zi, Bai Zhu, and Ba Ji Tian are used, life can still be snatched from death in an instant. However, without Ren Shen as the sovereign herb to command the formula, the heat of Fu Zi cannot be controlled, posing a risk of excessive Yang damaging Yin. If Yin-tonifying herbs are added later to support, bias will not occur.
For this syndrome, Shen Fu Wu Wei Tang (Ginseng, Aconite, and Schisandra Decoction) is also highly effective.
Ren Shen (Ginseng Radix et Rhizoma) 3 liang
Fu Zi (Aconiti Lateralis Radix Praeparata) 2 qian

A woman fond of sexual indulgence, engaging in unrestrained intercourse, causes deflagration of deficient fire and downward leakage of Yin essence. She falls into apparent death and revives, suffering from dizziness and vertigo, with only wandering Qi remaining. People assume this is Yin collapse, but who would know it is actually Yang collapse? Women are inherently quiet and non-active, making seminal leakage difficult because their bodies are purely Yin, with only a trace of Yang Qi existing in the Kidney. Immortal men collect women's Yang Qi to serve as the mother of elixirs, yet few succeed. Whenever a woman leaks essence, it must be at the extreme of movement, and the pleasure during leakage is indescribable; this is precisely the leakage of her Yang Qi. The leakage of Yang Qi involves the expulsion of the True Yang from the bone marrow through the uterus tube. However, this is merely the leakage of Qi, not Essence. Only when fire moves to the extreme does Liver Qi open widely, causing Blood to fail to store; if Blood fails to store, Essence cannot be secured, and the True Yin in the Kidney leaks out with it. At this time, the woman is in a state of extreme movement she cannot control, willing to die to follow her lover. Thus, the more active she is, the more she leaks, until she dies laughing after exhausting her essence. She relies on the man holding her tightly and breathing air into her mouth, keeping Yang Qi near the Yin gate, allowing her to revive from apparent death. This indicates Yang collapse while Yin is not yet extinct; why not urgently rescue Yin? However, rescuing Yin alone cannot restore Yang; one must still rescue Yang.

Fu Shen (Poria Cum Radix) 5 qian
Sheng Zao Ren (Ziziphi Spinosae Semen) 3 qian
Bei Wu Wei Zi (Schisandrae Chinensis Fructus) 1 qian
Decoct in water and take orally. Yang returns after one dose; Yin generates after two doses. Then add Shu Di Huang (Rehmanniae Radix Praeparata) 1 liang and Shan Zhu Yu (Corni Fructus) 5 qian to the formula. Decoct and take one dose, continuing for one month to restore original vitality.
This formula first uses Ginseng to temporarily restore Yang, then uses Shu Di Huang and Shan Yao to manage recovery in daily life. Ren Shen truly saves collapse and restores Yang but cannot fill dryness to tonify Yin. Tonifying Yang first and then Yin allows depleted essence to regenerate and un-depleted Qi to grow easily, preventing Yang from flourishing and Yin from declining.
For this syndrome, Shen Shu Tang (Ginseng and Atractylodes Decoction) can also save the patient.
Ren Shen (Ginseng Radix et Rhizoma) 3 liang
Bai Zhu (Atractylodis Macrocephalae Rhizoma) 3 liang

A person suddenly experiences chills and collapse during urination. Although there is no leakage of Yin essence, Qi leakage implies Essence leakage. People think this is Wind-Stroke, but who knows it is a dual collapse of Yin and Yang? The Qi transformation of the Bladder enables urination; this Qi is the Qi within the Kidney. Excessive seminal emission weakens Qi. Weak Qi leads to easy Essence leakage; Essence leakage further weakens Qi. Collapse during urination is invariably caused by excessive sexual activity and seminal emission. Collapse during intercourse due to extreme pleasure differs from collapse after intercourse during urination, which occurs in those who have suffered defeat and suffer from impotence. Collapses occurring on the bed often allow for revival, whereas collapses occurring in latrines are often fatal. The former retains the root of Yin and Yang; the latter lacks reliance on Yin and Yang. However, manifestations of collapse vary. If collapse involves fainting and retraction of the external genitalia, rescue is possible. Urgently pull out the glans penis to prevent retraction, then treat with Sheng Ren Tang (Reviving Person Decoction).

Decoct in water and take orally. Revival occurs after one dose; strength improves after two doses. Switch to Tiao Yin Hui Yang Tang (Regulating Yin and Restoring Yang Decoction):
Shu Di Huang (Rehmanniae Radix Praeparata) 2 liang
Shan Zhu Yu (Corni Fructus) 1 liang
Bai Zhu (Atractylodis Macrocephalae Rhizoma) 1 liang
Fu Shen (Poria Cum Radix) 3 qian
Ren Shen (Ginseng Radix et Rhizoma) 1 liang
Rou Gui (Cinnamomi Cortex) 1 qian
Bai Jie Zi (Sinapis Semen) 2 qian
Decoct in water and take orally. Recovery occurs after two months of regulation.
The former formula restores Yang in the realm of nothingness; the latter generates Yin when continuation is possible. Naturally, Yang returns without Yin abruptly severing, and Yin generates without Yang becoming excessively vigorous. Some argue that retracted glans penis indicates extreme cold and impending death, unaware that vitality remains because internal Yang is not yet extinct. If Yang were truly extinct, the glans would not retract deeply. Deep retraction signifies Yin desiring to enter Yang; thus, urgent administration of Yang-warming herbs leads to revival.
For this syndrome, Shen Shu Fu Zi Tang (Ginseng, Atractylodes, and Aconite Decoction) is also effective.
Ren Shen (Ginseng Radix et Rhizoma) 2 liang

A person faints and collapses during defecation, with eyes rolling upward, cold hands and feet, locked jaw, and inability to speak. People assume this is Wind-Stroke with aphasia, but who knows it is Yin collapse? Defecation is governed by the Kidney. Sufficient Kidney Water prevents constipation; deficient Kidney Water leads to diarrhea. The dryness or moisture of stool depends entirely on the Kidney. How can the Large Intestine disease cause sudden extinction? Extremes of dryness burn Water leading to Yin extinction; extremes of slipperiness suppress Fire leading to Yin extinction. Ultimately, Large Intestine Yin extinction stems from Kidney extinction. Therefore, Kidney collapse entails Large Intestine collapse; treating Kidney extinction is key.

Ze Xie (Alismatis Rhizoma) 6 qian

This formula is not specifically for saving collapse. However, Kidney Water exhaustion leads to Kidney extinction. Greatly nourishing Kidney Water provides abundant moisture to dryness, bringing life to all channels. Nourishing Water is precisely how one saves Kidney extinction; how could the Large Intestine, receiving Water, fail to be saved from collapse?
For this syndrome, Liang Yuan Tang (Two-Support Decoction) is also effective.

A person emits seminal fluid upon hearing a woman's voice, without actual sexual intercourse. Although this is not a severe case of Yin-Yang collapse, it is the beginning of collapse. Yin and Yang are inseparable. Prolonged intercourse without emission indicates prosperity of both Kidney Fire and Kidney Water. Only when Kidney Water is deficient does Fire move easily; only when Kidney Fire is deficient is Water hard to secure. Prolonged intercourse without emission reflects not just Kidney Fire/Water prosperity, but also Heart Fire/Water prosperity. Prosperous Heart Fire prevents Kidney Fire from usurping authority; prosperous Heart Water prevents Kidney Water from shifting control. Only when Heart Water is scant does Kidney Water face lower exhaustion; only when Heart Fire is scant does Kidney Fire face downward migration. Hearing a woman's voice causing emission indicates extreme deficiency and agitation of Heart Fire and Water, causing Kidney Fire and Water to leak externally following the agitation of the Heart Sovereign. If emission continues unchecked, it signals impending Yin-Yang collapse, a critical condition. Without urgent treatment, death is imminent. Treatment requires massive tonification of Heart and Kidney.

This formula tonifies both Heart and Kidney, adding slight astringent herbs to close the Jade Gate (external genitalia) and prevent excessive opening of meridians. When Heart and Kidney do not interact, the Jade Gate opens easily; when they interact, it closes firmly. Emission triggered by sound occurs because Essence has already separated from the Kidney Palace, exiting immediately upon hearing due to the open gates within. Hence, astringents are needed amidst tonification.
For this syndrome, Baojing Wan (Essence-Preserving Pill) is also excellent.
