Patient Qian: Guangyujin (Curcuma Aromatica Rhizoma, 3 qian), Xinhui Pi (Citri Reticulatae Pericarpium, 1.5 qian), Liuyi San (Six-to-One Powder, 3 qian, wrapped for decoction), Danzhuye (Lophatheri Herba, 3 qian), Tianhuafen (Trichosanthis Radix, 3 qian).
Commentary by Shao Jie: Fushu (latent summerheat) erupts from within, while Xinliang (new cold/autumn chill) binds from without; hence, initially there is alternating chills and fever, and the tongue coating is slightly white. The formula for clearing heat and promoting diuresis to remove dampness is extremely sound. However, in my opinion, it is advisable to remove Chuanhu (Dendrobii Caulis) and add herbs that penetrate and diffuse pathogenic factors, so that the newly acquired pathogen can be easily expelled, thereby avoiding the risk of dampness lingering.
Patient Gu from Anchang (Recorded by Jianji): Yangming Fushu (latent summerheat in the Yangming channel), with a scorched and greasy tongue coating, epigastric fullness and oppression, nausea and vomiting, wiry and fine pulse, and irritability with fever at night. The condition is tricky and difficult to treat; one should guard against external collapse of Yang qi or internal closure leading to coma, awaiting the recovery of righteous qi. (August 24th)
Gualouzi (Trichosanthis Fructus, 4 qian, crushed), Zhishi (Aurantii Fructus Immaturus, 1.5 qian), Danzhuye (Lophatheri Herba, 1.5 qian), Guangyujin (Curcuma Aromatica Rhizoma, 3 qian), Chao Chuanlian (Coptidis Rhizoma, stir-fried, 8 fen), Jiao Zhizi (Gardeniae Fructus, charred, 3 qian), Chiling (Poria Cocos with Red Host, 4 qian), Wancansha (Bombyx Batryticatus, 4 qian), Xian Banxia (Pinelliae Rhizoma Praeparatum cum Zingibere et Alumine, 1.5 qian), Yuanshishi (Talcum, 4 qian), Guang Xingren (Armeniacae Semen Amarum, 3 qian).
Commentary by Shao Jie: Fushu originates from the Yangming channel. Ancients used Baihu Tang (White Tiger Decoction) as the main formula. This patient suffers from turbid, hot, sticky, and cloying pathogens that linger in the epigastrium and diaphragm after originating from Yangming. The treatment follows the principle of Xiaoxianxiong Tang (Minor Inverted Chest Powder), which constitutes symptomatic therapy.
Patient Gao from Anchang: Autumn summerheat has not cleared; the pulse is choppy and stagnant, Qi mechanism is disharmonious, consciousness is occasionally confused, the essence gate is insecure after urination, the tongue coating is slightly white, and there is abdominal pain. Continue to follow the previous method with modifications. (July 29th)
Guang Huoxiang (Patchouli Herba, 2 qian), Xian Banxia (Pinelliae Rhizoma Praeparatum cum Zingibere et Alumine, 1.5 qian), Sheng Muli (Ostreae Concha, raw, 4 qian), Shilianzi (Nelumbinis Nucifera Semen, 3 qian), Shentoucao (Agastaches Herba, 3 qian), Fuling (Poria Cocos, 4 qian), Kouqiao (Amomi Fructus Pericarpium, 1.5 qian), Tongcao (Tetrapanacis Medulla, 1.5 qian), Chuanlianzi (Toosendan Fructus, 3 qian), Xiangfu (Cyperi Rhizoma, 3 qian), Zuojin Wan (Left Metal Pill, 8 fen).
Decoct in clear water, take three doses.
Commentary by Shao Jie: The patient has a history of spermatorrhea and is additionally affected by autumn summerheat. Therefore, the treatment method, while clearing summerheat and regulating Qi, still assists with astringent herbs for the lower jiao. Although both root and branch are addressed, attention should still be paid to eliminating the newly acquired pathogen.
