Headache due to heat jue (heat reversal) is caused by excessive stomach heat failing to descend downward. The symptoms include internal heat and pain in the head; even in severe cold, one prefers wind and cold. Slightly approaching a warm place or seeing smoke and fire causes the pain to recur. A rapid or large pulse characterizes this condition.
Xiao Qing Kong Gao (Minor Clearing Empty Paste): Cut Scutellariae Radix (Huangqin, 黄芩) finely, mix evenly with wine, dry, grind into powder, and take mixed with clear tea water.
Formula for treating headache due to heat jue: Stir-fry Rhizoma et Rhizoma Rhei (Dahuang, 大黄) three times with wine, grind into powder, and take mixed with clear tea water.
When heat qi resides in the head, using wind-dispelling drugs to guide it will cause the heat to become more intense and the pain to worsen. Rhubarm is bitter and cold, clearing heat; combined with wine, it can ascend to the brain to drain cerebral heat. This corresponds to the ancient saying: "The bird's nest is high on the peak, shoot it down." Tea has a clear and ascending nature, so it is frequently added to various formulas for treating headaches.
(Newly Formulated) Rehmanniae Radix Preparata (Shengdi, 3 qian), Anemarrhenae Rhizoma (Zhimu, prepared with wine), Scutellariae Radix (Huangqin, prepared with wine, each 1 qian), Menthae Haplocalycis Herba (Bohe), Gardeniae Fructus (Heishanzhi), Chrysanthemi Flos (Ganju), Glycyrrhizae Radix et Rhizoma (Gancao), Schizonepetae Spica (Jingjie, each 5 fen), Carthami Flos (Honghua, 3 fen). Take as one dose, decoct with water, and administer after meals. If constipation occurs, add 1.5 qian of wine-stir-fried Rhizoma et Rhizoma Rhei (Dahuang, 大黄). This formula treats headache with vexation and heat, preference for wind and cold, recurrence of pain upon slight exposure to smoke and fire, or constipation, often yielding effective results. Ancient methods state that heavy head pain triggered by movement and tidal heat belong to Stomach disorders. Danxi stated: For headache feeling like bursting, use half liang of wine-stir-fried Rhizoma et Rhizoma Rhei (Dahuang, 大黄), decocted with clear tea water.
Headache due to damp-heat arises from the combination of dampness and heat, steaming and郁 (stagnating) mutually, causing qi to ascend and contend with clear yang qi, resulting in pain. Dongyuan stated: For various damp-heat headaches, Xiao Qing Kong Gao (Minor Clearing Empty Paste) is the primary treatment. He also stated: For headaches caused by damp-heat in the head, bitter emetics must be used, or nasal insufflation drugs.
Qing Kong Gao (Clearing Empty Paste): Treats wind-damp-heat headache, upper congestion damaging the eyes, and chronic unceasing brain pain. Notopterygii Rhizoma et Radix (Qianghuo, 羌活), Saposhnikoviae Radix (Fangfeng, 防风, each 1 liang), Bupleuri Radix (Chaihu, 柴胡, 7 qian), Ligustici Chuanxiong Rhizoma (Chuanxiong, 川芎, 5 qian), Glycyrrhizae Radix et Rhizoma Praeparata cum Melle (Zhigancao, 炙草, 1.5 liang), Coptidis Rhizoma (Huanglian, 黄连, 1 liang, stir-fried), Scutellariae Radix (Huangqin, 黄芩, 3 liang, half prepared with wine, half stir-fried). Grind into fine powder. Take 2 qian per dose, add a little tea, mix with boiling water into a paste, smear inside the mouth, and swallow with a small amount of plain water before bed.
Chu Bi San (Nasal Insufflation Powder): Indigo Naturalis (Qingdai, 青黛), Gypsum Fibrosum (Shigao, 石膏), Mirabilite (Mangxiao, 芒硝), Curcumae Radix (Yujin, 郁金), Menthae Haplocalycis Herba (Bohe, 薄荷), Gleditsiae Sinensis Spina (Yazao, 牙皂). Grind into powder for nasal insufflation. Dongyuan's Baizhi San contains Angelicae Dahuricae Radix (Baizhi, 白芷) but lacks Gleditsiae Sinensis Spica (Yazao, 牙皂) and Indigo Naturalis (Qingdai, 青黛).
Furthermore, a sensation of the head being heavy as if mountains indicates dampness in the head. Use: Hong Dou San (Red Bean Powder). Phaseoli Semen Rubrum (Hongdou, 红豆, 10 grains), Ephedrae Herba (Mahuang, 麻黄), Cucumeris Fructus (Gua Di, 瓜蒂, each 5 fen), Forsythiae Fructus (Lianqiao, 连翘), Notopterygii Rhizoma et Radix (Qianghuo, 羌活, each 3 qian, burnt). Grind into powder for nasal insufflation.
Tou Ding San (Penetrating Top Powder, from *Ben Shi Fang*): Asarum Root (Xixin, 细辛, white variety, 3 stems), Cucumeris Fructus (Gua Di, 瓜蒂, 7 fruits), Eugeniae Flos (Dingxiang, 丁香, 3 grains), Oryza Sativa Germen (Nuomi, 糯米, 7 grains), Borneolum Syntheticum (Bingpian, 冰片), Moschus (Shexiang, 麝香, each size of a black soybean). Grind into extremely fine powder. Take an amount the size of a soybean, insufflate into the patient's nostrils alternately left and right. After a long time, when about one sheng of saliva is discharged, relief is achieved. The nature and flavor of this drug are slightly warmer than the previous nasal insufflation powder; it should be used after careful examination of the syndrome.
Hezi Shen Xiong Wan (Hezi Shenxiong Pills): Treats damp-heat stagnation obstructing the head and eyes, redness, swelling, pain, and obstruction/difficulty in urination and defecation. Rhizoma et Rhizoma Rhei (Dahuang, 大黄), Scutellariae Radix (Huangqin, 黄芩, each 2 liang), Semen Kochiae (Qianniu, raw), Talcum (Huashi, 滑石, each 4 liang), Coptidis Rhizoma (Huanglian, 黄连), Menthae Haplocalycis Folium (Bohe Ye, 薄荷叶), Ligustici Chuanxiong Rhizoma (Chuanxiong, 川芎, each half liang). Grind into powder, make pills with dripping water, size of wutong seeds. Take 50 pills per dose, swallowed with warm water after meals.
