Yellow Emperor asked: How should malaria be treated with acupuncture? Qibo replied: Regarding malaria, *Suwen* classifies it by the various meridians. Wei Shi used finger pricking (i.e., Shixuan points [Ten Exclamations]); if the tongue is red and purple, prick the purple blood vessels under the tongue to bleed.
Malaria of Foot-Taiyang presents with chills first followed by fever, lower back pain, heavy head, and incessant sweating: Prick Weizhong (BL60), depth 3 fen, moxa 5 zhuang.
Malaria of Foot-Shaoyang presents with mild chills and fever, palpitations/fearfulness, and profuse sweating: Prick Xiaxi (GB43), depth 2 fen, moxa 3 zhuang.
Malaria of Foot-Yangming presents with prolonged chills before heat arises, sweating, and a preference for sunlight or firelight: Prick Chongyang (ST42), depth 3 fen, moxa 3 zhuang.
Malaria of Foot-Taiyin presents with alternating chills and fever, frequent vomiting, which subsides after vomiting: Prick Gongsun (SP4), depth 4 fen, moxa 3 zhuang.
Malaria of Foot-Shaoyin presents with more heat than cold, vomiting, and in severe cases, a desire to close doors/windows: Prick Dazhong (KI4), depth 2 fen.
Malaria of Foot-Jueyin presents with fullness in the lower abdomen and difficulty urinating: Prick Taichong (LR3), depth 2 fen, moxa 3 zhuang.
Heart malaria: Prick Shenmen (HT7). Liver malaria: Prick Zhongfeng (LR4). Spleen malaria: Prick Shangqiu (SP5). Lung malaria: Prick Lieque (LU7). Kidney malaria: Prick Taixi (KI3). Stomach malaria: Prick Lidui (ST45).
Verse says: To treat malaria, needle Quchi (LI11); Jingqu (LU8) and Hegu (LI4) are suitable together. Needle these two acupoints at a depth of 5 fen, and the malaria will depart. If not cured, add Sanjian (LI3); do not hesitate to needle deeply at 5 fen. If accompanied by Qi pain and aversion to cold/heat, needle Neiguan (PC6) without delay.
In Shaoyang disease on the first day, needle Fengfu (GV16), selecting acupoints according to the sequence of Yin and Yang meridians.
Day 1 Taiyang: Fengfu (GV16) — Du Meridian. Day 2 Yangming: Ying-Spring points — Dachang Erjian (LI2), Weijing Neiting (ST44). Day 3 Shaoyang: Shu-Stream points — Dan Linqi (GB41), Sanjiao Zhongzhu (TE3), Xinbao Jianshi (PC5) [Note: Text mentions Zhongfeng here but attributes to Pericardium/Jianshi]. Treat exterior by needling the three Yang meridian points; treat interior by needling the three Yin meridian points. On the sixth day, if the disease has passed through its cycle without sweating, needle Qimen (LR14) and Zusanli (ST36); this is the ancient method. For Yin syndromes only, moxibustion of Guanyuan (CV4) is excellent.
The methods of sweating, vomiting, and purging have no other secrets; they rely on Hegu (LI4), Neiguan (PC6), and Yinjiao (CV7).
Sweating method: Needle Hegu (LI4), depth 2 fen. Perform the Jiujiu (9x9=81) manipulation, rubbing/twisting dozens of times. Males twist left, females twist right. Once sweat appears, apply reducing method. When sweating stops and body temperature warms, remove the needle. If sweating does not stop, needle Yinshi (ST31) and tonify Hegu (LI4).
Vomiting method: Needle Neiguan (PC6), depth 3 fen. Tonify six times, reduce three times. Perform Zi-Wu Daojiu (Midnight-Noon Ebb-Flow) method three times to raise Qi upward. Push/manipulate to induce shivering; let the patient call out several times until it stops. If vomiting continues, apply Nine Yang numbers (tonification), regulate breathing thirty-six times. When vomiting stops, slowly remove the needle and quickly press the point. If vomiting persists, tonify Zusanli (ST36).
Purging method: Needle Sanyinjiao (SP6), depth 3 fen. Males use left side, females use right side. Rotate the needle clockwise. Complete the Six Yin numbers. Hold breath with mouth and nose, swallow and distend the abdomen. Before purging, insert the needle once, and the patient will defecate. Inhale thirty-six times while applying manual reduction. Then release breath, re-insert needle, and defecation occurs. If diarrhea persists, needle Hegu (LI4) and apply Nine Yang numbers (tonification). All sweating, vomiting, and purging methods still require distinguishing Yin/Yang and applying tonification/reduction.
Cold headache: Hegu (LI4), Cuanzhu (BL2), Taiyang (EX-HN5). No sweat: Neiting (ST44), Hegu (LI4), Fuliou (KI7), Bailao (EX-HN2). Chest and hypochondriac pain: Daling (PC7), Qimen (LR14), Tanzhong (CV17), Laogong (PC8). Mania disregarding status/hierarchy: Quchi (LI11), Jiegu (GB39), Bailao (EX-HN2), Yongquan (KI1). Persistent high fever: Reduce Quchi (LI11), Jiegu (GB39), Zusanli (ST36), Dazhui (GV14), Yongquan (KI1), Hegu (LI4). Residual heat after fever breaks: Fengmen (BL12), Hegu (LI4), Xingjian (LR2), Jiegu (GB39). Profuse sweating: Neiting (ST44), Hegu (LI4), Fuliou (KI7), Bailao (EX-HN2). Constipation: Zhangmen (LR13), Zhaohai (KI6), Zhigou (TE6), Taibai (SP3). Urinary retention: Yinggu (KI10), Yinlingquan (SP9). Absence of all six pulses: Hegu (LI4), Fuliou (KI7), Zhongji (CV3). Spasm and unconsciousness: Quchi (LI11), Hegu (LI4), Renzhong (GV26), Fuliou (KI7). Jaundice: Wanggu (SI4), Shenmai (BL62), Waiguan (TE5), Yongquan (KI1). Hypochondriac pain: Zhigou (TE6), Zhangmen (LR14), Yanglingquan (GB34), Weizhong (BL40). Passed cycle without sweating: Qimen (LR14). Cold limbs (Jue-ni): Needle Fuliou (KI7) along the bone direction. Floating pulse: Tonify Jiegu (GB39) for cold, reduce Jiegu (GB39) for heat. Floating and surging pulse: Reduce. Deep and fine pulse: Tonify. Locked jaw, closed eyes, inability to take medicine: Hegu (LI4). Hu-huo syndrome with oral ulcers: Huanglian Xijiao Tang (Coptis and Rhinoceros Horn Decoction). Parasites eating inside: Dicang (ST4). Shaoyang vomiting roundworms: Wumei Tang (Mume Decoction), moxa Zhongwan (CV12). Epigastric fullness and hypochondriac mass pain: Qimen (LR14).
