Dian disease (epilepsy/mental disorder) begins with initial dissatisfaction. The patient experiences heaviness and pain in the head, upward gaze with redness of the eyes, and upon severe exhaustion, becomes irritable and distressed. Observe the facial complexion; needle the Hand Taiyang (Small Intestine), Hand Yangming (Large Intestine), and Hand Taiyin (Lung) meridians. Stop when the blood color changes from dark to bright red.
When Dian disease first manifests with mouth deviation, crying, calling, panting, and palpitations, observe the Hand Yangming and Hand Taiyang meridians. If the left side is tense, treat the right; if the right is tense, treat the left. Stop when the blood color changes.
When Dian disease first starts with body rigidity and backward arching, followed by spinal pain, observe the Foot Taiyang (Bladder), Foot Yangming (Stomach), and Hand Taiyang meridians. Needle the relevant points until the blood color changes.
In treating Dian disease, the physician should reside with the patient to observe and determine the appropriate acupoints. When an attack occurs, observe the patient; if there are areas of excess pathogenic qi, apply reducing methods. Collect the discharged blood in a gourd or vessel. At the time of the attack, only the blood at the diseased site will pulsate vigorously. If it does not pulsate, moxa the Qionggu (Coccyx) for 20 moxa cones. Qionggu refers to the coccygeal bone.
In cases of Bone Dian disease, the teeth and all muscle spaces around the Shu points are full and hard, bones protrude, sweating occurs accompanied by irritability, vomiting, excessive salivation, and downward leakage of Qi. These are difficult to treat.
In cases of Pulse Dian disease, the patient suddenly collapses, and the pulses of the four limbs are distended and flaccid. If the pulses are full, needle all full vessels to bleed them; if not full, apply moxibustion. Moxa the Dai Mai (Girdle Vessel) on the waist, three cun away from the point adjacent to the neck Taiyang channel, and needle the Ben Shu (Source Points) of the various muscle spaces. If there is excessive vomiting and salivation with downward leakage of Qi, it is difficult to treat.
If Dian disease manifests as mania during attacks, it is a fatal condition and difficult to treat.
Kuang disease (mania) begins with initial sadness, forgetfulness, distress, anger, and frequent fear, often caused by worry and hunger. Treat by selecting points on the Hand Taiyin and Hand Yangming meridians until the blood color changes, and additionally select points on the Foot Taiyin and Foot Yangming meridians.
When Kuang disease first erupts, symptoms include little sleep, lack of appetite, self-perception of being virtuous, wise, and noble, cursing day and night without cessation. Treat by selecting points on the Hand Yangming, Hand Taiyang, Hand Taiyin, and the Lesser Yin (Heart) channel under the tongue. Observe the vessels: needle those that are full/excess, and leave those that are not full/deficient alone.
If the patient speaks nonsense, is easily startled, laughs excessively, sings, and acts wildly without stopping, this results from great fright. Treat by selecting points on the Hand Yangming, Hand Taiyang, and Hand Taiyin meridians.
If the patient sees hallucinations with the eyes, hears hallucinations with the ears, and frequently calls out, this is generated by deficiency of Qi. Treat by selecting points on the Hand Taiyang, Hand Taiyin, Hand Yangming, and Foot Taiyin meridians, as well as the two sides of the head.
Manic patients who eat much, see spirits and ghosts, and laugh inwardly without outward expression result from excessive joy. Treat by selecting points on the Foot Taiyin, Foot Taiyang, and Foot Yangming meridians, and subsequently select points on the Hand Taiyin, Hand Taiyang, and Hand Yangming meridians.
For newly onset Kuang disease that has not yet progressed to these severe stages, first select the arterial pulse near the Ququan (LR8) point on both sides, and bleed those with excess pulses. After a while, if the condition persists, treat according to standard methods, and moxa the bone crevices for 20 moxa cones.
If there is high fever over the whole body, with mania involving hallucinations and delirious speech, observe the Foot Yangming (Stomach) channel and its large collaterals. Apply tonification for deficiency, and reduction for blood excess. Have the patient lie supine. Position yourself in front of their head. Use the four fingers of both hands to clamp and press the carotid artery. Hold firmly for a long time, then curl the fingers and push downwards towards the Quepen (ST12) area, then stop. Repeat this process until the heat dissipates. This is called "pushing and dispersing."
When disease affects the Yang meridians, characterized by alternating chills and fever, and alternating chills and fever in the muscle spaces, it is named Kuang. Acupuncture should reduce the pulse; observe the muscle spaces, and stop when the heat illness resolves.
If the disease initially recurs once a year or once a month, do not treat specifically. If it recurs four or five times a month, it is named Dian. In such cases, needle the various muscle spaces and meridians. For those without cold signs, regulate with acupuncture needles.
