Chest and hypochondriac pain with inability to breathe freely, inability to lie flat, upward rushing of Qi, shortness of breath; pain on one side with full and raised pulse, the meridian slants out from the coccyx, traverses the chest and hypochondrium, supports the Heart and penetrates the Diaphragm, ascending to the shoulder, similar to Tian Tu (CV2) slanting down to Jian Jing (GB21), then descending below the tenth thoracic vertebra.
Wind pathogen entering from outside causes chills, sweating, headache, heaviness of the body, and aversion to cold. Treatment should focus on Feng Fu (GV16), regulating Yin and Yang: tonify deficiency, sedate excess.
Severe wind disease causing head and neck pain is treated by needling Feng Fu (GV16), located at the upper cervical vertebrae. Wind disease causing profuse sweating is treated by moxibustion on Yi Xi (BL48), located three cun lateral to the lower back spine; press this point while having the patient exclaim "Yi Xi." If wind induces further wind (worsening condition), needle the eyebrow region. Stiff neck (Shi Zhen) pain located between the transverse bone of the shoulder. Wind disease presenting with both chills and fever, where sweating brings relief but occurs multiple times a day, first needle the collateral vessels in the muscle intervals. If chills and fever persist after sweating, needle every three days until cured after one hundred days.
Patients with Li Feng (Leprosy/Mal de Alastres) are initially treated with Su needles (or standard acupuncture) on the swelling. Use a sharp needle to puncture the lesion, pressing to expel evil Qi until the swelling subsides. Bone joint heaviness with loss of eyebrows and beard is named Da Feng (Great Wind). Needle the muscle layer to induce sweating for one hundred days. Needle the bone marrow layer to induce sweating for another hundred days. Total two hundred days; stop needling when eyebrows and beard regrow.
Hemiplegia (Pian Ku) presents as unilateral immobility with pain, normal speech, and clear consciousness; the disease is located in the muscle and interstitial spaces. Treat with Ju Zhen (Giant Needles), tonifying deficiency and reducing excess to restore function. Fei Bing (Flaccidity Paralysis) is characterized by absence of pain and flaccid limbs. If mental confusion is mild and speech is weak but understandable, it is treatable; if severe enough to cause aphonia, it is untreatable. The disease originates in the Yang aspect and enters the Yin aspect; therefore, treat Yang meridians first, then Yin meridians, using superficial floating techniques.
Wei Jue (Atrophy and Cold Extremities) is treated by binding the four extremities tightly, then rapidly releasing them. Perform this twice daily. For numbness (Bu Ren), sensation returns within ten days. Continue treatment without rest until recovery. Annually, stimulate the nose with grass to induce sneezing, which can cure the condition. If Qi stagnation requires rapid guidance and drawing, symptoms cease immediately; sudden shock may also be effective.
The generation of Yong Ju (Carbuncles and Ulcers) and formation of pus and blood do not come from heaven or earth, but arise from accumulation of minute factors. Thus, sages prevent disease before form appears, while fools suffer after it manifests. Once pus and blood have formed, mortality is high (nine deaths for one survival). Sages prevent formation and record excellent prescriptions on bamboo and silk for transmission, ensuring no end to their benefit, so people do not suffer. The Emperor asked: Can small needles treat existing pus and blood? The answer is: Small needles have little effect on large diseases; large treatments may harm greatly. Therefore, for established pus and blood, only Bian Shi (Stone Lancets) and Pi Feng (Lancet Knives) are suitable. Gently press the abscess to observe Qi movement. First needle shallowly nearby, then gradually deepen, repeating up to three times. Observe depth based on floating/sinking. After needling, apply heat therapy (Wen) to drive heat inward, weakening the pathogen until the large abscess ruptures. To treat rotting swellings, pierce the rotting area, adjusting depth based on size. Large abscesses yield much blood; small ones require deep insertion. Insert the needle steadily and correctly. If Yong Ju is neglected, it changes rapidly, becoming indistinct and unresponsive to pressure, fluctuating unpredictably. Needle three points beside Hand Taiyin Lung Meridian and two points of Ying Mai (Vessel connecting to the throat). For hot axillary abscesses, needle Foot Shaoyin Kidney Meridian. If heat persists after five needle insertions, needle Hand Jueyin Pericardium Meridian three points. For great bone intersections along Hand Taiyin meridian, needle three times. Sudden Jin Yong (Sinew Abscess) causes pain according to muscle divisions, with incomplete魄汗 (Po Sweat) and insufficient Bao Qi (Bladder/Essence Qi); treat via Jing Shu (Meridian-Shu) points. For stagnant Yong Qi, use needles to open and remove it. For excessive Qi and accumulated Blood, use Bian Shi (Stone Lancets) to sedate. When needling swellings, rock the needle. Left foot corresponds to Li Chun (Start of Spring), days Wu Yin and Ji Chou. Left hypochondrium corresponds to Chun Fen (Spring Equinox), day Yi Mao. Left hand corresponds to Li Xia (Start of Summer), days Wu Chen and Ji Si. Chest, throat, head correspond to Xia Zhi (Summer Solstice), day Bing Wu. Right hand corresponds to Li Qiu (Start of Autumn), days Wu Shen and Ji Wei. Right hypochondrium corresponds to Qiu Fen (Autumn Equinox), day Xin You. Right foot corresponds to Li Dong (Start of Winter), days Wu Xu and Ji Hai. Waist, coccyx, and lower orifices correspond to Dong Zhi (Winter Solstice), day Ren Zi. Six Fu Organs and three Zang organs below the Diaphragm correspond to Zhong Zhou (Central Region). The greatest taboos are the days when Tai Yi resides, and all Wu and Ji days. In these nine cases, observe the positions of the Eight Regularities (Ba Zheng) and their corresponding body regions. For patients with swellings, do not incise or treat on the corresponding value-day; this is called Tian Ji Ri (Heavenly Taboo Day).
