The Mingtang refers to the nose. The Que refers to the area between the eyebrows. The Ting refers to the forehead (Yintang region). The Fan refers to the sides of the cheeks. The Bi refers to the Ear Gate (Tinghui, 听会) area.
Wind pathogen is the beginning of all diseases. Jue-ni syndrome arises from cold-dampness; it is commonly observed by inspecting the Que area: a thin and lustrous color indicates wind; a turbid and rushing color indicates Bi syndrome; a sunken color indicates Jue. This is the normal manifestation.
If redness appears on both cheekbones, the size of a thumb, even if the illness seems slightly improved, sudden death will inevitably occur. If blackness appears in the Ting area, the size of a thumb, sudden death will occur without prior illness.
When water swelling begins, there is slight swelling above the eye sockets, resembling the appearance after just waking up. The neck vessels pulse, there is occasional coughing, coldness in the inner thighs, swelling of the lower legs, and the abdomen becomes distended. At this point, water disease has formed. Pressing the abdomen with the hand causes it to rise immediately upon release, resembling a bag filled with water; this is its sign. (The *Wu Long Jin Ye Pian* states: When the Qi pathways of Yin and Yang are blocked, the Four Seas are closed, the Sanjiao fails to drain, and fluids fail to transform, water and grain travel together within the intestines and stomach. They separate at the ileum, remain in the Lower Jiao, cannot enter the bladder, causing Lower Jiao distension; overflowing water results in water swelling.)
Fu-zhang (Skin Distension) occurs when cold qi lodges between the skin. The abdomen is distended but not firm; the abdomen is large, the whole body is swollen, the skin is thick, and pressing the abdomen leaves an indentation that does not rise. (The pathology is in the Qi level.) The abdominal color remains unchanged; this is its sign.
Gu-zhang (Drum Distension) presents with abdominal distension and general body enlargement, equal in degree to Fu-zhang. The complexion is pale yellow, and abdominal collateral veins are prominent; this is its sign (distinguished by the prominence of abdominal collaterals).
Chang-qin (Intestinal Polyp/Tumor) occurs when cold qi lodges outside the intestines, struggling with Wei Qi (Defensive Qi), preventing proper circulation. It adheres internally, evil Qi arises, and growths form inside. Initially, it is the size of a chicken egg, gradually increasing until it resembles pregnancy. It may last for years. Pressing it feels hard, pushing it moves it, and menstruation still arrives on time (since the pathology is outside the intestines, it does not affect menstruation).
Shi-jia (Stone Fibroid) originates in the Baozhong (Uterus). Cold qi lodges in the Zimen (Cervical Os), blocking it so Qi cannot flow. Evil blood should be discharged but remains stagnant, enlarging daily, resembling pregnancy, and menstruation does not arrive on time. These conditions occur only in women and can be treated with guiding and purging methods. ("Pi" sounds like 'pi', suspected to refer to degenerated blood.)
