Two legs cramp with severe pain, suddenly the whole body's cyanotic veins swell and rise as thick as chopsticks, releasing toxic blood from everywhere; use Si Yi Wan.
The heart feels incessantly stifled and uncomfortable; there is a desire to eat but inability to do so, while walking and sitting remain normal. Even after drinking warm fluids, no ominous signs appear, nor is there pain in the heart, abdomen, waist, or back, only increasing emaciation day by day. If treatment is not known, it will also cause great harm. This is a slow and mild form of Sha (a condition characterized by sudden illness due to pathogenic factors). Releasing the blood via pricking will lead to recovery. There are cases with headache and fever, abdominal distension resembling Taiyang disease (Exterior Syndrome), alternating chills and fever similar to malaria but not quite, and incessant stifling discomfort. There are cases with cough and stifling discomfort resembling wind-cold invasion. There are cases with swelling of the head and face, eyes like fire. There are cases with redness and swelling of the four limbs, heaviness of the body making turning difficult. This is a slow and severe form of Sha. Mistakenly consuming hot-natured foods leads to heaviness, coma, or rapid breathing and mania. Such cases require careful examination of the pulse and differentiation of symptoms. For pathogens in the exterior, use scraping (Gua Sha); for those in the middle, use bloodletting; for those in the interior, use pills, powders, or decoctions, which must be taken continuously for several doses until slight relief is achieved, followed by gradual conditioning (refer to Si Er, San, Si).
A widow, in April, suddenly fell into heavy coma with blackened complexion, a surging left pulse, and a deep and faint right pulse; this is An Sha (Dark Sha)! Three cyanotic veins in the popliteal fossa were pricked, releasing purple-black blood like a stream, but she did not wake up. The next day, using Shi Er Fang, she slightly revived. On the fifth day, Sha was pricked again, and using Si Er Fang, she fully awakened, then recovered after further conditioning.
An elderly man, in June, suffered from fever and heaviness, coma without consciousness, black coating with prickles on the tongue, continuous cursing, and all six pulses hidden/unfelt, with no blood flowing upon pricking. Using Shi Er Fang and Si San Fang made into a frozen drink for him to consume, and then Jin San Fang, he slightly revived the next day, but his body remained heavy as stone, and the tongue coating and prickles did not recede. He recovered using Jin Liu Fang.
A young pregnant woman, hearing of a fire downstairs at night, fell and fainted into a state of suffocation, making no sound. Fetal-calming and spirit-settling medicines were ineffective. After two days, a dead fetus was expelled. I was called to diagnose: no pulse felt, fine pressing felt like a spider's thread, but the four limbs were warm and soft as if sleeping. Urgently releasing blood from the hands and feet, she began to groan. One dose of Di Sha San was administered, leading to revival, and she recovered using Si San and Si Fang.
An eleven-year-old maid, who had not eaten for six days, had slight swelling of the head and face. She requested a digestion-promoting prescription. I said: "The pulse is faint, and the face is swollen; it is likely Sha." Two needles were inserted in the popliteal fossae, and one dose of Shi Er Fang led to recovery.
