Xie, Left: The Kidney governs the bones, and teeth are considered the surplus of bones; the gums belong to the Stomach meridian. After variola (smallpox/variolous rash), heat-toxins accumulate internally in the Kidney and Stomach meridians, leading to rotting of the gum gangrene (a disease characterized by gum necrosis and bleeding). The tongue coating is yellow, and the pulse is rapid. If allowed to spread unchecked, there is a risk of cheek perforation and tooth loss, indicating a severe condition. Tentatively prescribe Lulu Xiaogang Yin (Aloe Vera Decoction for Eliminating Gangrene) with modifications to clear the Yangming meridian heat and resolve heat-toxins.

This has persisted for half a year; concern arises regarding everted flower-like bleeding. There is soreness and weakness in the lower body.
Prescribe Nourishing Ying and Clearing Upper methods.

Shen, Right: Re-exposure to misty pathogens triggers latent warm pathogens, manifesting externally as warm toxins. The face is red and swollen, extending to the back of the head; lumps form at the root of the ears and persist without resolving. There is aversion to cold with fever, which resolves after sweating beyond the usual time. Chest fullness and lack of appetite are present. The tongue coating is thin, greasy, and slightly yellow. The pulse is wiry-rapid on the left and soggy-rapid on the right. Concern exists that the condition may become chronic and worsen. Tentatively prescribe clearing and resolving latent warm pathogens, transforming phlegm, and removing blood stasis.

Second Visit: Recurrence of Dato Wen (Big Head Plague). The face is red and swollen with pain. Chills and fever occur twice daily, resolving with sweating. Chest fullness and lack of appetite persist. Dry mouth but little desire to drink. Ear root lumps persist. Tongue coating is thin and greasy. Pulse is wiry-rapid on the left and soggy-rapid on the right. Latent warm pathogens and seasonal qi reside in the collaterals of Shaoyang and Yangming. Warmth originates internally. As Wu Keyi stated: "In treating warm diseases, there are cases of inducing sweating again after initial sweating." Although the constitution is weak, nourishment should not be used blindly, fearing retention of pathogens. Yesterday, Puji Xiaodu Yin (Puji Detoxification Decoction) with modifications was effective. It is advisable to maintain the original method.

Third Visit: After Dato Wen, facial redness has not subsided. Upon waking, there is occasional feeling of hot flashes, which subside after some time. Tongue coating is dry, white, and greasy. Pulse is wiry-rapid on the left and soggy-slippery on the right. Residual warmth lingers in the Shaoyang and Yangming collaterals, stirring up Jueyin Liver Yang ascent. Phlegm-dampness obstructs the Middle Jiao. Yangming descending function fails, leading to reduced food intake and short, dark urine. This is the cause. Nourishing Yin retains pathogens; drying dampness injures Yin, causing imbalance. Again, prescribe clearing and draining latent warmth as the main approach, assisting with dispersing phlegm-dampness.
Shuang Sangye (Mori Folium, frosted, 3 qian) Sheng Chishao (Paeoniae Radix Rubra, 2 qian) Chi Fuling (Poria with Red Hostwood, 3 qian) Xiakucao (Euphorbiae Humifusae Herba, 1.5 qian) Chu Juhua (Chrysanthemi Indici Flos, 3 qian) Lianqiao Qiao (Forsythiae Fructus, 3 qian) Fu Zexie (Alismatis Rhizoma, 1.5 qian) Ku Bi Zhu (Bamboo charcoal, 3 qian) Bohe Tan (Menthae Haplocalycis Herba charred, 8 fen) Qing Mabo (Polyporus Mass, light, 8 fen) Xiang Beimu (Fritillariae Cirrhosae Bulbus, 3 qian) Xian Zhuru (Bambusae Caulis in Taenia, fresh, 1.5 qian) Jinyinhua Lu (Lonicerae Japonicae Infusion, added later, 6 liang)
Fourth Visit: Yesterday's prescription of clearing latent warmth and dispersing phlegm-dampness slightly reduced facial redness and hot flashes. Food intake remains low. Tongue is dry, white, and greasy. Residual dampness lingers in Yangming collaterals; Jueyang easily ascends. Phlegm-dampness mutually obstructs the Middle Jiao; Spleen and Stomach transportation power is lacking. Efficacy is seen; maintain the original intent with modifications. Although Yin deficiency exists, nourishment is contraindicated. Once latent warmth is cleared, Yin will recover naturally.

Fifth Visit: Facial redness gradually recedes. Hot flashes and aversion to cold come and go. Chest fullness and discomfort persist. Food intake is reduced. Tongue center is slightly peeled, posterior part is thin and greasy. Pulse is soggy-small on the left and soggy-slippery on the right. Yin deficiency is inherent; Liver meridian Qi-fire easily ascends. Damp-phlegm obstructs the middle; Stomach loses harmonious descent. Collateral dampness is not yet cleared; Ying and Wei lose their normal rhythm. Today, prescribe clearing and draining Jueyang, harmonizing Stomach, and transforming phlegm. After latent warmth is pacified, proceed with nourishing Yin and subduing Yang.

Zhu, Left: Head and face swollen like a bucket, chills and fever, dry mouth, sore throat, and fu-organ constipation indicate a severe case of Dato Wen (Big Head Plague). The head is the summit of all Yang meridians; only wind can reach it. Wind is the Yang Qi of Heaven, attacking the Upper Jiao first. Liver and Stomach fire ascend along with it, affecting all three Yang meridians. Prescribe Puji Xiaodu Yin (Puji Detoxification Decoction) with modifications.

Tao, Right: Head and face diffusely swollen and red, chills and fever alternating day and night. Previous physicians prescribed Chengqi Tang (Major Order the Qi Decoction); after three doses, symptoms remained unchanged. The upper half of the body is where Heaven's Qi gathers, the pivot of all Yang. External wind-warm pathogens trigger Shaoyang Gallbladder fire ascending, filling the clear orifices. The pure Yang region becomes like a land of clouds and mist. Chengqi Tang purges excess heat from the Stomach; since the disease is in the Upper Jiao, unprovoked purging causes the condition to progress rather than retreat. Dong Yuan's Puji Xiaodu Yin (Puji Detoxification Decoction) is specifically designed for this disease. Use with modifications to observe progress.

Du, Left: At the vertex, only wind can reach. Wind-warm epidemic pathogens reside in the Upper Jiao. Dato Wen presents with head and face redness, swelling, pain, high fever, dry mouth, dark urine, and constipation. Tongue coating is thin and greasy; pulse is stagnant, slippery, and rapid. Wind is Yang; warmth transforms into heat, spreading like smoke and fog through the clear spaces, steaming Yangming. The condition is not mild. Urgently prescribe Puji Xiaodu Yin (Puji Detoxification Decoction) with modifications to clear wind pathogens and unblock fu-organs. Follow the classic principle: "For fire depression, vent it"; "For knots, scatter them." In warm diseases, purging is not considered too early.

One dose unblocked the fu-organs; remove Chuanjun. Three doses resulted in cure.
Chen, Left: Dato Wen presents with head and face redness, swelling, pain, intense fever, thirst with desire to drink, splitting headache, delirium at night, gray and rough tongue, and surging-rapid pulse. Seasonal epidemic pathogens reside in the Upper Jiao; epidemic evil transforms into fire, transmitting to the interior of Yangming. Body fluids are damaged; Jueyang stands alone in hyperactivity. Fear of syncope! Urgently prescribe generating fluids and clearing warmth to control the flame.
