Pediatric patients presenting with Chongshe (double tongue/swelling under the tongue resembling a second tongue) and Mutong (wooden tongue/stiff and swollen tongue) suffer from heat accumulation in the Heart and Spleen attacking the center. A second layer forms under the tongue, causing the tongue to become stiff, hard, swollen, and painful. Urgently puncture with a needle to release blood; the flow of fresh red blood is acceptable. Applying Calcined Alum (Kufan) has miraculous effects, resolving heat toxicity and eliminating wind pathogens.
The upper teeth belong to the Stomach, the lower teeth belong to the Large Intestine, and the teeth themselves belong to the Kidney.
Delayed tooth eruption indicates insufficient Kidney Qi. The Kidney governs bone, and teeth are considered the surplus of bone. Insufficient Kidney Qi leads to marrow deficiency; deficient marrow fails to nourish the teeth, resulting in delayed eruption. Treat with Dihuang Wan (Rehmannia Pill).
Swelling of the upper and lower gingiva indicates Excess Heat in the Yangming channel. Use Liangge San (Cool the Diaphragm Powder), with Wine-processed Rhubarb (Jiu Zheng Dahuang) as the sovereign drug, supplemented by Anemarrhena Rhizome (Zhimu), Gypsum (Shigao), and Bupleurum Root (Shengma) as minister drugs. Frequently gargle and swallow.
Chongyin (gingival swelling covering the tooth root) is caused by accumulated heat in the Kidney. Swelling and pain of the gums are termed Chongyin. Puncture with a needle to release blood, clean with salt water, and apply Powder of Phellodendron Bark (Huangbai).
Excessive meat consumption in children can cause severe halitosis from the teeth, indicating Heat in the Yangming channel. Internally administer Shenxiao Wan (Miraculous Effect Pill); externally, gargle and swallow with Ginger Juice (Jiangzhi) and Thunberg Fritillary Bulb juice (Jingli).
Yagan (gangrenous stomatitis) resembles the symptoms of Huhu disease. Initially, there is foul odor, followed by blackening of the teeth. In severe cases, the gingiva rots and bleeds, named Xuanlu. This is due to Kidney heat rushing to the Upper Jiao. It is compared to a "running horse" to signify rapid progression, requiring immediate treatment. If the upper and lower lips crack, the nose perforates, or teeth fall out, it is named Bingsha. Accompanied by shortness of breath, phlegm surging, and reduced appetite, it becomes incurable. Internally administer Huangbai Wan (Phellodendron Pill); externally apply Rusheng San (Like-Sage Powder).
Teeth grinding (bruxism) is caused by Wind-Heat. Accumulated heat in the Yangming and Large Intestine channels generates wind when heated excessively, causing teeth to clash and produce sound. This typically occurs during sleep because movement scatters wind to the exterior, while stillness allows wind to return to the interior. Treat primarily with Xuanfeng San (Dispel Wind Powder).
Sores on the upper lip where worms erode the viscera are called Huo (Confusion). Sores on the lower lip where worms erode the anus are called Hu (Fox). This terminology is derived from *Shanghan Zhangzhang* (Guiding Palms for Cold Damage). The terms Fox and Confusion imply hesitation and indecision.
Bruxism is critical only when occurring in the context of variola (smallpox/variolae); otherwise, it poses no major harm. There are also cases of teeth chattering due to illness-induced tremors; treating the underlying disease will stop it naturally.
Teeth falling out without regrowth is caused by the tongue licking the wound, leading to thickened, stubborn tissue. Puncture with a needle to release blood, then apply Shugu San (Rat Bone Powder) to stimulate regrowth.
[Wenge San]
Treats Yagan (gangrenous stomatitis).
Realgar (Xionghuang) and Calcined Alum (Kufan), each 5 fen; Chinese Gall (Wubeizi) 2 qian; Silkworm Cocoon Paper (Can tui zhi, burnt to ash but retaining its nature) 1 qian. Grind into powder. First, wash the affected area with rice washing water, then apply the medicine. Apply three to four times daily until healed.
