The nephew of Grand Secretary Liang suffered from swelling, pain, and continuous bleeding due to a metal weapon injury, accompanied by alternating chills and fever, and dry mouth. This indicates Qi deficiency causing blood to lose its attachment and leave the vessels. Treat with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), adding Wuweizi (Schisandra Berry) and Maimendong (Ophiopogon Tuber), to restore Yang Qi and achieve recovery. (Formula see Abscess and Carbuncle section.)
Regarding Tetanus (Poshang Feng), Liu Hejian stated: Wind pathogens are agile and change rapidly; upon entering the zang organs, death or survival can occur in an instant. One must urgently differentiate between exterior/interior and excess/deficiency patterns for treatment.
If the pathogen is at the exterior level in tetanus, symptoms include muscle spasms, intermittent chills and fever, muscle twitching and convulsions, and a floating, wiry pulse. Dispersing methods should be used.
Qianghuo Fangfeng Tang (Notopterygium and Saposhnikovia Decoction): Treats tetanus where the pathogen is initially at the exterior. Administer urgently to resolve the exterior condition. Delay allows the pathogen to enter the interior, rendering the medication ineffective.
Qianghuo (Notopterygium Root), Fangfeng (Saposhnikovia Root), Gancao (Licorice Root), Chuanxiong (Sichuan Lovage Rhizome), Ben Cao (Ligusticum Wallichii Rhizome), Danggui (Angelica Sinensis Root), Baishao (White Peony Root, 1 qian each), Diyu (Sanguisorba Root), Xixin (Asarum Root, 0.5 qian each).
Cut one dose, decoct in water, and take after meals.
If the pathogen is at the semi-exterior/semi-interior level in tetanus, symptoms include slight sweating on the head but no sweating on the body. Harmonizing methods should be used.
Qianghuo Tang (Notopterygium Decoction): Treats tetanus at the semi-exterior/semi-interior level. Administer urgently. If delayed, the pathogen enters the interior, and this formula is contraindicated.
Qianghuo (Notopterygium Root), Juhua (Chrysanthemum Flower), Mahuang (Ephedra Herb), Chuanxiong (Sichuan Lovage Rhizome), Shigao (Gypsum Fibrosum), Fangfeng (Saposhnikovia Root), Qianhu (Peucedanum Root), Huangqin (Scutellaria Root), Xixin (Asarum Root), Gancao (Licorice Root), Zhike (Bitter Orange Fruit), Bai Fuling (Poria, 0.5 qian each), Jingjie Zi (Schizonepeta Seed), Bohe (Mint Leaf, 0.25 qian each), Baizhi (Angelica Dahurica Root).
Cut one dose, decoct in water, and take.
If the pathogen has entered the interior in tetanus, symptoms include stiff tongue, locked jaw, opisthotonos (arching of the back), muscle twitching and convulsions, profuse phlegm-saliva, chest and abdominal fullness, constipation and dark urine, occasional bleeding, and a surging, rapid, and wiry pulse. Guiding/draining methods should be used.
Daxiong Huang Tang (Major Cyperus and Rhubarb Decoction): Treats tetanus with the pathogen in the interior. Use guiding/draining methods and administer urgently.
Chuanxiong (Sichuan Lovage Rhizome), Qianghuo (Notopterygium Root), Huangqin (Scutellaria Root), Dahuang (Rhubarb Root, 3 qian each).
Cut one dose, decoct in water, take warm, until the internal organs are unobstructed and harmonious.
A person was beaten in a fight, suffering from a broken brow ridge infected with tetanus, resulting in swollen and hot head and face. Administer Jiwei Qianghuo Tang (Nine-Ingredient Notopterygium Decoction) hot to induce sweating. Externally, crush Almond (Xingren), mix with a little wheat flour, apply with fresh cold water to the wound. Swelling subsided and heat resolved.
Jindao Rusheng San (Golden Knife Like Sage Powder): Treats tetanus.
Cangzhu (Atractylodes Rhizome, 8 qian), Baizhi (Angelica Dahurica Root), Chuanxiong (Sichuan Lovage Rhizome), Xixin (Asarum Root), Mahuang (Ephedra Herb, 5 qian each), Chuanwu (Aconitum Carmichaelii Lateral Root, processed), Caowu (Aconitum Kusnezoffii Root, processed, 4 qian each), Bohe (Mint Leaf, 1 qian).
Grind into powder. Take 1 qian per dose, mixed with hot yellow wine, cover to induce sweating throughout the body for efficacy. For treating gout, add 1 qian of Milixiang (Milky Frankincense).
Yuzhen Gao (Jade Truth Ointment): Treats tetanus, sharp instrument injuries, contusions, and mad dog bites. Relieves pain and promotes tissue regeneration.
Tiannanxing (Arisaema Rhizome, processed with Fangfeng so it does not cause numbness), Fangfeng (Saposhnikovia Root, equal parts).
Grind into powder. For tetanus, apply powder to the wound, then take 1 qian mixed with warm wine. For trismus (locked jaw) or opisthotonos, take 1 qian mixed with urine from a young boy.
First: For severe beating near death, if the heart region remains warm, pour down 2 qian of urine from a young boy, followed by two more doses.
Second: For mad dog bites, rinse with Jiangshui (fermented grain water), wipe dry with cotton, apply medicine. It prevents recurrence and is effective for wounds without suppuration.
Another Formula: Treats tetanus.
Stir-fry 1 he of Sophora fruit (Huai Zi), decoct in one bowl of good yellow wine until reduced to 0.8 volume. Take hot; sweating indicates recovery.
Another Formula: Treats tetanus.
Grind half-raw and half-stir-fried Perilla seed (Yezisu) into powder, form pills with refined honey the size of a finger. Take one pill per dose with hot yellow wine.
External Treatment Methods for Tetanus:
For falls causing head/face fractures and knife wounds causing heavy bleeding from hands/feet: Take Qingxi (Bitumen/Pine Rosin) in any amount, grind into fine powder. Pinch the broken wound edges together, apply a thick layer of the powder, and bind tightly with clean cloth. Immune to wind and water; recovers in ten days.
Treat Tetanus: Equal parts Gancao (Licorice Root) and Gansui (Kansui Root), ground into powder. Mix with honey and old scallion bulbs from last year into a paste. Lift the scab slightly, sprinkle a little Shexiang (Musk) first, then apply the medicine. Burn moxa stick up to four cun long; whole-body sweating indicates recovery.
Moxibustion Method: Treats tetanus and mad dog bites; this method is easy and highly effective.
Take half a walnut shell, fill densely with human feces, press Huaimupi (Sophora Bark) against the wound, and perform moxibustion on the walnut shell. If the patient sweats all over and feels exhausted, they will recover. For chronic cases, moxibustion on the wound alone suffices.
