The patient experiences pain on one side of the waist and hypochondrium, extending from the big toe to the upper aspect of the dorsum of the foot, ascending along the medial aspect of the lower leg, encircling the genital region, reaching the lower abdomen, and causing distending pain in areas around the umbilicus. The feet are cramped and urgent, the throat is dry, there is vomiting and regurgitation of qi, and diarrhea with watery stools. This is caused by the flow and accumulation of the four pathogenic factors (wind, cold, dampness, heat) in the Foot-Jueyin Liver Meridian (Foot-Jueyin Liver Meridian: a meridian pathway associated with the liver). The predominance of the four pathogenic factors is as described previously. Treatment should be adjusted according to the severity of the pathogenic invasion.
To treat the Foot-Jueyin Liver Meridian invaded by the four pathogenic factors affecting the liver, resulting in hemiplegia on either the left or right side, flooding of phlegm saliva leading to stupor and obstruction, hemiplegia, numbness and stiffness of the hands and feet, slurred speech, dizziness and vertigo, locked jaw, asthma with spontaneous sweating, mental confusion, weakness of the limbs, pathogenic factors attacking upwards to the head and eyes and downwards to the legs and knees, stagnation of Ying Qi (Nutritive Qi), and generalized body pain. It also treats wind-stroke in women after childbirth, opisthotonos (body arching backward like a bow), injuries from falling off vehicles or horses, bruises and trauma, and internal blood stasis. Ingredients include: Danggui (Angelicae Sinensis Radix, soaked in wine), Tianma (Gastrodiae Rhizoma), Chuanxiong (Ligustici Chuanxiong Rhizoma), Qianghuo (Notopterygii Rhizoma et Radix), Baishao (Paeoniae Radix Alba), Shudihuang (Rehmanniae Radix Praeparata) (all in equal proportions; another method excludes Qianghuo and adds Mugua (Chaenomelis Fructus) and Ejiao (Colla Corii Asini) in equal proportions). Grind the above ingredients into powder, mix with honey to make pills, sized like chicken egg yolks. Take one pill per dose. For leg bi-syndrome (numbness and pain in the legs), take with wine infused with Mugua and Tusizi (Cuscutae Semen); for wind-stroke, take with warm rice gruel.
In Shanghan (Cold Damage) disease, the Three Yang meridians often present combined syndromes, whereas the Three Yin meridians do not, due to differences in zang-fu organs; however, this does not prevent the transmission of the disease. Hongxiangjiao (Hongxiangjiao: a condition characterized by swelling and weakness of the lower legs, often associated with beriberi or similar neuropathies) is different. It arises because pathogens linger and obstruct the qi of the zang-fu, flowing and accumulating according to the deficiency, excess, cold, or heat of the constitution. Therefore, the symptoms often present as combined syndromes, which must be understood.
To treat deficiency of the Liver and Kidney zang-organs, where Wind-Dampness invades and flows into the legs and knees, causing difficulty walking, gradually developing into Wind-Damp Hongxiangjiao, with burning sensation in the soles of the feet, upward rushing of qi with rapid breathing, insensitivity in the lower abdomen, and complete loss of appetite. Ingredients include: Chixiaodou (Adzuki Bean, lightly stir-fried), Wulingzhi (Trogopteri Dung), Baijiaoxiang (Styrax), Poguzhi (Cassiae Semen, stir-fried), Gouji (Rhododendri Daurici Caulis), Mibiezi (Strychni Semen, shelled), Haitongpi (Pteroceltidis Cortex), Weilingxian (Agrimoniae Herba), Dilong (Pheretima, stir-fried), Caowu (Aconiti Radix, soaked in rice water for three days, washed clean, skin and tips removed). Each ingredient is taken in one liang, ground into powder, mixed with wine paste to form pills sized like wutong seeds, coated with Chen Sha (Cinnabar). Take fifty pills per dose, freely with salted wine on an empty stomach.
