Thrombophlebitis, various ancient texts generally attribute this condition to the infection of damp-toxin (Shi Du), leading to Qi and Blood stasis and disharmony of the collaterals. Therefore, in the acute phase, treatment often focuses on clearing heat, draining dampness, and detoxifying, assisted by resolving phlegm and unblocking collaterals. However, clinical verification reveals that while this method is effective for patients with lower limb involvement, its efficacy is minimal for other body parts. After repeated verification, it is believed that the main reason lies in the failure to distinguish between Zang-Fu organs and meridians. A patient, Wang XX, male, 63 years old, was treated. Two months prior, he suddenly developed low-grade fever and pain in the right hypochondrium, where a cord-like mass approximately 10 cm long could be palpated. He urgently sought surgical consultation at a certain hospital and was diagnosed with thrombophlebitis. He underwent antibiotic treatment for over 40 days and took more than ten doses of Chinese herbal medicine aimed at clearing heat, detoxifying, draining dampness, and unblocking collaterals, but with no significant effect. Upon reviewing his pulse and symptoms, aside from the aforementioned signs, his spirit and complexion were still good, the tongue coating was yellow and greasy, and the pulse was wiry and slippery. Synthesizing the pulse and symptoms, it was determined that the previous therapeutic principles were basically correct, but the differentiation of meridians and Zang-Fu organs was not sufficiently aligned. Thus, the strategy of soothing the Liver, resolving phlegm, broadening the chest, and detoxifying was adopted. The prescription consisted of: Chai Hu (Bupleuri Radix) 15g, Ban Xia (Pinelliae Rhizoma) 15g, Gua Lou (Trichosanthis Fructus) 40g, Su Mu (Sappan Lignum) 6g, Chi Shao (Paeoniae Radix Rubra) 15g, Huang Qin (Scutellariae Radix) 10g, Qing Pi (Citri Reticulatae Pericarpium Viride) 10g, Ju Ye (Citri Reticulatae Folium) 10g, Xia Ku Cao (Prunellae Spica) 30g. After two doses, there was no improvement or worsening, but night toothache appeared. Analyzing the syndrome, the night toothache was attributed to Yin deficiency with hyperactive fire. Yuan Shen (Scrophulariae Radix) 30g was added to the original formula. After six doses, the pain decreased significantly. Dang Gui (Angelicae Sinensis Radix) was then added to invigorate blood within the blood level, and after continuing for 12 doses, the patient recovered.
Mastitis (Ru Yong)
A patient, Zhang XX, female, 22 years old, was treated. She suddenly developed swelling and pain in the breast. The physician diagnosed mastitis. Emergency treatment included antibiotics and Chinese herbal medicine for clearing heat and detoxifying, along with instructions to apply hot compresses. After two days of treatment, not only was there no effect, but the swelling and pain worsened, accompanied by alternating chills and fever, nausea, and a body temperature of 39°C. I was invited to treat her. Examination revealed redness, swelling, and burning heat in the affected breast, feeling hot to the touch. I urgently advised changing from hot to cold compresses, frequently applying towels soaked in cold water, replacing them with cold ones as soon as they warmed up. Within two hours, the towels were changed about 40 times. The pain, swelling, and chills/fever decreased significantly, and the body temperature dropped to 38.2°C. She was instructed to continue taking the previous medication and recovered.
Another patient, Geng XX, female, adult, presented with swelling and pain in the left breast accompanied by chills and high fever for over ten days. The physician used antibiotics and large doses of Chinese herbs for clearing heat and detoxifying, such as Yin Hua (Jasmini Flos), Lian Qiao (Forsythiae Fructus), Pu Gong Ying (Taraxaci Herba), Cao He Che (Smilacis Glabrae Rhizoma), Xia Ku Cao (Prunellae Spica), Huang Qin (Scutellariae Radix), and Huang Lian (Coptidis Rhizoma), but without effect. Examination showed a body temperature of 39.8°C, redness, swelling, and pain in the breast with fluctuation upon pressure, and a wiry, slippery, and rapid pulse. Synthesizing the pulse and symptoms, it was concluded that Liver Qi stagnation led to untransformed phlegm-heat, blocking the collaterals, causing congestion of Ying Qi, and heat rotting flesh into an abscess. The treatment should focus on detoxifying, eliminating carbuncles, resolving phlegma, dissipating nodules, regulating Qi, and invigorating blood. The prescription included: Chai Hu (Bupleuri Radix) 21g, Qing Pi (Citri Reticulatae Pericarpium Viride) 15g, Chi Shao (Paeoniae Radix Rubra) 15g, Ju Ye (Citri Reticulatae Folium) 15g, Gua Lou (Trichosanthis Fructus) 60g, Jie Geng (Platycodonis Radix) 30g, Zhi Ke (Aurantii Fructus Immaturus) 15g, Pu Gong Ying (Taraxaci Herba) 30g, Yin Hua (Jasmini Flos) 15g. After one dose, pain and fever decreased significantly; after two doses, fever disappeared and swelling/pain reduced greatly; after continuing for 10 doses, she recovered. An intern asked: "Clearing heat and detoxifying is the orthodox method for eliminating carbuncles; why did it fail here?" I replied: "Because the stagnant Qi was not resolved. If stagnation is not resolved, pathogenic toxins become even more congested; merely providing detoxification cannot resolve the toxin."
E Zhuo Feng (Hand Eczema/Tinea Manuum)
E Zhuo Feng is a common dermatological disease. Since it is a skin disease, many advocate external treatment, with some believing internal treatment is unnecessary. For example, *TCM Surgery* states: "This disease does not require internal treatment," advocating the use of Feng You Gao, Hong You, and smoke fumigation methods. However, some patients who frequently used external washes and applications saw no effect. Clinical trials show that internal medication often brings about a sudden cure. For instance, Patient Li XX, male, 30 years old, suffered from dry, cracked, and peeling skin on both palms for three years, worsening in the last year. Long-term external use of Chinese and Western medicines failed. Examination revealed numerous rice-grain-sized vesicles on the palmar surfaces and interdigital spaces, which broke and peeled off, recurring continuously with unbearable itching. The non-peeling skin was thickened with fissures. The tongue coating was thin and white, the tongue body tender and red, and the pulse deep and fine. Synthesizing the pulse and symptoms, the diagnosis was dry-heat in the blood with external wind-pathogen invasion. The strategy was to nourish blood, moisten dryness, disperse wind, and dispel cold. Modified Qu Feng Di Huang Wan: Sheng Di (Rehmanniae Radix) 12g, Shu Di (Rehmanniae Radix Praeparata) 12g, Bai Ji Li (Tribuli Fructus) 9g, Chuan Niu Xi (Cyathulae Radix) 9g, Tu Si Zi (Cuscutae Semen) 9g, Zhi Mu (Anemarrhenae Rhizoma) 6g, Huang Bo (Phellodendri Cortex) 6g, Gou Qi Zi (Lycii Fructus) 6g, Du Huo (Angelicae Pubescentis Radix) 3g. Itching decreased after one dose, and recovery occurred after 10 doses.
Jiao Shi Qi (Foot Tinea)
Jiao Shi Qi, also known as Chou Tian Luo, is traditionally attributed to damp-fire attacking from the Foot Yangming Stomach Meridian, treated with clearing heat, draining dampness, killing parasites, and stopping itching. However, some cases do not respond to this method. For example, Patient Liu XX, female, 50 years old, had thickened, itchy skin on the sole, lateral heel, and between toes for over two years, worsening since summer. Diagnosed with tinea pedis at a hospital, external Western medicine failed, and subsequent external Chinese herbal washes/applications worsened the maceration. Examination showed complete maceration of the sole, redness and swelling of the dorsum, severe odor, hot pain in the sole preventing walking, pale tongue with white coating, and deep, fine pulse weak at the chi position. Initial treatment with modified San Miao Tang and Bi Xie Shen Shi Tang for seven days worsened swelling, pain, and maceration. Reflecting on this, although damp-heat is common, prolonged use of damp-draining and heat-clearing methods fails here and damages Yang Qi. Further attack would deplete healthy Qi and exacerbate dampness. Considering the onset at Yongquan (KI 1), a point of the Kidney Meridian, and the deep, fine, weak chi pulse indicating Kidney Qi deficiency, urgent tonification of Kidney and Mingmen (Gate of Life) Qi was needed to eliminate dampness. Modified Shi Wei Di Huang Tang: Sheng Di (Rehmanniae Radix) 24g, Shan Yao (Dioscoreae Rhizoma) 12g, Wu Wei Zi (Schisandrae Fructus) 9g, Fu Ling (Poria) 9g, Ze Xie (Alismatis Rhizoma) 9g, Dan Pi (Moutan Cortex) 9g, Fu Zi (Aconiti Lateralis Radix Praeparata) 9g, Rou Gui (Cinnamomi Cortex) 9g, Yuan Shen (Scrophulariae Radix) 15g, Mai Dong (Ophiopogonis Radix) 15g, Che Qian Zi (Plantaginis Semen) 12g (wrapped), Huai Niu Xi (Achyranthis Bidentatae Radix) 10g. Symptoms improved after one dose, and recovery occurred after eight doses.
Xue Feng Chuang
Chen Shigong stated: "Xue Feng Chuang arises from the interaction of wind-heat, damp-heat, and blood-heat, causing extreme itching, rupture with exudation of serum, and gradual spreading." Differentiating the proportions of these factors is difficult. I use the principle of Heaven-Man correspondence to distinguish them. For example, Patient Zhao XX, female, 38 years old, had continuous peeling and itching of palms for five years, unable to wash clothes. Suddenly, generalized itching started two months ago, worse at night, with serum and blood exuding upon scratching. Diagnosed with tinea manuum and seborrheic dermatitis. Western medicine failed, and subsequent damp-draining, wind-dispersing, heat-clearing herbs were ineffective. Examination showed generalized papules, scratch marks everywhere, especially at axillae, inframammary folds, umbilicus, interscapular region, antecubital fossa, and perianal area. Worse at night, insomnia, palm peeling, small vesicles, and fissures on thenar/hypothenar eminences bleeding. Tongue coating thin white, pulse wiry and fine. Diagnosis: Blood dryness generating wind. Strategy: Nourish blood, moisten dryness, invigorate blood, disperse wind. Modified Di Huang Yin Zi: Dang Gui (Angelicae Sinensis Radix) 12g, Sheng Di (Rehmanniae Radix) 15g, Yuan Shen (Scrophulariae Radix) 30g, Shu Di (Rehmanniae Radix Praeparata) 10g, Dan Pi (Moutan Cortex) 10g, Sheng He Shou Wu (Polygoni Multiflori Radix) 15g, Bai Ji Li (Tribuli Fructus) 10g, Jiang Can (Bombyx Batryticatus) 10g, Hong Hua (Carthami Flos) 10g, Gan Cao (Glycyrrhizae Radix et Rhizoma) 6g, Du Huo (Angelicae Pubescentis Radix) 6g, Gou Qi Zi (Lycii Fructus) 10g, Zhi Mu (Anemarrhenae Rhizoma) 10g. Symptoms decreased significantly after six doses, allowing 1-2 hours of sleep, but palm peeling persisted. After 12 more doses, all symptoms disappeared, and she recovered. Another patient, Liu XX, female, 50 years old, had dry hair/skin and body itching when undressing before bed for over ten years, worsening recently. Generalized severe itching on head, face, limbs, shoulders, waist, eyelids, neck, and perineum. Diagnosed with dermatitis. Local anti-itch agents, sedatives, calcium salts failed. TCM doctor diagnosed wind-damp-heat toxin, prescribed wind-dispersing, damp-draining, heat-clearing, detoxifying herbs for six doses, worsening itching. Examination showed widespread scratch marks and blood scabs, restlessness, wiry-fine pulse. Considering worsening at night (Yin time), it was Blood Dryness Generating Wind. Prescribed Di Huang Yin Zi: Sheng Di (Rehmanniae Radix) 25g, Shu Di (Rehmanniae Radix Praeparata) 9g, Sheng He Shou Wu (Polygoni Multiflori Radix) 10g, Dang Gui (Angelicae Sinensis Radix) 10g, Hong Hua (Carthami Flos) 10g, Yuan Shen (Scrophulariae Radix) 30g, Bai Ji Li (Tribuli Fructus) 10g, Jiang Can (Bombyx Batryticatus) 10g, Gan Cao (Glycyrrhizae Radix et Rhizoma) 6g. Itching decreased after one dose, reduced by 80-90% after two doses, and resolved after five more doses. As the *Neijing* says: "Since antiquity, those connected to Heaven are the root of life." When differentiation is unclear, seeking guidance from the Heaven-Man correspondence theory has great value.
Bai Dian Feng (Vitiligo)
Bai Dian Feng is often caused by wind-pathogen lingering on the skin, leading to disharmony of Qi and Blood over time. Chen Shigong said: "Zi Bai Dian Feng are two forms of one entity. Purple is due to Blood stasis, White is due to Qi stasis, both caused by wind-damp invading a heat-prone body, condensing in pores, blocking Qi and Blood flow." Following this theory, treating vitiligo patients has been very effective. For example, Fan XX, female, 19 years old, had over 40 white patches on limbs and trunk for over a year, ranging from soybean to palm size. Eight months of Chinese/Western medicine failed. Tongue coating white slippery, pulse floating. Strategy: Diffuse Lung, release exterior, drain dampness, clear heat, assist Kidney tonification. Modified Ma Huang Lian Qiao Chi Xiao Dou Tang: Ma Huang (Ephedrae Herba) 9g, Lian Qiao (Forsythiae Fructus), He Shou Wu (Polygoni Multiflori Radix), Hei Zhi Ma (Sesami Fructus Niger), Sang Bai Pi (Mori Cortex) each 15g, Chi Xiao Dou (Adzuki Bean) 30g, Bai Ji Li (Tribuli Fructus) 12g, Sheng Jiang (Zingiberis Rhizoma Recens) 3 slices, Da Zao (Jujubae Fructus) 5 pieces. After 12 doses, some skin darkened, some hair follicles returned to normal color. After 21 more consecutive doses, skin color normalized, and she recovered.
Ancients cured it with Bai Ji Li powder, Bai Dian Feng Wan, or Bu Gu Zhi wine externally. Observing clinical cases, chronic Kidney deficiency requires tonifying Liver/Kidney; pure wind-pathogen requires Bai Ji Li; wind-damp-heat requires Ma Huang Lian Qiao Chi Xiao Dou Tang, supplemented by methods addressing exterior/interior, cold/heat, deficiency/excess. This case's success lies in this comprehensive approach.
Shi Du Chuang and Shi Zhen (Eczema)
Eczema has acute and chronic types. Acute with obvious erosion/exudation is called Jin Yin Chuang or Huang Shui Chuang in TCM; generalized papules are Su Chuang. Infant eczema is Nai Xuan. Chronic eczema is Shi Du Chuang or Shi Qi Chuang. Treatment for acute cases often uses damp-draining, wind-dispersing, heat-clearing methods like Bi Xie Shen Shi Tang, Jia Wei Er Miao San, Fang Feng Tong Sheng San. Chronic cases may have theories of wind-dispersing/damp-draining, but often involve healthy Qi deficiency, Spleen Qi damage impairing Middle Jiao ascending/descending, or San Jiao failing to transform dampness; or excessive damp-draining causing Blood dryness generating wind, leaving pathogenic toxins unresolved. Sheng Yang Yi Wei Tang contains "Liu Jun Zi Tang assisting Yang, tonifying Spleen, resolving phlegm, heavy Huang Qi tonifying Qi and securing Stomach; Qiang Huo, Du Huo, Chai Hu draining dampness and raising Yang; Ze Xie, Fu Ling draining heat and lowering turbidity; adding Shao He harmonizing blood and astringing Yin; slight Huang Lian retreating Yin fire." "Releasing with containment, tonifying with dispersing" (Wang Ang). Thus, it is consistently effective for Spleen deficiency with damp-stagnation. For blood dry-heat, nourishing blood, invigorating blood, and moistening dryness with modified Si Wu Tang is appropriate; do not add drying herbs due to its moistening nature, lest wind arise. For example, Zhang XX, male, 50 years old, had recurrent eczema on head/neck for 19 years. Long-term treatment failed, leading to epigastric pain, diarrhea, poor appetite, irritability, palpitations, sweating, frequent colds. Examination showed scattered eczema on head/forehead/neck, some coalescing with yellow purulent crusts, slight yellow fluid upon removal, itchy but not painful, yellow greasy tongue coating, slightly large wiry pulse. Prescribed Sheng Yang Yi Wei Tang: Huang Qi (Astragali Radix) 15g, Dang Shen (Codonopsis Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Huang Lian (Coptidis Rhizoma), Ban Xia (Pinelliae Rhizoma), Gan Cao (Glycyrrhizae Radix et Rhizoma), Chen Pi (Citri Reticulatae Pericarpium), Fu Ling (Poria), Ze Xie (Alismatis Rhizoma), Fang Feng (Saposhnikoviae Radix), Qiang Huo (Notopterygii Rhizoma et Radix), Du Huo (Angelicae Pubescentis Radix), Chai Hu (Bupleuri Radix), Bai Shao (Paeoniae Radix Alba) each 9g, Sheng Jiang (Zingiberis Rhizoma Recens) 3 slices, Da Zao (Jujubae Fructus) 3 pieces. After 26 doses, rash disappeared, other symptoms resolved. Geng XX, female, adult, had generalized rash for 10 years, needle-point to sorghum-grain size, untreatable by dermatology. Most prescriptions were wind-dispersing/damp-draining, but responses were ineffective or worsened itching, especially at night, requiring scratching to bleed for relief. Pulse deep and fine. I stated: Blood dryness generating wind, suitable for nourishing blood, invigorating blood, cooling blood. Dan Shen Yin Qiao Yin: Dan Shen (Salviae Miltiorrhizae Radix) 15g, Dang Gui (Angelicae Sinensis Radix), Chuan Xiong (Chuanxiong Rhizoma), Sheng Di (Rehmanniae Radix), Bai Shao (Paeoniae Radix Alba) each 10g, Yin Hua (Jasmini Flos) 12g, Lian Qiao (Forsythiae Fructus) 12g, Bo He (Menthae Haplocalycis Herba) 3g. Reduced after 4 doses, cured after 10 doses. Early eczema is often damp, but not exclusively so, especially chronic eczema.
Chan Yao Huo Dan (Herpes Zoster)
In 1976, during study away from home, I suddenly experienced incessant right hypochondriac pain, initially suspecting muscle strain. Acupuncture failed and worsened it. Urgent surgical consultation diagnosed Herpes Zoster. Vitamin B12 injection failed. That night, pain prevented sleep. Forced acupuncture on ear tragus pressure point provided immediate slight relief; twisting for half a minute greatly reduced pain; pain stopped gradually after 10 minutes, allowing sleep. Next morning, removing the needle coincided with herpes disappearance and recovery.
Que Ban (Freckles)
Chen Shigong believed freckles result from "Kidney Water failing to nourish the upper, Fire stagnating and forming spots." Qin Bowei believed they arise from "Heat stagnating in Sun Collaterals, Wind-pathogen binding externally, developing gradually." They proposed "Liu Wei Di Huang Wan to nourish the source," "Yu Rong Wan applied morning/evening," and "Shi Zhen Zheng Rong San externally." Following Qin's intent, using Chai Hu Gui Zhi Tang for Liver stagnation/Spleen deficiency, Heat stagnating in Sun Collaterals, and disharmony of Ying/Wei has shown excellent results. For example, Niu XX, female, 23 years old, had light brown/dark brown spots on face/neck for over a year, varying from pinhead to 1-2mm, round/oval. Diagnosed as freckles. Examination revealed chest fullness, irritability, epigastric distension/poor appetite, epigastric pain, menstrual irregularities, thin white tongue coating, slow wiry pulse. Prescribed Chai Hu Gui Zhi Tang plus Sheng Yi Mi (Coicis Semen): Chai Hu (Bupleuri Radix) 10g, Dang Shen (Codonopsis Radix) 10g, Ban Xia (Pinelliae Rhizoma) 10g, Huang Qin (Scutellariae Radix) 10g, Gan Cao (Glycyrrhizae Radix et Rhizoma) 6g, Da Zao (Jujubae Fructus) 5 pieces, Sheng Jiang (Zingiberis Rhizoma Recens) 4 slices, Gui Zhi (Cinnamomi Ramulus) 10g, Sheng Yi Mi (Coicis Semen) 30g. After 12 doses, chest fullness/irritability/epigastric pain improved, freckles significantly reduced. After 12 more doses, freckles disappeared, and she recovered.
You (Warts)
Patient Zhang XX, male, 8 years old, suddenly developed hundreds of warts on the body, rice-grain to sorghum-grain size, mildly itchy. One month of dermatology treatment failed. Spirit and appetite were normal. Prescribed Chai Hu Gui Zhi Tang for regulation: Chai Hu (Bupleuri Radix) 10g, Ban Xia (Pinelliae Rhizoma) 10g, Huang Qin (Scutellariae Radix) 10g, Dang Shen (Codonopsis Radix) 10g, Gan Cao (Glycyrrhizae Radix et Rhizoma) 6g, Da Zao (Jujubae Fructus) 5 pieces, Sheng Jiang (Zingiberis Rhizoma Recens) 3 slices, Gui Zhi (Cinnamomi Ramulus) 10g, Bai Shao (Paeoniae Radix Alba) 10g, Sheng Yi Mi (Coicis Semen) 30g. Cured after 4 doses.
Yang Feng (Pruritus)
Patient Luo XX, female, 10 years old, had frequent skin itching since age 1, worse at night, requiring parental scratching to sleep. Diagnosed with pruritus by multiple hospitals. Five years of Western medicine and three years of Chinese medicine failed. Over 800 doses of Chinese herbs were used, either ineffective or worsening itching. Examination showed scratch marks, rough skin, slight scaling, poor spirit, thin white tongue coating, deep fine pulse. Struggling with the case, a student asked: "Why only night itching? Why must skin be scratched to relieve?" I realized: "Heaven-Man correspondence. *Suwen: Shengqi Tongtian Lun* states: 'Yang Qi governs the exterior daily... dawn generates Qi, noon peaks Yang, evening Yang declines, pores close. Thus evening gathers/rejects...' *Suwen: Jingui Zhenyan Lun* states: 'Dawn to noon is Heaven's Yang, Yang among Yang... Night to rooster-crow is Heaven's Yin, Yin among Yin... Humans correspond. Exterior is Yang, Interior is Yin.' Insufficient Yin-Blood leads to Blood dryness generating wind. Itching is venting wind; scratching reduces it." Treatment: Nourish blood, invigorate blood, cool blood, disperse wind. Modified Dan Shen Yin Qiao Yin: Dan Shen (Salviae Miltiorrhizae Radix) 15g, Yin Hua (Jasmini Flos) 10g, Lian Qiao (Forsythiae Fructus) 10g, Dang Gui (Angelicae Sinensis Radix) 10g, Bai Shao (Paeoniae Radix Alba) 10g, Chuan Xiong (Chuanxiong Rhizoma) 6g, Bo He (Menthae Haplocalycis Herba) 3g. Itching greatly reduced after 6 doses. Student asked: "Doctor says wind causes itching, yet only Bo He disperses wind; why such effect?" I replied: "Previous failures lay here. Wind diseases use wind-dispersing herbs, but they can also injure blood. This case is Yin deficiency/Blood dryness; wind from this should be treated by nourishing blood/moistening dryness/dispatching wind, i.e., resolving through blood to Qi. Overusing Qi-level wind herbs exacerbates Blood dry-heat generating wind. Thus, follow 'treat wind by first treating blood; when blood moves, wind extinguishes.'" Continued for 24 doses and recovered.
Bai Tou Chuang (Tinea Capitis)
