Over a year after cholecystectomy, the patient frequently felt fatigue and weakness, with poor appetite. For the past 8+ months, fatigue and weakness worsened, accompanied by increasing irritability, excessive thirst and drinking, restlessness, insomnia, and soreness/heaviness in the lower back and legs. Examination at a certain hospital diagnosed Diabetes Mellitus (Tang Niao Bing). Western medicine treatment was administered for over 3 months without symptom improvement. Subsequently, Chinese herbal medicine for nourishing Yin and generating fluids was added; however, besides worsening epigastric fullness and discomfort, other symptoms did not improve. Careful examination of the syndrome revealed, in addition to excessive thirst and drinking, restlessness, insomnia, lower back/leg soreness, and epigastric fullness, frequent fatigue with drowsiness yet difficulty falling asleep, a white tongue coating, and a soggy-slow pulse (Ru Huan Mai). Synthesizing pulse and symptoms: The syndrome of Xiao Ke (Wasting-Thirst) is mostly due to Yin deficiency and fluid depletion. Why did this treatment fail? *Si Yan Ju Yao* states: "The pulse is the blood vessel, the precursor of Qi and Blood, the tunnel of Blood, responding to breath." Why not seek the answer in the pulse? The current pulse is soggy-slow. A soggy-slow pulse indicates stagnation of Phlegm-Dampness; a fine-rapid pulse indicates Yin deficiency with Fire hyperactivity; a slippery-rapid pulse indicates Phlegm-Heat or Excess Heat in the Stomach. The soggy-slow pulse combined with fatigue/soreness and epigastric fullness indicates stagnation of Phlegm-Dampness. How can one continue to use herbs that nourish Yin and generate fluids, thereby aiding Phlegm-Dampness? This is an error. The method should be tonifying Qi and nourishing Yin, regulating Qi and resolving Phlegm. Prescription: Huangqi 15g, Danggui 6g, Renshen 10g, Maimen 10g, Wuweizi 10g, Zhuru 10g, Zhishi 10g, Banxia 10g, Chenpi 10g, Fuling 10g, Changpu 10g, Yuanzhi 10g, Shengdi 10g.
After taking 7 doses, excessive thirst/desire to drink, restlessness, insomnia, and fatigue/soreness all decreased. After continuing for another 14 doses, most symptoms disappeared largely; urine sugar changed from (++++) to (-), and blood glucose normalized. Continuing the above prescription for 2 months resulted in recovery.
3. Clinging to the view of Yin deficiency and fluid depletion, stubbornly adhering to treatments that nourish Yin and generate fluids, ignoring pulse and syndrome, seeking only the disease name, leading to stagnation and lack of treatment.
Fan XX, Male, 58 years old.
Excessive thirst and drinking, polyphagia, and polyuria for over 1 year. Diagnosed with Diabetes Mellitus. Western medicine treatment for over 4 months was ineffective. Subsequently, Chinese herbal medicine for nourishing Yin and generating fluids was added; not only did symptoms not decrease, but urine sugar remained continuously at (+++). Moreover, in the last 2+ months, urine protein (++) was found, with occasional red and white blood cells and granular casts. The physician wanted to add hormone therapy but feared worsening the diabetes, so he requested the patient use only Chinese medicine. Careful examination revealed, besides excessive thirst/drinking and polyphagia/polyuria, fatigue/weakness, soreness/heaviness in the lower back, restlessness, insomnia, a white and slightly greasy tongue coating, and a soggy-slow pulse with a wiry Chi (Cun Kou position) pulse. Synthesizing pulse and symptoms: A soggy-slow pulse in chronic illness indicates simultaneous Qi and Yin deficiency with failure to transform Phlegm-Dampness; a wiry Chi pulse indicates Kidney Yang insufficiency. Treatment should tonify Qi and nourish Yin, regulate Qi and resolve Phlegm, assisted by warming the Kidney. Prescription: Huangqi 15g, Danggui 6g, Maimen 10g, Dangshen 10g, Wuweizi 10g, Zhuru 10g, Zhishi 10g, Banxia 10g, Chenpi 10g, Fuling 10g, Gancao 6g, Changpu 10g, Yuanzhi 10g, Shengdi 10g; concurrently administer Shenkangling, 4 pills each time, 3 times daily.
After 7 doses, major symptoms decreased significantly. After continuing for 25 doses, all symptoms disappeared; urine routine test became (-). Recovery followed.
A certain physician said: Diabetes is inherently difficult to treat, with limited efficacy; combining it with Diabetic Nephropathy makes it even harder. In this case, no Western medicine was used, yet the effect was so significant. Why? Answer: Whether Diabetes or Diabetic Nephropathy, both are difficult diseases, likely true in both Western and Traditional Chinese Medicine. Regarding why it was effective without combining Western medicine, I believe there are three reasons: 1. Accurate pattern differentiation and appropriate treatment formulas. 2. No mutual antagonism between Chinese and Western medicines. 3. No mutual antagonism among Chinese herbs.
4. Clinging to the view of Xiao Ke Yin deficiency, stubbornly adhering to Yin-nourishing treatments, subpleural fluid retention (Zhi Yin in the diaphragm area), failure of fluids to distribute, long-term treatment ineffective.
Guo XX, Male, 57 years old.
Thirst, excessive drinking and eating, progressive emaciation for over 7 years. Diagnosed with Diabetes Mellitus. Western medicine treatment for half a year improved symptoms significantly, and urine sugar dropped from (++++) to (+). However, shortly after discharge, symptoms returned to previous levels, and urine sugar quickly increased from (+) to (+++). Chinese herbal medicine for nourishing Yin and generating fluids was added; initially symptoms improved, but after 2 months, they reverted to original state. Afterwards, famous doctors were consulted for over 6 years, but symptoms never relieved. In the last 1+ year, not only did Diabetes not improve, but vision gradually declined, with two episodes of sudden blindness. Ophthalmological examination diagnosed early cataracts and fundus hemorrhage. In the last 5 months, numbness and pain in both legs developed, causing mobility issues. Neurological examination diagnosed peripheral neuritis. Consequently, hospitalized again for over 1 year, but symptoms not only failed to improve but worsened. In the last 1+ month, lower limbs were almost immobile, accompanied by poor appetite, reduced food intake, nausea, and vomiting. Examination diagnosed Diabetic Ketoacidosis. Due to worsening condition despite hospitalization, out of anger, the patient stopped all medications. Later, persuaded by family, TCM treatment was invited. Careful examination revealed desire to drink, vomiting immediately after drinking slightly more, epigastric fullness with poor appetite, numbness/pain/paralysis in lower limbs, blurred vision, yellow-white tongue coating, and wiry-tight-rapid pulse. Synthesizing pulse and symptoms: Thirst with desire to drink, vomiting after drinking too much, constitutes Water Reversal Syndrome (Shui Ni Zheng). Wiry-tight pulse indicates Cold and knotting; rapid indicates Heat. Tight and rapid fighting together indicates Cold-Fluid retention condensing and transforming into Heat. Correlating syndrome and pulse: Cold-Fluid knotting/stagnation, constrained transforming into Heat, preventing fluid distribution. Treatment should combine bitter and acrid methods, resolve Fluids and dissipate knots. Prescription: Fangji 10g, Guizhi 10g, Renshen 10g, Sheng Shigao 15g, Fuling 10g, Muli 6g.
After 3 doses, symptoms slightly decreased. At this time, the physician and family said: Chinese medicine acts slowly, can only improve symptoms, not solve root problems; without Western medicine, it would be unimaginable. But the patient did not listen, continuing only Chinese medicine. After continuing 20 doses, lower limb pain/numbness/paralysis completely disappeared, and nausea/vomiting resolved. Add Yuanshen 10g, take for 2 months, total 60 doses, all symptoms disappeared, except vision still poor due to cataracts.
5. Only relying on symptoms for differentiation, not examining pulse/color, disregarding Cold/Heat, applying only "ice and snow" [cold/nourishing Yin], suppressing Yang Qi, failure of fluids to distribute.
Zhang XX, Male, 29 years old.
Thirst, excessive drinking, fatigue/weakness, progressive emaciation for over 1 year. Diagnosed with Diabetes Mellitus. Western medicine treatment for over half a year was ineffective. Subsequently, Chinese herbal medicine for nourishing Yin/generating fluids, Ren Shen Bai Hu Tang, etc., not only failed to reduce symptoms but caused increasing epigastric fullness. Careful examination revealed, besides thirst/desire to drink, easy hunger/consumption of food, and epigastric fullness, emaciation/fatigue, dry skin, slightly reddish complexion, white tongue coating, and wiry-tight pulse. Synthesizing pulse and symptoms: Wiry-tight pulse indicates Cold, knotting, and Fluid retention. Combined with symptoms, Cold-Fluid knots in the Middle Jiao/Diaphragm area, constrained for a long time transforming into Heat. Treatment should use bitter-acrid descending methods, pivot Qi mechanism, resolve Fluids and generate fluids. Prescription: Fangji 10g, Guizhi 10g, Renshen 10g, Sheng Shigao 12g, Fuling 10g, Mangxiao 3g.
After 12 doses, excessive thirst/drinking greatly reduced, spirit doubled, weight increased by 9 kg, urine sugar dropped from (++++) to (±).
A certain physician asked: The patient originally did not mention constipation, why did you add Mangxiao? Answer: Mangxiao not only softens hardness and promotes bowel movement, but also softens hardness, dissipates knots, and removes Phlegm. This is the method of Zhang Zhongjing. In *Jin Gui Yao Lue*, regarding "If excess recurs after 3 days and does not heal with previous treatment, use Mu Fang Ji Tang removing Shigao and adding Fuling and Mangxiao," the use of Mangxiao in treating Phlegm-Fluid diseases lies here.
6. Clinging to Yin-nourishing, assisting Phlegm-Fluid, constraining Liver/Spleen, disorder of ascending/descending, disease severity increases.
Gao XX, Male, 25 years old.
Suffering from Diabetes Mellitus for over 2 years. Western medicine treatment was ineffective initially; subsequently, Chinese herbal medicine for nourishing Yin/generating fluids exacerbated the condition. In the last 2+ months, sudden frequent vomiting, restlessness, and palpitations occurred. Diagnosed with Diabetes Mellitus and Diabetic Ketoacidosis. Hospitalized for over 2 months with no symptom reduction. Careful examination revealed, besides frequent nausea/vomiting, dizziness/headache, restlessness/palpitations, epigastric fullness/pain, tenderness upon pressure, yellow-white greasy tongue coating, and wiry-tight-rapid pulse. Synthesizing pulse and symptoms: Wiry-tight pulse relates to Liver and Cold; tight-rapid combination indicates Cold-Excess knotting. Epigastric distension/pain with tenderness indicates Excess. Combined with symptoms: Liver-Stomach disharmony, Cold-Excess knotting, constrained transforming into Heat. Method should soothe Liver and harmonize Stomach, eliminate accumulation and guide stagnation. Prescription: Chaihu 10g, Banxia 10g, Renshen 10g, Huangqin 10g, Ganjiang 4g, Gancao 6g, Dazao 4 pieces, Cangzhu 15g, Houpo 10g, Chenpi 10g, Rougui 6g, Dahuang 3g.
After 2 doses, vomiting greatly reduced, epigastric fullness/pain alleviated, diet increased. After continuing 6 doses, vomiting disappeared, other symptoms reduced by 70-80%, ketonuria disappeared.
Oculomotor, Trochlear, and Abducens Nerve Diseases
Relying solely on reasoning, not examining pulse/syndrome, failing to distinguish Deficiency/Excess, delaying disease stage.
You XX, Male, 35 years old.
Three months ago, forehead and bilateral orbital areas were struck by a wooden stick during a fight, causing profuse bleeding. After surgical treatment, healing was rapid, but inability to move eyeballs bilaterally, blurred vision, and diplopia were discovered. Multiple consultations in ophthalmology and neurology confirmed Oculomotor, Trochlear, and Abducens Nerve Diseases. Western medicine treatment for over 1 month showed no improvement. Subsequently, acupuncture and Chinese herbal medicine for nourishing Kidney and Liver for 2 months still showed no improvement. Examination revealed inability to move both eyeballs, dilated pupils, left upper eyelid ptosis unable to open, slight right upper eyelid ptosis, blurred vision, diplopia, white tongue coating, right wiry-large pulse, left wiry-soggy-choppy pulse. Synthesizing pulse and symptoms: Disease originated after trauma; trauma must involve Blood Stasis. Right wiry-large, left wiry-soggy, right larger than left indicates Qi Deficiency. Pulse and syndrome combined indicate simultaneous Qi and Blood Deficiency with Blood Collateral Stasis. Treatment should tonify Qi and nourish Blood, invigorate Blood and unblock collaterals. Prescription: Huangqi 40g, Chishao 10g, Chuanxiong 10g, Danggui 10g, Taoren 10g, Honghua 10g, Dilong 10g, Chongweizi 6g.
After 6 doses, vision significantly clearer, diplopia obviously improved. After continuing 20 doses, all symptoms disappeared, recovery achieved.
A certain physician asked: Previous nourishment of Kidney/Liver was ineffective, subsequent tonification of Qi and invigoration of Blood restored eye movement. Why? Answer: Failure to examine pulse/syndrome, failure to treat based on syndrome, prescribing based on imagination.
Trigeminal Neuralgia
1. Clinging to disease name, crude medication application, stopping pain only when pain is seen, not examining pulse/syndrome, treating Deficiency as Excess, delaying disease progression.
Guo XX, Male, 49 years old.
Paroxysmal exacerbation of pain in left upper and lower teeth for over 20 years. Diagnosed with Trigeminal Neuralgia. Initially treated with tooth extraction; after extracting all left incisors, canines, and molars, pain persisted. Then switched to oral Western medicine, blocks, Chinese medicine, and acupuncture, but pain did not decrease. Careful examination revealed, besides paroxysmal severe pain in left upper/lower gums, cheek, and temple, restlessness, dry mouth/thirst, poor appetite/reduced food intake, thin-white tongue coating, and large-wiry-deficient pulse. Synthesizing pulse and symptoms: Large-wiry-deficient pulse indicates simultaneous Qi and Yin Deficiency with failure of Clear Yang to ascend and Turbid Yin to descend. Treatment should tonify Qi and nourish Yin, ascend Clear and descend Turbid. Prescription: Renshen 10g, Gancao 6g, Huangqi 15g, Danggui 6g, Maimen 10g, Wuweizi 10g, Qingpi 10g, Chenpi 10g, Shenqu 10g, Huangbai 10g, Gegen 15g, Cangzhu 15g, Baizhu 10g, Shengma 10g, Zexie 10g.
A certain physician asked: Since the manifestation is toothache, why not use modified Qing Wei San, Xie Huang San, or Gan Lu Yin? Patient replied: All have been used, but ineffective. Physician further said: The above formula is Dongyuan's Qing Shu Yi Qi Tang; this formula was designed by Dongyuan for Summerheat disease. I have never heard of using it for Trigeminal Neuralgia. Answer: Although the original Qing Shu Yi Qi Tang was not designed for Trigeminal Neuralgia, since it possesses functions of tonifying Qi/Yin and ascending Clear/descending Turbid, it is used for this patient with Trigeminal Neuralgia.
After 3 doses, pain greatly reduced. After continuing 20 doses, pain completely disappeared, recovery followed.
2. Not observing season/time, not asking etiology, not examining pulse/syndrome, relying only on habit, vainly administering drugs.
Su XX, Male, 29 years old.
Pain in left upper and lower teeth for over 2 months. Diagnosed with Trigeminal Neuralgia. Initially treated with Western medicine, pain slightly reduced, but intensified after 2 weeks. Another physician was invited to treat with Chinese herbs for nourishing Yin and purging fire, clearing Stomach and purging fire; nearly 40 doses taken, but effect still insignificant. Careful examination revealed, besides paroxysmal exacerbation of upper/lower toothache, whole-body stiffness/discomfort, thin-white tongue coating, and floating-tight pulse. Further inquiry revealed onset occurred shortly after sweating while loading/unloading cargo in a car on a cold winter day with strong north wind. Synthesizing pulse and symptoms: Disease originated after contraction of Wind-Cold; floating-tight pulse indicates external invasion of Wind-Cold. Treatment should disperse Wind and scatter Cold. Prescription: Chan Tui 10g, Jiang Can 10g, Chuanxiong 10g, Jingjie 10g, Fangfeng 10g, Xixin 3g, Baizhi 10g, Bohe 6g, Gancao 6g, Qianghuo 10g.
A certain physician asked: Physicians all say upper/lower toothache should be treated with Gan Lu Yin, Qing Wei San, etc., why do you not use them? Answer: What is truth in science? I agree with the view that what is proven correct in practice is truth. Since facts prove the previous medicine did not solve the problem, it proves it incorrect, thus requiring alternative formulas. This syndrome originated after contracting Wind-Cold, with floating-tight pulse, naturally requiring Wind-dispersing and Cold-scattering herbs.
After 1 dose, pain greatly reduced. After continuing 4 doses, all symptoms disappeared, recovery achieved.
3. Not examining pulse/syndrome, unaware of changes, clinging to empirical formulas, stubbornly adhering to secret recipes, conservative stagnation, condition not improving.
Ding XX, Male, 55 years old.
Paroxysmal burning pain in left upper tooth, orbit, and temple for over 8 years. Diagnosed with Trigeminal Neuralgia. Initially Western medicine slightly reduced pain, but continued use had no obvious effect. TCM was invited to treat with nourishing Yin/purging fire, clearing Stomach/purging fire, dispersing Wind/scattering Cold, invigorating Blood/removing Stasis, secret empirical formulas involving Wugong, Quanxie, Fengfang, and patent medicines for toothache; apart from occasional slight effect, mostly no obvious effect. In the last 3+ years, pain progressively worsened, especially in the last 3+ months, where washing face, brushing teeth, eating, drinking, shaving, yawning, speaking, and breeze blowing on face all triggered severe pain; touching the face or lecturing caused unbearable severe pain. Careful examination revealed, besides above, occasional restlessness, insomnia, thin-white tongue coating, and wiry-tight-rapid pulse. Synthesizing pulse and symptoms: Wiry pulse is Liver pulse; tight pulse is Cold; tight-rapid combination indicates Cold-Phlegm condensation. Combined with syndrome, Phlegm constraint and Qi knotting, Liver Wood failing to diffuse. Treatment should soothe Liver and regulate Qi, resolve Phlegm and dissipate knots. Prescription: Chaihu 10g, Banxia 10g, Huangqin 10g, Renshen 10g, Shengjiang 3 slices, Dazao 5 pieces, Gancao 6g, Guizhi 10g, Shu Jun 3g, Longgu 15g, Muli 15g, Fuling 15g.
After 6 doses, pain greatly reduced, but still painful at start of lectures. After continuing 18 doses, pain no longer improved, and whole-body stiffness/pain/discomfort felt. Re-examining pulse still wiry-tight. Pulse and syndrome combined: Fear of severe Cold entering collaterals. Prescribe Wutou Guizhi Tang to remove exterior-interior Cold. Prescription: Chuanwu 12g, Guizhi 10g, Baishao 10g, Shengjiang 3 slices, Dazao 7 pieces, Zhi Cao 10g, Xixin 3g.
After 3 doses, pain nearly disappeared. Instructed to take Horn Sugar-Coated Tablets for consolidation.
4. Not examining etiology, knowing only pain relief, or treating Cold with Cold, or treating Heat with Heat, ultimately not the correct treatment, delaying for years.
Zhu XX, Female, 58 years old.
Pain in right upper/lower gums, orbit, and temple for 15 years. Diagnosed with Trigeminal Neuralgia. Initially oral/blocked Western medicine reduced pain but could not solve root problem. Subsequently combined with Chinese herbs for invigorating Blood/stopping pain, insect-based stopping pain, purging fire/stopping pain, and topical pain relief, but none had obvious effect. In the last 2 years, pain was almost constant; speaking, brushing teeth, or washing face made pain unbearable. In the last 1 month, not only unbearable pain, but frequent restlessness/palpitations, dizziness/insomnia, easily startled/afraid, occasional counterflow Qi rushing up, with sweating and worsened palpitations upon rushing, numbness/swelling in hands, poor appetite/bitter taste. Careful examination revealed, besides above, yellow-white tongue coating, and wiry-tight-choppy pulse. Synthesizing pulse and symptoms: Wiry-tight pulse indicates Liver constraint and Cold stagnation; choppy indicates stagnation. Combined with symptoms, Liver constraint and Qi knotting, Cold-Phlegm knotting failing to transform. Treatment should soothe Liver and resolve constraint, resolve Phlegm and dissipate knots. Prescription: Chaihu 10g, Banxia 10g, Huangqin 10g, Dangshen 10g, Shengjiang 10g, Dazao 5 pieces, Guizhi 10g, Fuling 10g, Shu Jun 3g, Longgu 15g, Muli 15g.
After 4 doses, symptoms improved, pain reduced. After continuing 25 doses, pain disappeared, recovery achieved.
