Bupi Wan (Spleen Tonifying Pill): Suitable for patients with Spleen Qi Deficiency who are averse to taking decoctions. Baizhu Wan (Atractylodis Macrocephalae Pill): Indicated for the same condition as above. Ingredients: Baizhu (Atractylodis Macrocephalae Rhizoma, 8 liang), Shaoyao (Paeoniae Radix Alba, 4 liang). Grind into powder and form pills using congee.
Da'an Wan (Great Peace Pill): Functions to strengthen the Spleen and Stomach and resolve food stagnation. Ingredients: Shanzha (Crataegi Fructus) and Baizhu (Atractylodis Macrocephalae Rhizoma, 2 liang each); Fuling (Poria), Shenqu (Medicated Leaven, stir-fried), Banxia (Pinelliae Rhizoma, 1 liang each); Chenpi (Citri Reticulatae Pericarpium), Laifuzi (Raphini Semen, stir-fried), Lianqiao (Forsythiae Fructus, 5 qian each). Severe pain in the heart and diaphragm region, radiating to the lower back; extreme pain leading to syncope; inability to retain any medication. In cases of severe vomiting, induce further emesis by probing the throat with a goose feather. After expelling approximately one large bowl of phlegm accumulation, the pain ceases.
A male patient presented with neck stiffness and inability to look back, with slight pain upon movement. His pulse was wiry, rapid, and full, more pronounced on the right hand. This was diagnosed as Phlegm-Heat lodging in the Taiyang Channel (Foot-Taiyang Bladder Channel). He was treated with Erchen Tang (Two-Cured Decoction, a basic formula for drying dampness and resolving phlegm) supplemented with Huangqin (Scutellariae Radix), Qianghuo (Notopterygii Rhizoma et Radix), and Honghua (Carthami Flos). He recovered after two days.
Another male patient suddenly developed a linear pain originating from the interscapular region, extending upwards across the shoulder to the lateral chest and hypochondrium. The pain persisted day and night and was unbearable. His pulse was wiry and rapid; upon deep pressure, it felt hollow yet forceful, with the left pulse larger than the right. The scapular region belongs to the Small Intestine Channel, while the chest and hypochondrium belong to the Gallbladder Channel. This indicates that excessive thinking damaged the Heart. Although the Heart organ itself had not yet manifested disease, its paired Fu organ (Small Intestine) became diseased first, hence the pain started from the back/scapula. Due to indecisiveness caused by overthinking, the pathogen returned to the Gallbladder, causing pain to ascend to the chest and hypochondrium. This represents Small Intestine Fire attacking Gallbladder Wood, where the "child" attacks the "mother," constituting an Excess Pathogen. Inquiry revealed the illness originated from frustration due to unfulfilled plans. He was treated with Ren Shen (Ginseng Radix, 4 qian) and Mutong (Akebiae Caulis, 2 qian) decocted in water to administer Longhui Wan (Dragon Bile Pill). After several doses, he recovered.
Another patient suffered from Spleen and arm pain. Internally, he took Erchen Tang (Two-Cured Decoction) added with Jiu Jin Huangqin (Alcohol-steeped Scutellariae Radix), Cangzhu (Atractylodis Rhizoma), and Qianghuo (Notopterygii Rhizoma et Radix). Externally, Fengxian Ye (Impatiens leaf) was crushed and applied to the painful area.
This describes Dampness in the Upper Jiao spreading horizontally through the meridians. Treatment uses Erchen Tang (Two-Cured Decoction) added with Cangzhu (Atractylodis Rhizoma), Xiangfu (Cyperi Rhizoma), Weilingxian (Salviae Miltiorrhizae Radix et Rhizoma - *Note: Standard WHO term for Weilingxian is Saposhnikoviae Radix, but context implies moving wind-damp; assuming Saposhnikoviae based on standard TCM usage for this herb name*), Jiu Qin (Wine-processed Scutellariae Radix), Nanxing (Arisaematis Rhizoma Praeparatum), and Baizhu (Atractylodis Macrocephalae Rhizoma), decocted with fresh ginger. Another variation adds Danggui (Angelicae Sinensis Radix) and Qianghuo (Notopterygii Rhizoma et Radix), named Huoluo Tang (Activate Collaterals Decoction).
If the pathology is on the left, it is attributed to Wind-Dampness. Herbs include Chaihu (Bupleuri Radix), Xiong (Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix), Qianghuo (Notopterygii Rhizoma et Radix), Duhuo (Angelicae Pubescentis Radix), Banxia (Pinelliae Rhizoma), Cangzhu (Atractylodis Rhizoma), Xiangfu (Cyperi Rhizoma), and Gancao (Glycyrrhizae Radix et Rhizoma). If the pathology is on the right, it is attributed to Phlegm-Dampness. Herbs include Nanxing (Arisaematis Rhizoma Praeparatum) and Cangzhu (Atractylodis Rhizoma), among others.
