The Appendix of *Xinfa* states: Hong Kong Jiao (Gouty Arthritis / Foot Qi) includes patterns of Damp-Heat, Food Stagnation with Flowing Accumulation, Wind-Dampness, and Cold-Dampness.
To overcome dampness, the main herbs are Cangzhu (Atractylodis Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Fangji (Stephaniae Tetrandrae Radix), and Chuanxiong (Chuanxiong Rhizoma). Alternatively, Liuwu Fuzi Tang (Six-Ingredient Decoction with Aconite Root) or Danggui Niantong Tang (Angelica Sinensis Decoction for Relieving Pain) may be used.
For severe stagnation in Hong Kong Jiao Qi, Zhouche Wan (Boat Cart Pill) or Chushi Dan (Dampness-Removing Pellet) are indicated; if there is fluid retention, Dongyuan's Kaijie Daoyin Wan (Kaijie Guiding-Out Fluid Retention Pill) is used.
When treating Hong Kong Jiao requiring exterior resolution, drugs such as Mahuang Zuojing Tang (Ephedra Left Meridian Decoction) are selected according to the affected meridian; if complicated by Phlegm-Qi and Cold-Dampness, Wuji San (Five Accumulations Powder) with Mugua (Chaenomeles Fruit) is added.
For simultaneous release of exterior and interior pathogens, Dahuang Zuojing Tang (Rhubarb Left Meridian Decoction) or Dongyuan's Qianghuo Daozhi Tang (Notopterygium Decoction for Directing Out Stagnation) are used.
For regulating blood, Bawei Wan (Eight-Ingredient Pill) or Siwu Tang (Four Substances Decoction) plus Qianghuo (Notopterygii Rhizoma et Radix) and Tianma (Gastrodiae Rhizoma) are used; alternatively, Siwu Tang plus Huangbai (Phellodendri Cortex) and Nanting (Arisaematis Rhizoma Praeparatum), or Jianbu Wan (Strong-Legs Pill) may be employed.
For dispersing wind and nourishing blood, Du Huo Ji Sheng Tang (Radix Angelicae Pubescentis Decoction for Expelling Wind-Dampness and Nourishing Blood and Liver-Kidney) is most effective.
In spring and summer, when Qi overflows causing swelling and pain, Xiangsu San (Fragrant Perilla Powder) plus Wujia Pi (Acanthopanacis Cortex), Mugua, Binglang (Arecae Semen), and Chuanlianzi (Evodiae Fructus) are used.
For heat with redness and swelling, Baidu San (Defeat Toxin Powder) plus Mugua and Cangzhu are used.
After autumn and winter, Wuji San plus Mugua, Niuxi (Achyranthis Bidentatae Radix), Binglang, and Wuzhuyu (Evodiae Fructus) are used.
Danxi stated: There are patterns of Phlegm and Blood Heat. For Blood Heat, Siwu Tang plus Huangbai, Zhimu (Anemarrhenae Rhizoma), and Niuxi are used; for those with Phlegm saliva, Wuji San plus Mugua is used.
Siwu Tang (Formula seen in the Tonifying Deficiency Section.)
Wuji San (Formula seen in the Middle Cold Section.)
Zhouche Wan, Chushi Dan (Both seen in the Dampness Category.)
Kaijie Daoyin Wan (Formula seen in the Phlegm-Saliva Section.)
To treat foot convulsions (Charley Horse), pain, and spasmodic tightness, use Songjie (Pinus Massoniana Node) two liang, finely chopped; Rushxiang (Olibanum) one qian. In a silver or ceramic vessel, stir-fry slowly over low heat until charred but retaining its nature. Grind into fine powder. Take one to two qian per dose, administered with Mugua wine.
To treat Hong Kong Jiao with bone and tendon pain, use Jinyinhua (Lonicerae Japonicae Flos) ground into powder. Take two qian per dose, mixed with hot wine. Alternatively, chop the herbs finely and decoct together with Mugua, Baishao Yao (Paeoniae Radix Alba), Guigui (Cinnamomi Ramulus), Danggui (Angelicae Sinensis Radix), and Gancao (Glycyrrhizae Radix et Rhizoma) in half water and half wine. Strain the dregs and take warm on an empty stomach.
