Zhang Zhongjing said: When the pulse is fine and rapid (Wei Shu), moxibustion must be strictly avoided. Because fire becomes a pathogenic factor, leading to irritability and rebellious qi. This approach pursues deficiency and drives out reality (excess), causing blood to scatter within the vessels. Although the fire energy appears slight, its internal attack is powerful, scorching bones and injuring sinews, making blood difficult to restore. Chang Shi stated that Guizhi Qu Shaoyao Jia Shujie Muli Longgu Jiuni Tang (Cinnamon Twig Decoction without Paeonia plus Fleeceflower Root, Oyster Shell, and Dragon Bone for Rescue from Reversal) is suitable. If sweating is desired, Chaihu Guizhi Tang (Bupleurum and Cinnamon Twig Decoction) is suitable.
It is also said: When the pulse is floating (Fu), the disease should be resolved by inducing sweating. However, if moxibustion is applied in error, the pathogen has no outlet to exit. Due to the fire, the pathogen becomes prosperous. The disease below the waist will definitely feel heavy and numb (Zhong Mu). This is named Fire Reversal (Huo Ni). If it is to resolve spontaneously, there must first be irritability, followed by sweating, and resolution follows the sweat. How do we know this? Because the pulse is floating, we know that sweating will lead to resolution.
It is also said: In cases of Fire Reversal treated incorrectly with purgation, where burning needles cause irritability, Guizhi Gancao Longgu Muli Tang (Cinnamon Twig, Licorice, Oyster Shell, and Dragon Bone Decoction) is the primary treatment.
It is also said: When the pulse is floating and heat is severe, applying moxibustion constitutes treating an excess condition as if it were deficient. Driven by fire, it will inevitably cause dry throat and hemoptysis (vomiting blood).
It is also said: In Shanghan (Cold Damage) with a floating pulse, if a doctor uses fire methods to force sweating, resulting in loss of Yang, fright, mania, and restlessness (sitting and lying down uneasily), Guizhi Qu Shaoyao Jia Shujie Muli Longgu Jiuni Tang (Cinnamon Twig Decoction without Paeonia plus Fleeceflower Root, Oyster Shell, and Dragon Bone for Rescue from Reversal) should be used.
Pang Shi said: After moxibustion or burning needles, if symptoms arise due to fire coercion, they should be treated according to the principles for fire-induced conditions. For irritability, fright, and mania, Liushi Fengyin Tang (Six-Metal Wind-Driving Decoction) is particularly excellent. Chaihu Jia Longgu Muli Tang (Bupleurum Plus Oyster Shell and Dragon Bone Decoction) is also generally applicable.
It is also said: If moxibustion is mistakenly applied when inappropriate, allowing fire pathogen to enter the abdomen and disrupt the five Zang organs, compounded by irritability leading to death. Guo Yong stated: For all cases of Fire Pathogen Reversal, Guizhi Gancao Longgu Muli Tang (Cinnamon Twig, Licorice, Oyster Shell, and Dragon Bone Decoction) is suitable.
Zhang Zhongjing said: Burning needles to induce sweating, where the needle site is exposed to cold, forming nodules that are red, will inevitably trigger Ben Tun (Running Piglet Qi). If Qi rushes from the lower abdomen up to the heart, apply one moxa cone on top of the nodule, and administer Guizhi Jia Gui Tang (Cinnamon Twig Decoction with Added Cinnamon Twig), increasing the dosage of Rougui (Cinnamon Bark).
It is also said: In Shaoyin Disease, occurring one or two days after onset, with a normal mouth sensation (no bitter taste or dryness) and aversion to cold in the back, moxibustion should be applied.
It is also said: In Shaoyin Disease, with vomiting and diarrhea, but hands and feet are not cold, and instead there is fever, the prognosis is not fatal. If the pulse is imperceptible, apply seven moxa cones to Shaoyin acupoints. Guo Yong stated: For all cases of Shaoyin diarrhea treated with moxibustion, Sini Tang (Frigid Extremities Decoction) should be taken concurrently.
It is also said: In Shaoyin Disease, with diarrhea, a fine and choppy pulse, vomiting, and sweating, there will be frequent defecation attempts but with little output. One should warm the upper part and apply moxibustion. Another version states: Apply fifty moxa cones to Jueyin channels. (It should be Baihui GV20).
It is also said: For all types of diarrhea, Dada SP2 (Great Market) on the foot can be moxibusted with five cones. (Another version says seven cones). Shangqiu SP5 and Yinlingquan SP9 can each be moxibusted with three cones.
It is also said: In diarrhea with cold extremities and absent pulse, if moxibustion does not warm the body and the pulse does not return, but instead slight panting occurs, the prognosis is fatal. If the Shaoyin pulse is weaker than the Duboyang (Foot Yangming) pulse, it is considered a favorable sign.
It is also said: In Shanghan with a hurried pulse (Cu) and cold extremities, moxibustion can be applied. This is because moxibustion on Shaoyin and Jueyin channels treats reversal conditions.
Pang Shi said: If moxibustion should have been applied but was not, causing cold stagnation in the patient, which becomes firmly established over time, Qi rushing to the heart will result in death.
