Deafness, hypochondriac pain, alternating chills and fever, vomiting with a bitter taste in the mouth, indicate a syndrome of the Foot-Shaoyang Gallbladder Channel (Zu Shao Yang Dan Jing). If the onset is characterized by aversion to cold and fever, deafness, and hypochondriac pain, it is the Ben (Root) disease. If subsequently vomiting, dry tongue, and bitter taste occur, it is the Biao (Branch) disease. Since the Gallbladder has no direct entry or exit, the pathology lies between the Exterior and Interior. Only Xiaochaihu Tang (Minor Chaihu Decoction), modified as appropriate, should be used to harmonize the Exterior and Interior; no other formula is suitable. (This formula has its own modification rules.) This channel has three prohibitions: sweating, purging, and emesis are contraindicated. If treated correctly, what adverse reactions could arise? One must always bear this in mind and exercise caution.
Deafness, hypochondriac pain, alternating chills and fever, vomiting with a bitter taste and dry tongue, constitute a Half-Exterior Half-Interior syndrome. Treatment does not follow the standard Ben or Biao approaches but employs the "Middle Treatment" (harmonizing method).
A wiry and rapid pulse indicates a syndrome of this channel.
For cases presenting with deafness, hypochondriac pain, alternating chills and fever, vomiting, bitter taste, and dry tongue, use Chaihu Shuangjie Yin (Chaihu Dual-Resolving Drink). (This is essentially a modified Xiaochaihu Tang.)
Abdominal fullness, spontaneous diarrhea, inability of fluids to reach the throat, and warm hands and feet indicate a syndrome of the Foot-Taiyin Spleen Channel. If the onset begins with abdominal fullness and dry throat, it is the Ben disease. If jaundice (yellowing of the body and eyes) develops subsequently, it is the Biao disease. There is a distinction between Cold and Heat patterns internally; they must not be treated indiscriminately. Clinical application requires appropriateness.
Abdominal fullness, dry throat, and jaundice belong to Fu-Organ Heat. Spontaneous diarrhea without thirst, or with vomiting, belongs to Zang-Organ Cold.
A deep and forceful pulse indicates suitability for purging. A deep and weak pulse indicates suitability for warming.
For abdominal fullness, dry throat, warm hands and feet, and abdominal pain, use Guizhi Dahuang Tang (Cinnamon Twig and Rhubarb Decoction). (This is a modified Guizhi Tang.) For jaundice, use Yinchene Dahuang Tang (Artemisiae Scopariae and Rhubarb Decoction). (This is a modified Yincheng Hao Tang.) For spontaneous diarrhea without thirst, or with vomiting, use Jiawei Lizhong Yin (Modified Lizhong Drink). (This is a modified Lizhong Tang.) In severe cases, use Huiyang Jiuji Tang (Restoring Yang and Saving from Crisis Decoction). (This is a modified Sini Tang.)
Dry tongue and parched mouth indicate a syndrome of the Foot-Shaoyin Kidney Channel. If the onset begins with dry tongue and parched mouth, it is the Ben disease. If delirium and solid stool develop subsequently, it is the Biao disease. When reaching the Yin channels, fixed rules cannot be rigidly applied; one may use warming methods or purging methods. Direct invasion into the Yin branch constitutes a Cold syndrome; transmission via meridians constitutes a Heat syndrome. This elucidates insights previously unexpressed by earlier authors.
