When a Tianxing disease (epidemic infectious disease) has persisted for more than seven days, the heat pathogen becomes firmly entrenched. There is constipation with difficult defecation, epigastric and abdominal fullness and stagnation, inability to take food or drink, mental confusion and trance, delirious speech, and a deep, fine pulse. Among all these symptoms, none indicates a possibility of recovery; therefore, Fuzi Biejia Tang (Aconite Turtle Shell Decoction) is decided upon. (It should not be prepared as a decoction powder.)
Bie Jia (Turtle Shell), Baixianpi (Dense Twigged Melia Bark), Yinchen (Artemisiae Scopariae Herba) (each 0.5 liang); Xixin (Asari Radix), Guizhi (Cinnamon Twig), Baizhu (Atractylodis Macrocephalae Rhizoma), Wuzhuyu (Evodiae Fructus), Fuzi (Prepared Aconiti Lateralis Radix), Zhishi (Aurantii Fructus Immaturus) (each 1 fen); Dahuang (Rhei Radix et Rhizoma) (3 fen); Shengjiang (Fresh Ginger) (1 liang).
Crush the ingredients, add three sheng of water, boil down to one sheng, and divide into three warm doses.
If Shanghan (Cold Damage) management is improper, and too many cold-natured medicines have been taken, latent heat remains in the zang organs. The hands and feet are cold (Jue Ni), and the fingernails are slightly cyanotic. There is fear that yang qi will gradually decline, leading to Yin Du (Yin Toxin) syndrome. Hesitating for a moment, the condition may transform into Huhuo (Foxes' Confusion) disease, presenting with a mottled face resembling brocade patterns. Mutong Guizhi Tang (Akebia Twig and Cinnamon Twig Decoction) is used.
Mutong (Akebiae Caulis), Guizhi (Cinnamon Twig) (each 1 liang); Wuzhuyu (Evodiae Fructus), Xixin (Asari Radix) (each 1 fen); Gancao (Licorice Root), Congbai (Scallion White) (six stalks), Zao (Jujube Fruit) (nine fruits).
Crush the ingredients, add two and a half sheng of water, boil down to one sheng and two he, strain, and divide into four warm doses.
Between twenty-one and twenty-eight days after the onset of Shanghan, if labor does not cease and heat toxin persists, with alternating fever and chills, intermittent remission and recurrence, and movements resembling malaria, Biejia Hengshan Tang (Turtle Shell and Swallowwort Root Decoction) is used.
Bie Jia (Turtle Shell), Hengshan (Swallowwort Root), Niuxi (Achyranthis Bidentatae Radix) (each 3 fen); Daqing (Isatidis Folium), Mudanpi (Moutan Cortex) (each 0.5 liang); Shigao (Gypsum Fibrosum) (2 liang); Wumei (Mume Seed), Gancao (Licorice Root) (each 1 fen); Danzhuye (Lophatheri Herba), Chi (Fermented Soybean), Sheng Maidong (Ophiopogonis Radix), Sheng Dihuang (Rehmanniae Radix) (each 2.5 he).
Crush the ingredients, add five sheng of water, boil down to two and a half sheng, and drink one zhan warm.
If Shanghan persists for eight or nine days without resolution, it is called Bai Shanghan (Defeated Cold Damage). For cases where various medicines fail to cure, Biejia Xijiao Tang (Turtle Shell and Rhinoceros Horn Decoction) is used.
Bie Jia (Turtle Shell), Shengma (Cimicifugae Rhizoma), Chaihu (Bupleuri Radix), Wumei (Mume Seed), Zhishi (Aurantii Fructus Immaturus), Xijiao Xie (Rhino-Ceratis Cornu), Huangqin (Scutellariae Radix) (each 0.5 liang); Gancao (Licorice Root) (1 fen); Sheng Dihuang (Rehmanniae Radix) (2.5 he).
Crush the ingredients, add three sheng of water, boil down to one and a half sheng, divide into five doses, administered three times during the day and twice at night.
For Bai Shanghan presenting with headache, bone and muscle pain, delirious and absurd speech, which physicians cannot treat, use the previously mentioned Heinu Wan (Black Slave Pill). (The formula is located in the section on miscellaneous sweating syndromes.)
Pang Anshi said: "There was a patient suffering from Shiqi (Seasonal Qi) disease. After six or seven days, due to mania, his eyes stared fixedly without blinking, his hands were contracted and curled, his mouth was either tightly closed or open. He was unconscious and unaware of his surroundings, with no breath coming from his nose or mouth, yet his heart remained warm, his complexion was peaceful, and all six pulses were palpable. This resembled Shi Jue (Corpse Stupor). Such unconsciousness lasted for five or six days. Considering it a hopeless case, I rescued him using Fengyin Tang (Wind-Driving Decoction) plus Fuzi (Prepared Aconite). After administering two doses via gavage, he regained consciousness."
Fengyin Tang Plus Fuzi Decoction
Hanshuishi (Halloysitum Rubrum), Shigao (Gypsum Fibrosum), Chishizhi (Halloysitum Rubrum), Baishizhi (Halloysitum Album), Zi Shiqiong (Quartzum Purpureum), Huashi (Talcum) (each 6 qian); Fuzi (Prepared Aconite) (one piece); Longgu (Fossilized Bones), Dahuang (Rhei Radix et Rhizoma), Jiangjiang (Zingiberis Rhizoma) (each 4 qian); Gancao (Licorice Root), Muli Fen (Concha Ostreae Powder) (each 2 qian); Guizhi (Cinnamon Twig) (3 qian).
Make into coarse particles. For each dose, take three qian bi, add one zhan of water, boil down to seven-tenths, strain, and drink warm.
