If the eyes are bright and capable of clear vision, the condition is treatable. If the eyes are dim and unable to recognize people, or if they look upward, stare straight ahead, remain fixed in a round shape, exhibit "dai yan" (eyes fixed upward and unable to move), or if the eyelids are sunken, these are all untreatable conditions. Those who keep their eyes open and wish to see others have a Yang syndrome; those who keep their eyes closed and do not wish to see others have a Yin syndrome. A sensation of blurriness in the eyes and lack of coordination in gaze indicates intense heat within. Eye pain belongs to Heat of the Yangming channel; redness of the eyes also indicates intense heat. Photophobia will inevitably lead to epistaxis (nosebleed). Yellowing of the sclera indicates that jaundice (body yellowing) is about to develop. When a disease is about to resolve, signs include the fading of eye yellowness, brightness of the nasal tip, and luminosity of the shangen (root of the nose).
A bluish color at the nasal tip indicates abdominal pain with cold sensations, which is a fatal prognosis. A slightly black color indicates fluid retention (water qi); a yellow color indicates difficult urination; a white color indicates Qi deficiency; a red color indicates Lung Heat; and a bright, lustrous color indicates retained fluids (liu yin). Dry nostrils belong to Heat of the Yangming channel and will inevitably lead to epistaxis. Dry nostrils that are black like soot indicate severe deep-seated heat from Yang toxicity. Cold, slippery, and black nostrils indicate extreme cold from Yin toxicity. Snoring during sleep indicates Wind-Warmth (Feng Wen); nasal congestion with turbid discharge indicates Wind-Heat. Flaring of the nostrils indicates Lung Wind or impending collapse of Lung Qi, which is untreatable.
Chapped and dry lips indicate Spleen Heat; if chapped but red, the prognosis is good; if chapped and black, it is ominous. Swelling and redness of both lips indicate intense heat. Cyanosis and blackness of both lips indicate extreme cold. Bitter taste in the mouth indicates Gallbladder Heat; a sweet taste indicates Spleen Heat; dry mouth and throat indicate Kidney Heat. Dry tongue and mouth with a desire to drink water indicate Heat of the Yangming channel. Trismus (lockjaw) and difficulty speaking indicate Jing Feng (spasmic wind). Sores on the upper lip indicate Hu Chong (roundworms) eroding the internal organs; sores on the lower lip indicate Huo Chong eroding the anus. If the lips are cyanotic, the tongue is curled, the corners of the mouth are blue, the area around the mouth is dark, the mouth is open with uncontrolled exhalation, the mouth resembles a fish's gape, the lips tremble uncontrollably, and breath does not return, these are all untreatable conditions.
Reddish-pink earlobes indicate life; yellow, white, black, or blue earlobes that are dry and brittle indicate death. Thin and white, or thin and black earlobes both indicate failure of the Kidneys. Deafness and ear pain belong to Heat of the Shaoyang channel and are still treatable. However, deafness accompanied by a curled tongue and cyanotic lips belongs to the Jueyin channel and is difficult to treat.
A fresh red tongue color is auspicious; blue indicates cold; blue-purple indicates Yin cold; red-purple indicates Yang heat. A black tongue is extremely hyperactive and difficult to treat. A white and slippery tongue coating indicates exterior cold; alternatively, this may indicate Heat in the Dantian and Cold in the chest. A yellow and dry coating with thirst indicates excess heat. A black and dry coating with thirst indicates extreme heat and hyperactivity. If there is no thirst, and the black coating is slippery, it indicates Cold and Yin. A curled, scorched-black, and dry tongue indicates extreme heat from Yang toxicity. A blue tongue with a slippery coating indicates extreme cold from Yin toxicity. Tongue swelling, dry cracking, and prickles on the tongue all indicate intense heat. Stiffness, rigidity, shortening of the tongue, confusion of spirit, and unclear speech indicate death. In the case of Yin-Yang transmission disease, a tongue protruding several inches indicates death. The tongue is the orifice of the Heart and belongs to Fire; a red color is auspicious, whereas black indicates Water overcoming Fire, making it difficult to treat.
