Mr. Chen Wensu said: Within one to ten days after the eruption of smallpox or chickenpox, if there is high fever throughout the body and yellow, thick stool, this indicates excess in both the exterior and interior. The lesions will necessarily be lustrous and raised, necessarily full, and will easily scab over without causing damage. He also said: In cases of smallpox with fever, thirst, and incessant irritability, cold water, honey, persimmons, watermelon, and other raw/cold foods, as well as medicines like Qingliang Yin (Cooling Drink) and Qingdu San (Clear Toxin Powder), must strictly be avoided. Otherwise, it may damage the Spleen and Stomach internally, leading to abdominal distension, wheezing, and fullness; if accompanied by shivering and teeth grinding, it becomes difficult to treat. I believe that for the aforementioned symptoms, if urination and defecation are naturally regulated, diet is mild, thirst is relieved by drinking hot decoctions, and the hands and feet are not hot, this is deficiency heat, and raw/cold foods should not be eaten. If urination and defecation are constipated, the patient prefers cold food/drink, thirst is relieved by drinking water, and both hands and feet are hot, this is excess heat, and cold water can be given. Generally, when fever persists during the eruption of smallpox, once the eruption is complete, the fever will cease on its own.
A child presented with abdominal distension and fever. The eruptions were dense with unclear bases, lacking a ruddy and vibrant color, and insufficient pus formation. My father attributed this to Spleen Qi deficiency with incomplete toxin clearance. We used Shenqi Sisheng San (Ginseng and Astragalus Four Holy Powders). The eruptions subsequently re-emerged, the fever ceased, and the lesions became ruddy and distinct. Finally, we used Shenqi Neituo San (Ginseng and Astragalus Interior-Supporting Powder) until they scabbed over.
Another child presented with abdominal distension, thirst, and patchy fever. My father diagnosed Spleen Qi deficiency with incomplete toxin clearance. Two doses of Shenqi Sisheng San were administered. The earlier eruptions filled with pus, and the later ones suppurated, leading to recovery.
A child with smallpox presented with abdominal distension, free urination and defecation, and cold hands and feet. My father diagnosed Spleen and Stomach deficiency-cold. One dose of Shi'erwei Yigong San (Twelve-Ingredient Yi Gong Powder) was given, followed by two doses of Wuwei Yigong San (Five-Ingredient Yi Gong Powder) added with Muxiang (Aucklandiae Radix). Muxiang was then removed, and one more dose resulted in a cure.
A child with smallpox presented with fever and thirst. This was due to incomplete eruption and Spleen/Stomach deficiency generating heat. One dose of Renshen Maidong San (Ginseng and Ophiopogon Powder) was used; the eruptions re-emerged, and fever/thirst ceased. Renshen Baizhu San (Ginseng and Atractylodes Macrocephala Powder) was used to improve appetite. Shenqi Sisheng San was used to facilitate pus formation, and Tuoli San (Interior-Supporting Powder) was used until the sores scabbed over.
A child with smallpox presented with abdominal distension, cold feet, internal heat, and thirst. This was attributed to Stomach Qi deficiency and insufficiency of Body Fluids. I first administered two doses of Wuwei Yigong San, followed by Shenqi Sisheng San to promote pus formation, and Renshen Baizhu San until scabbing occurred.
A postpartum woman presenting with smallpox exhibited shivering, teeth grinding, abdominal distension, thirst, cold feet, and body heat. This indicated internal Spleen/Stomach deficiency-cold with external false heat. Ten Complete Great Tonifying Decoction (Shiquan Dabu Tang) added with Guizhi (Cinnamon Twig) and Fuzi (Prepared Aconite Root) was administered for four doses. Then, Fuzi was removed and replaced with Ganjiang (Dried Ginger) for another four doses. Finally, Shenqi Sisheng San and Wuwei Yigong San added with Danggui (Angelicae Sinensis Radix) and Huangqi (Astragali Radix) were used until scabbing occurred.
A pregnant woman presenting with smallpox had fever, cold feet, and abdominal distension. This was due to Spleen/Stomach weakness and unexpressed toxins. Zicao Muxiang San (Arnebia and Aucklandia Powder) was used, followed by Bazhen San (Eight-Treasure Powder) to promote pus formation. Ginseng and Astragalus were doubled, and after several doses, she recovered.
Renshen Maidong San (also known as Maidong San): Treats thirst caused by smallpox.
Maidong (Ophiopogonis Radix) (1 liang), Renshen (Ginseng Radix), Gancao (Licorice Root), Chenpi (Citri Reticulatae Pericarpium), Baizhu (Atractylodis Macrocephalae Rhizoma), N Houpo (Magnoliae Officinalis Cortex processed with ginger) (each 0.5 liang).
Take 3 qian per dose, decoct in water, and adjust dosage according to the child's age/size.
Note: The above formula is suitable for thirst caused by heat-toxin. If thirst is caused by Middle Jiao Qi deficiency, Renshen Baizhu San should be used.
Zicao Muxiang San: Treats internal deficiency with itching and collapse, black sunken lesions, and fever in smallpox.
Zicao (Arnebiae Radix seu Rubra), Fuling (Poria), Gancao (Licorice Root), Baizhu (Atractylodis Macrocephalae Rhizoma), Muxiang (Aucklandiae Radix), Renshen (Ginseng Radix) (equal parts), Nuomi (Glutinous Rice).
Take 3 qian per dose, decoct in water.
