Qian Zhongyang stated: Pediatric epilepsy occurs because the child's blood and qi are not yet full, and their spirit (Shen) is not yet solid. It may be caused by wind pathogen invasion, or triggered by fright and palpitations. There are also cases caused by the mother's emotional disturbances and fright during pregnancy. If symptoms include staring eyes, eye deviation, locked jaw, drooling, abdominal distension with convulsions, opisthotonos (backward arching of the back and neck), stiff and strong lumbar spine, a body shape resembling death, and unresponsiveness throughout the day, it is classified as epilepsy. If the face is red, eyes are wide open, the tongue protrudes and bites the lips, there is mental restlessness and shortness of breath, and the cry resembles that of a sheep, it is called Heart Epilepsy. If the face and lips are cyanotic, eyes roll upward, limbs contract and spasm, the body arches backward, and the cry resembles that of a dog, it is called Liver Epilepsy. If the face is black, eyes tremble, saliva foams, the body resembles a corpse, and the cry resembles that of a pig, it is called Kidney Epilepsy. If the face looks like dry bone, eyes roll back showing whites, there is startle jumping and arching, head shaking, and foaming at the mouth, with a cry resembling that of a chicken, it is called Lung Epilepsy. If the complexion is sallow, eyes stare straight, abdomen is full, there is spontaneous diarrhea, limbs cannot be controlled, and the cry resembles that of an ox, it is called Spleen Epilepsy. The Five Epilepsies generally use Wu Se Wan (Five-Color Pill) as the main formula, supplemented by herbs corresponding to each meridian.
For Heart Epilepsy due to Blood deficiency, use Yang Xin Tang (Nourish the Heart Decoction). For excess heat manifesting as fever and desire for cold drinks, use Hu Jing Wan (Tiger Eye Pill). For deficient heat manifesting as fever and desire for warm drinks, use Miao Xiang San (Miraculous Fragrance Powder). For Liver Epilepsy, if it is a deficiency syndrome, use Di Huang Wan (Rehmannia Pill); if spasms are forceful indicating excess pathogen, use Chai Hu Qing Gan San (Bupleurum Clearing Liver Powder); if constipation is present, use Xie Qing Wan (Drain Green Pill). For Kidney Epilepsy, use Di Huang Wan; ... [text gap] ... use Wu Wei Yi Gong San (Five-Ingredient Extraordinary Efficacy Powder); if the face is red, it indicates Yin fire rushing up to the Lung, use Di Huang Wan. For Spleen Epilepsy, use Wu Wei Yi Gong San; if there is cyanosis, diarrhea, poor appetite, and reluctance to eat, use Liu Jun Zi Tang (Six-Gentlemen Decoction) added with Mu Xiang (Aucklandiae Radix) and Chai Hu (Bupleuri Radix). If there is fever, convulsions, supine posture, lustrous complexion, and floating pulse, the disease is in the Fu organs (Yang) and is easy to treat. If the body is cold, no convulsions, prone posture, dark complexion, and deep pulse, the disease is in the Zang organs (Yin) and is difficult to treat. For any such symptoms, first examine the area between the high bone behind the ear. If blue vein patterns appear beforehand, pricking them to bleed can prevent the onset. These are all conditions of insufficient Yuan Qi (Original Qi). Treatment must focus on Zi He Che (Placenta Hominis) pills, assisted by tonic herbs. If one indiscriminately uses attacking/draining methods, further damaging the Original Qi, attacks will occur frequently, eventually becoming critical and often incurable. There are also three types: Fright Wind, Food Epilepsy, etc., detailed later, still treating according to Fright Wind and Fetal Wind protocols.
Clinical Cases:
An elderly man had a son just turning one year old. Early autumn brought sudden cold, causing sudden convulsions resembling fright epilepsy. After the episode, his breathing was faint. This was caused by weak Original Qi; [formula name truncated in text, likely Bu Zhong Yi Qi Tang].
Two children, aged ten and seven respectively, both suffered from epilepsy, occurring twice a year. Later, due to smallpox and food stagnation, attacks became frequent. Used Liu Jun Zi Tang... [truncated].
A child suffered from the aforementioned condition, vomiting phlegm and feeling exhausted for half a day before recovering after each attack; various medicines were ineffective. By age thirteen, attacks became frequent. Used Zi He Che (Placenta Hominis) raw and ground into paste, mixed with Ginseng and Angelica sinensis powder, formed into pills the size of sesame seeds. Each dose was 30-50 pills, taken 3-5 times daily, dissolved in milk. Gradually improved over one month, then cured with Ba Zhen Tang (Eight-Treasure Decoction) as an adjunct.
A seven-year-old child developed fright epilepsy. My father instructed him to drink human breast milk freely. Subsequent attacks became less frequent and milder. At age fourteen, it recurred; human milk was used again but was ineffective. I instructed using thick Placenta Hominis ground into paste, mixed with breast milk to form a mud-like consistency, taken 2-3 times daily until cured. Later, Jia Jian Ba Wei Wan (Modified Eight-Ingredient Pill) was commonly used for stability. At age twenty-three, it recurred with cold extremities; the previous method was used again, assisted by Ba Wei Wan and Shi Quan Da Bu Tang (Ten Complete Great Tonifying Decoction) for recovery.
Wu Xian Wan (Five-Epilepsy Pill): Treats various epilepsies.
Xiong Huang (Realgar), Zhen Zhu (Pearl) (each 1 liang, finely ground), Zhu Sha (Cinnabar, water-triturated, 0.5 liang), Shui Yin (Mercury, 2.5 qian, mixed with 2 liang of molten lead, fried until coagulated, cooled).
Grind into powder, refine honey to make pills the size of hemp seeds. Take 2-3 pills per dose, washed down with decoction of gold and silver.
Qian Shi She Huang Wan (Qian's Snake Yellow Pill): Treats fright epilepsy caused by shock and terror, characterized by crying/wailing and trance-like states.
She Huang (True Snake Gallstone, 3 genuine pieces, fired and quenched in vinegar), Yu Jin (Curcumae Radix, 7 fen, ground together), She Xiang (Musk, add 1 spoon separately).
Grind into powder, make pills with rice paste the size of sesame seeds. Take 1-2 pills per dose, dissolve in water used for grinding knives (containing gold/silver particles).
Niu Huang Wan (Ox Gallstone Pill): Treats wind epilepsy caused by sweating and open pores allowing wind pathogen to invade deficiency, leading to confusion, convulsions, salivation, and fingers curling as if counting.
Niu Dan Nan Xing (Arisaema cum Bile), Quan Xie (Whole Scorpion, roasted), Chan Tui (Cicada Slough) (each 2.5 qian), Fang Feng (Saposhnikoviae Radix), Bai Fu Zi (Typhonii Rhizoma, raw), Tian Ma (Gastrodiae Rhizoma), Zhen Jiang Can (Staphylinidae, roasted) (each 1.5 qian), She Xiang (Musk, 0.5 zi).
