Mr. Chen Wensu said: During the entire course of smallpox and measles (Douzhen), it is not advisable to give water to drink. If mistakenly given water, after the scabs form, the scabs may fall off slowly or abscesses (Yong) may develop. If punctured with a needle, they can become ulcerated and eroded (Gan Shi), leading to difficult healing. This is because the Spleen and Stomach govern the muscles externally. Drinking too much water damages the Spleen and Stomach with dampness. Dampness combats the skin and muscles, causing a decline in body fluids. Qi and Blood cannot circulate throughout the body, condensing and failing to disperse. Thus, the scabs fall off slowly, and abscesses and swellings appear on the body.
Mr. Zhang Jiegu said: Abscesses occurring on the front of the body belong to the Hand-Yangming Meridian; those occurring on the limbs belong to the Foot-Yangming Meridian. Mr. Danxi said: Abscesses caused by smallpox are mostly due to residual toxins and blood heat. Medication should be administered based on the upper or lower location, with cooling blood as the primary method. If the stool is dry and solid, add Rhubarb (Dahuang). If there is no response, differentiation should be made according to the meridian involved, the deficiency or excess of Qi and Blood, and whether pus has formed. I believe that for these symptoms at the initial stage without pus formation, use Xianfang Huoming Yin (Immortal Life Elixir Decoction) and Garlic-Partitioned Moxibustion to dissipate them. For those about to form pus, use Xianfang Huoming Yin to resolve and promote suppuration. For those with Qi and Blood deficiency, use Bazhen Tang (Eight Treasure Decoction) to tonify and promote suppuration. For those deficient and unable to heal, use Tuoli San (Support the Interior Powder) to tonify and promote contraction. Generally, for fever, swelling, and pain with non-constipated stools, use Xianfang Huoming Yin and Garlic-Partitioned Moxibustion. For constipation, use Xianfang Huoming Yin plus Rhubarb. Once the stool is unblocked but swelling and pain persist, administer another dose of Xianfang Huoming Yin, followed by Tuoli San to tonify Yuan Qi (Original Qi). For fever and fatigue with normal stools, use Buzhong Yiqi Tang (Center-Supplementing Qi-Boosting Decoction); if ineffective, also use Garlic-Partitioned Moxibustion. For post-ulceration fever, dry mouth, and limb fatigue, use Dongyuan Shengyu Tang (Dongyuan's Holy Salvation Decoction). For dripping pus and intermittent fever, use Sijunzi Tang (Four Gentlemen Decoction) plus Ginseng and Astragalus. If caused by the nursing mother's Liver Meridian Blood deficiency and fever, use Jiawei Xiaochaihu Tang (Modified Minor Blue Green Dragon Decoction). If caused by Liver and Spleen depression and anger leading to fever, use Jiawei Guipi Tang (Modified Heart-Spleen Tonifying Decoction). If caused by accumulation of heat from rich foods, use Jiawei Qingwei San (Modified Stomach-Clearing Powder). If only cooling blood agents like Baidu drugs are used, it will further damage Yuan Qi, inevitably resulting in cases that should suppurate failing to do so, and those already suppurated failing to heal.
A child suffered from this on the inner side of the leg, with severe pain, thirst, and constipation, accompanied by abdominal distension. This indicated residual toxins remaining in both the exterior and interior. One dose of Xianfang Huoming Yin plus Mangxiao (Mirabilite) and Dahuang was administered, causing all symptoms to subside immediately. Mangxiao and Dahuang were then removed, and two more doses were given, leading to recovery.
A child suffered from severe pain near the left hypochondrium close to the abdomen, with aversion to cold, fever, and limb pain. This was residual toxin combined with external pathogens. One dose of Xianfang Huoming Yin plus Ephedrae Herba (Mahuang) was administered, completely resolving the external pathogens and reducing the toxin. The previous formula minus Mahuang and Shengyu Tang were then used for recovery.
A child had red, swollen, and painful lesions. Both internal and external treatments used cold-natured medicines. One dose of Xianfang Huoming Yin stopped the pain immediately, but the swelling remained. This was due to blood coagulation caused by cold medicines. Liuwei Huoxue San (Six-Ingredient Blood-Invigorating Powder) and Garlic-Partitioned Moxibustion were used for recovery.
A child had very dense smallpox eruptions, initially developing swelling in the four limbs. I diagnosed this as Heat-Toxin of the Spleen Meridian and cured it with Xianfang Huoming Yin-like formulas. Later, the child developed oral ulcers (Kou Gan) with salivation and gum erosion. Two doses each of Da Wuyi (Large Chelidonium) and Xianfang Huoming Yin were used, followed by Chanwu Wan (Toad Pill) applied with Renzhongbai San (Human Tooth Ash Powder) for recovery.
A child had a red, swollen, and painful arm lesion, indicating impending pus formation. Two doses of Tuoli Xiaodu San (Interior-Supporting Toxin-Resolving Powder) led to pus formation, and two more doses led to rupture. Tuoli San was used for healing, but fever and aversion to cold appeared. Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction) was used for recovery.
A child had swelling in both legs, arms, and knees, unable to move and in pain. Soybean powder mixed with hot water was applied externally, and Xianfang Huoming Yin was taken internally, leading to resolution.
A child had ulcerated smallpox toxin with persistent swelling, irritability, thirst, urination resembling strangury, and picking at empty air. This was Liver and Spleen deficiency heat. Two doses each of Bazhen Tang and Jiawei Bawei Wan (Modified Eight-Ingredient Pill) stabilized the condition, followed by Dabu Tang for recovery.
A child had swelling in the legs and knees due to smallpox toxin, caused by Spleen and Kidney deficiency with toxin flow. Qiyi Tang (Qi-Boosting Decoction) and Dihuang Wan (Rehmannia Pill) were then used for recovery.
A child had ulcerated leg and knee swelling with continuous pus discharge, afternoon fever, and fatigue. My father attributed this to re-injury of Yuan Qi due to Yin deficiency. Buyin Bazhen Tang (Yin-Nourishing Eight Treasure Decoction) and Dihuang Wan were used for recovery.
A child had smallpox toxin that ulcerated internally due to cold-natured topical applications, with excessive thin pus. This indicated Yuan Qi deficiency. Qiyi Tang was taken in the morning and Bazhen Tang in the evening, over fifty doses each, assisted by Douchi Bing (Fermented Soybean Cake), leading to recovery.
A child had facial toxin. After Xianfang Huoming Yin reduced swelling and pain, extreme limb fatigue indicated that pathogens were gone but Yuan Qi was deficient. Shengyu Tang slightly restored Yuan Qi, and Tuoli San led to recovery.
A child had red, swollen, and painful lesions extending from below the left ear to the neck, diagnosed as Fire Syndrome of the Shaoyang Gallbladder Meridian. Zhizi Qinggan Tang (Gardenia Clearing Liver Decoction) was used for the mother, and Xianfang Huoming Yin for the child, leading to recovery. Upon recurrence, Baidu San (Toxin-Resolving Powder) was mistakenly taken, causing ulceration without healing and white wound edges. Tuoli San was used for recovery.
Seven days after smallpox eruption, a child had alternating chills and fever with thirst, and smallpox carbuncles (Ding) on the hypochondrium and outer arm, belonging to the Gallbladder Meridian. Black blood was pricked out, and one dose of Xiaochaihu Tang plus Shengdi Huang (Rehmannia Root) stopped fever and thirst. Xianfang Huoming Yin was then used for recovery.
A child had facial swelling with normal flesh color, floating and large pulse, fine upon pressure. I diagnosed this as Yuan Qi deficiency with pathogens following. Tonifying Yuan Qi was recommended. The family disagreed, administering Xijiao Wan (Rhinoceros Horn Pill) and Huadu Wan (Toxin-Resolving Pill), resulting in death.

Commentary: Huadu Dan is a cooling, blood-cooling, detoxifying, and cold-natured agent. Xiaodu Wan is a clearing-heat, detoxifying, and blood-breaking agent. Children's organs are delicate, and Yuan Qi is easily injured. Post-smallpox, Qi and Blood are both deficient; how can they withstand such agents? If Stomach Qi is damaged, unformed toxins won't dissipate, formed toxins won't suppurate, and ulcerated wounds won't generate flesh. Smallpox originates from Zang-Fu organs, turning whole-body blood into pus. Recurrence after healing indicates Spleen-Stomach weakness, muscle wasting, and shortness of Ying and Wei. Physicians must examine carefully!
