In cases of traumatic injury, if the affected area is red, swollen, painful, and bleeding does not stop, this indicates internal heat in the Liver and Heart; use Chaihu Zhizi San (Bupleurum and Gardenia Powder). If the area is pale, painless, but bleeding persists, this indicates deficiency of Qi in the Spleen and Lung; use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). If there is diffuse swelling that does not subside, this indicates weakness of Yuan Qi (Primary Qi); use Wuwei Yigong San (Five-Ingredient Powder for Extraordinary Merit). If the swelling is dark and does not dissipate, this indicates stagnation of blood stasis; use Jia Wei Xiaoyao San (Modified Free and Easy Wanderer Powder). If the muscles are painful with excessive bleeding accompanied by restlessness and fever, this indicates "blood loss leading to floating Yang and restlessness"; use Dushen Tang (Single Ginseng Decoction). If restlessness and anxiety arise due to blood loss, this indicates injury to both Ying (Nutritive) and Wei (Defensive) Qi; use Bazhen Tang (Eight-Treasure Decoction) added with Chai Hu (Bupleuri Radix) and Mudan Pi (Moutan Cortex). If pain persists for a long time, it suggests impending suppuration; use Tuoli San (Support the Interior Powder). If pressing the swelling with a finger causes it to rebound quickly, pus has formed; it should be lanced and drained. If pain worsens after pus discharge, this indicates internal deficiency of Qi and Blood; use Shiquan Dabu Tang (Perfect Ten Great Tonifying Decoction). If a dislocated bone reconnects but dislocates again, this indicates weakness of the Liver and Kidney; use Dihuang Wan (Rehmannia Pill). If other symptoms coexist, refer to the respective chapters for treatment.
A child suffered an arm injury with bleeding and pain, presenting with a bluish-red complexion. This was caused by Liver Fire stirring due to fright. Initially, Chaihu Qinggan San (Bupleurum Liver-Clearing Powder) was used. After bleeding stopped and pain subsided, Tuoli Xiaodu San (Interior-Supporting Detoxification Powder) was administered, leading to gradual ulceration of the affected area. Finally, Tuoli San was used, resulting in recovery. Later, due to the mother's consumption of rich, greasy foods, the child developed aversion to cold, fever, swelling, pain, and redness. Misadministration of cooling/detoxifying herbs led to trismus (lockjaw), salivation, and cold extremities. I diagnosed this as secondary damage to the Spleen and Stomach manifesting as deficiency-cold. Six Junzi Tang (Six Gentlemen Decoction) added with Sheng Jiang (Zingiberis Rhizoma Recens) and Rou Gui (Cinnamomi Cortex) was given for several doses, gradually restoring Yuan Qi. Subsequently, Wuwei Yigong San was used, and after more than a month, the wound healed.
A child suffered an excessive finger injury with bleeding, leading to coma, trismus, flaccid hands, and occasional convulsions. This indicated Qi deficiency and Blood collapse. Dushen Tang was administered for several doses, stabilizing the condition. Recovery followed with Wuwei Yigong San and Tuoli San.
A child had a foot injury resulting in ulceration and necrosis with pale flesh that failed to heal. This was due to Spleen and Stomach deficiency, preventing Qi and Blood from reaching the affected area. Wuwei Yigong San was first used to support Stomach Qi, followed by Shiquan Dabu Tang to nourish Ying Blood. The child recovered after over a month.
A child suffered a foot injury with internal suppuration, poor appetite, and nausea, indicating Spleen-Stomach Qi deficiency generating Phlegm. Liujunzi Tang (Six Gentlemen Decoction) was used, immediately improving appetite and promoting pus drainage. However, fatigue and afternoon tidal fever persisted. Bu Zhong Yi Qi Tang was taken in the morning, Wuwei Yigong San in the evening, with intermittent Bazhen Tang. The condition improved. Later, improper diet caused fever, extensive ulceration with pus, trismus, tremors, and salivation. I diagnosed Stomach Qi deficiency with internal Liver Fire. Dushen Tang was applied externally for four doses. Bu Zhong Yi Qi Tang was taken in the morning, and Wuwei Yigong San added with Chai Hu and Sheng Ma in the evening. As Yuan Qi recovered, Tuoli San was added to promote healing.
A child’s finger injury was treated with cooling herbs, causing swelling to spread to the dorsum of the hand with thin pus and poor appetite. This indicated Qi and Blood deficiency. Yigong San was taken in the morning, Tuoli San in the evening. The pus thickened, and the area became ruddy. Recovery occurred with Bazhen Tang.
A child suffered a leg injury with persistent cyanotic swelling and sallow complexion. The family insisted on Qi-moving and Blood-breaking treatments. I diagnosed this as secondary Spleen Qi injury causing Blood stasis. Ignoring my advice, they used Blood-breaking formulas, leading to anorexia and alternating chills and fever resembling malaria. I prescribed Bu Zhong Yi Qi Tang in the morning, Bazhen Tang in the evening, along with Cong Yun (Scallion Moxibustion/Fomentation). The child recovered.
A child with an arm injury presented with ulceration and alternating chills and fever. Bazhen Tang gradually resolved the symptoms. Later, dietary injury caused persistent vomiting and diarrhea, head shaking, and teeth grinding, indicating Spleen Qi deficiency with Liver Pathogen invasion. Liujunzi Tang added with Sheng Ma and Chai Hu stabilized the condition. Recovery followed with Shiquan Dabu Tang and Liujunzi Tang.
A child twisted their ankle, presenting with swelling and pain but normal skin color. This indicated Yang Qi deficiency with bone injury. Frequent Cong Yun was used. He Xue Ding Tong Wan (Blood-Nourishing Pain-Relieving Pill) was taken at dawn, and Bazhen Tang during the day. After several days, Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) was added. Recovery took over three months.
A child with a fractured and dislocated arm, once reduced, presented with swelling, pain, and fever. Cong Yun and Yigong San added with Chai Hu, Xu Duan (Dipsaci Radix), and Gu Sui Bu (Drynariae Rhizoma) were used for four doses. Bu Zhong Yi Qi Tang completed the cure.
A child with an ankle sprain and bone pain, with normal skin color, received frequent scallion fomentation. He Xue Ding Tong Wan was taken at dawn, and Sizijunzi Tang (Four Gentlemen Decoction) with Chuan Xiong (Ligustici Chuanxiong Rhizoma) and Dang Gui (Angelicae Sinensis Radix) during the day. Dihuang Wan was later used. Recovery took over three months. Since the Kidney governs bones, Dihuang Wan was used to tonify the Kidney.
A child with a broken arm and excessive bleeding presented with restlessness, thirst, red face, and a surging, rapid pulse that was empty upon pressure. This indicated "blood loss leading to floating Yang and restlessness." Danggui Buxue Tang (Angelica Tonifying Blood Decoction) stabilized the condition. Orthopedic reduction was performed, and Jiegu Dan (Bone-Connecting Pill) was taken. The next day, the child woke startled, indicating residual Blood deficiency. Since Blood is generated by Qi, Wuwei Yigong San added with Chai Hu, Sheng Ma, and Dang Gui was used. Later, mild convulsions of limbs and twitching of eyebrows/lips indicated Blood deficiency with internal Liver Fire. Sizijunzi Tang, Chuan Xiong, Dang Gui, Gou Teng Uncinariae Ramulus cum Hooka, and Chai Hu gradually improved the condition. Tuoli San and Bazhen Tang completed the cure. Using Siwu Tang (Four-Substance Decoction) in such cases would further injure Spleen Qi, often leading to death. Misuse of Baihu Tang (White Tiger Decoction) accelerates danger.
A child with a fractured and dislocated arm, once reduced, presented with swelling, pain, fever, and anorexia. Cong Yun stopped the pain. Liujunzi Tang, Huang Qi (Astragali Radix), Chai Hu, Jie Geng (Platycodonis Radix), Xu Duan, and Gu Sui Bu improved appetite and reduced swelling. Bu Zhong Yi Qi Tang added with Maidong (Ophiopogonis Radix) and Wuweizi (Schisandrae Fructus) cleared the fever. Recovery followed.
A fifteen-year-old child suffered a leg injury with internal suppuration of foul pus and full manifestation of deficiency signs. Large tonics gradually improved the condition. Labor induced flaccid limbs, closed eyes, and profuse sweating. Hot mugwort fomentation was applied frequently to the umbilicus and Qihai (CV6). A decoction of Ren Shen (Ginseng Radix et Rhizoma) 4 liang and Pao Fuzi (Prepared Aconiti Lateralis Radix) 5 qian was slowly administered. After some time, arm movement returned. A second dose opened the eyes and restored speech, though Qi remained unconnected. A formula of Ren Shen, Huang Qi, Dang Gui, and Bai Zhu (8 liang total) plus Fuzi (3 qian) was taken twice, slightly restoring Qi. Fuzi was then reduced, and three more doses gradually restored Yuan Qi. Dushen Tang was subsequently taken in large amounts for full recovery.
A woman twisted her right arm, experiencing fever and pain during Yin-Mao hours (3-7 AM). I diagnosed Gallbladder Channel Blood deficiency with excess Fire. Siwu Tang combined with Xiao Chaihu Tang (Minor Bupleurum Decoction) for four doses cleared the fever. Siwu Tang added with Xiang Fu (Cyperi Rhizoma), Chen Pi (Citri Reticulatae Pericarpium), Bai Zhu (Atractylodis Macrocephalae Rhizoma), and Fu Ling (Poria) was used. Zhi Zi (Gardeniae Fructus) 5 fen, Huang Qin (Scutellariae Radix), Lian Zi (Coptidis Rhizoma), and Gan Cao (Glycyrrhizae Radix et Rhizoma) 3 fen each were taken for over twenty doses, resolving swelling and curing the condition.
A child with a twisted arm presented with swelling, pain, fever, aversion to cold, and poor appetite. I diagnosed Spleen-Stomach Qi deficiency with congestion/swelling. Bu Zhong Yi Qi Tang was taken in the morning, Wuwei Yigong San in the evening, with intermittent Bazhen Tang. Recovery occurred after three months as form and Qi filled out.
A child fell and injured the shin, presenting with diffuse swelling and pain but normal skin color. Administration of Qi-moving and Blood-breaking herbs increased abdominal pain, which relieved slightly upon pressure. I diagnosed this as Spleen-Stomach weakness exacerbated by inappropriate Blood-breaking herbs, not blood stasis. Soon, the swelling subsided, color turned dark, anorexia set in, abdominal pain worsened, and extremities became cold. Ren Shen (1 liang) and Fuzi (1 qian) for several doses restored the Spleen and Stomach. Appetite improved, and the shin swelling returned with redness. Initially, insufficient Yuan Qi failed to circulate, causing subsidence and darkness. Post-medication, Yuan Qi replenished, causing swelling and redness. Da Bu Tang (Great Tonifying Decoction) and Tuoli San were used for over three months for recovery.
A child with a twisted arm presented with swelling, pain, and a pale/ashen face. Insisting on Liuyin Yin (Flowing Qi Drink) worsened the condition. I stated this was a case of both Form and Disease being deficient, warning against previous medications. The family argued that swelling/pain resulted from Qi stagnation and Blood congealing, requiring Liuyin Yin to dredge collaterals and Blood-breaking herbs to dissipate obstruction. Ignoring my warnings, the condition worsened with fever and restlessness. Upon seeking treatment, I diagnosed exhausted Yuan Qi with the emergence of the "Seven Evils" (ominous signs). Even Lu Bian (Lu Yu and Bian Que) could not save him. The child died.
A child with arm injury/swelling used internal and external cooling analgesics. Half a year later, ulceration and swelling/pain appeared. I stated that only Interior-Supporting and Middle-Warming methods could generate tissue. Unheeding, the family persisted with cooling herbs, leading to exhaustion of Qi and Blood and death.
A child with a leg fall injury presented with cyanotic swelling. All administered herbs moved Qi and broke Blood.
