(For children under twenty years of age, the condition is Gan accumulation; for those over twenty, it is Lao exhaustion.)
Brain Gan (Gan syndrome affecting the brain): The scalp is shiny and tight, hair forms tufts or spikes. There may be sores or swelling extending to the fontanelle; if the fontanelle swells, it often damages the eyes. The head feels hot like fire, the fontanelle bulges, and there is profuse sweating all over the body.
Spine Gan (Gan syndrome affecting the spine): Parasites erode the spinal bones like saw teeth; tapping the back produces a drum-like sound. Sores appear on the backs of the ten fingers, the patient frequently bites their fingernails, accompanied by irritability, fever, sallow complexion, and emaciation.
Zouma Gan (Running Horse Gan): This condition erodes the gums, causes bleeding from mouth ulcers, foul breath, blackening and loss of teeth. If the fontanelle presents with a pit, it is named "Zouma" (Running Horse), attributed to the surging heat of the Yangming meridian resembling a galloping horse.
Wugu Gan (Innocent Gan): A mass appears behind the ear/head. Initially soft and painless, it contains parasites resembling rice flour. If not promptly ruptured and removed, the pathogenic heat gradually increases, and the parasites disperse via Qi and Blood, damaging the Zang-Fu organs. This leads to sallow complexion, emaciation, a large head, and weak limbs, eventually resulting in death. (Ancient texts mention a bird named "Wugu" that hides during the day and moves at night. If infants' laundry is left exposed overnight and contaminated by feathers from this bird flying overhead, wearing such clothes can cause this disease.)
Dingxi Gan: Abdominal distension with a thin neck, sallow complexion, and emaciation characterize this condition. "Ding" refers to the extreme thinness of the limbs and neck; "Xi" refers to the distended abdomen. In severe cases, the sacrum protrudes, muscles waste away, the umbilicus bulges, and the chest feels full. The patient may crave eating raw rice, soil, or charcoal.
Pulu Gan: Alternating deficiency-heat, splitting of the cranial sutures, pica (eating earthworms), irritability, thirst, vomiting, and retching occur. The bones are emaciated, leaving only skin covering them. Generally, all Gan syndromes arise from long-term Spleen and Stomach deficiency leading to physical wasting. All these conditions are suitable for treatment with Shiquan Dan.
Shiquan Dan (Ten Perfection Pill)
Chenpi (Aged Tangerine Peel), Qingpi (Green Tangerine Peel), Pengzhu (Curcuma Phaeocaulis) [Note: This herb is too harsh; replace with Xuanhusuo (Corydalis Rhizome)]
Chuanxiong (Ligusticum Rhizome), Wulingzhi (Trogopterus Dung), Baidoukou (White Cardamom), Binglang (Areca Seed), Lushen (Aloe) [each 5 qian]
Muxiang (Costus Root), Shijunzi (Quinine Tree Fruit) [each 2 qian]
Grind the above into powder. Soak steamed cake in pig bile, mix with the powder to form pills the size of hemp seeds. Take 20–30 pills with rice water.
If deficiency is severe, add 1 qian of Renshen (Ginseng).
For Cold Gan, use Zhisheng Wan (Sage Pill) as the primary treatment, which also manages diarrhea and dysentery associated with various Gan syndromes.
For Heat Gan, use Wufu Huadu Dan (Five Blessings Detoxification Pill) as the primary treatment.
Fei'er Wan (Fat Child Pill) [This formula eliminates Gan, resolves accumulation, dissipates masses, clears heat, soothes the Liver, tonifies the Spleen, promotes appetite, and kills parasites.]
Huhuanglian (Picrorhiza Rhizome) [5 qian], Shijunzi (Quinine Tree Fruit) [4.5 qian], Renshen (Ginseng), Huanglian (Coptis Rhizome, stir-fried with ginger), Shenqu (Medicated Leaven), Maiya (Barley Sprout, stir-fried), Shan-zha Rou (Crataegus Flesh) [each 2.5 qian], Baizhu (Atractylodes Macrocephala Rhizome), Baifuling (Poria), Gancao (Licorice Root, honey-fried) [each 3 qian], Lushen (Aloe, roasted) [2.5 qian].
Grind into powder. Mix with yellow millet paste to form pills the size of mung beans. Take 20–30 pills with rice water.
Xiaoshi Wan (Digestion Promoting Pill) [Universally treats the Five Gans, kills parasites, reduces heat, dissolves accumulation, and promotes appetite. The "Five Gans" refer to Gan syndromes affecting the five Zang organs.]
Shijunzi (Quinine Tree Fruit), Caolongdan (Gentiana Scabra Root), Maiya (Barley Sprout, stir-fried), Chenpi (Aged Tangerine Peel), Wuyi (Toosendan Fruit), Shenqu (Medicated Leaven), Huanglian (Coptis Rhizome), Shan-zha Rou (Crataegus Flesh) [equal parts].
Mix with aged rice meal to form pills the size of millet seeds. Take 20–30 pills with rice water, or decoct into a soup.
For various Gan syndromes: Pills made from dried snake meat are most effective and have verified efficacy.
For Dental Gan: Dried meat of the field mouse ground into powder, mixed with clear liquid and applied topically, is excellent.
Another method: Applying snake bile is also effective.
